Schwellung der Stimmlippen, die vor allem bei Nikotinabusus und Stimm-Missbrauch auftritt. Das Reinke Ödem ist durch eine erniedrigten Stimmlage und schlechte Stimmqualität gekennzeichnet, was den beruflichen Alltag vor allem von Personen in Sprechberufen (Lehrer, Call Center Agenten etc.) deutlich erschwert. Bis dato gibt es keine kausale Therapie für das RÖ, auch der Goldstandard der Therapie, Stimmlippenchirurgie, bringt keine zufriedenstellenden Ergebnisse und stellt keine kausale Therapie dar.\r\nIm vorgestellten Projekt wollen wir die zellulären Ursachen des RÖ mittels Protein Analyse (Proteomics) erforschen. Damit werden erstmals die kausalen Ursachen dieser Krankheit erforscht. Aus Schleimhaut Biopsien die während Stimmlippen-OPs betroffener Patienten entnommen werden, sollen spezifische RÖ-Stimmlippenzellen kultiviert und für weitere Analysen und Versuche vorbereitet werden. Die Proteomics Analyse liefert eine Auflistung aller Proteine, die diese Zellen produzieren. Diese sollen mit den Proteinen aus Stimmlippenzellen aus gesundem Gewebe verglichen werden.\r\nIn darauffolgenden funktionellen Experimenten sollen bestimmte Proteine („Kandidaten-Proteine“), die vor allem von RÖ-Fibroblasten produziert werden, auf ihre biologische Relevanz überprüft werden. Mittels Verstärkung oder Blockierung dieser Kandidaten Proteine können wesentliche Rückschlüsse auf die Krankheitsmechanismen von RÖ gezogen werden, die den Weg zu neuen (nicht-chirurgischen) Therapieformen ebnen sollen.\r\n", "en": "Reinke’s edema (RE) is a uni- or bilateral swelling of the vocal folds, occurring in patients with nicotine and voice abuse. Voice of patients is characterized by decreased pitch and detrimental voice quality, making it often difficult for voice professionals who rely on their voices (teacher, call center agents etc.)\r\nSo far treatment is only symptomatic, still the gold standard of therapy, microsurgery of the vocal folds, is far from being satisfactory. \r\nIn the proposed project we seek to explore the cellular mechanisms of RE by protein analysis (proteomics), revealing the true pathophysiology of RE for the first time. During routine vocal fold surgery for RE we will take small mucosa specimens. From these we will extract vocal fold fibroblasts for subsequent analysis. Proteomic analysis will deliver a complete list of proteins produces specifically by these cells. These will be compared with vocal fold cells grown out of healthy laryngeal mucosa. In subsequent functional experiments we will explore certain proteins of interest (aka. candidate proteins) that are produced in a high number by RE fibroblasts for their biological relevance. The tool supports the physicians in all treatment stages (diagnosis, planning, intervention and monitoring) and can also be used during clinical traineeship. A key point is the development of an algorithm for the semi-automatic and automatic segmentation of bone structures and soft tissue in CT and MRI acquisitions. A segmentation enables a three-dimensional localization, quantification and visualization of biological structures in a very short time. Based on the radiological images from the clinical routine, treatment diagnosis can be visualized, and surgical planning processes or alternative surgery options can be simulated. In addition, the postoperative simulation results can be presented preoperatively in a photo-realistic manner. In particular, this will be applied in the reposition of bony structures, the reconstruction of facial defects and the removal of tumors in complex anatomical areas. Moreover, we plan the development of patient-individual three-dimensional implants, which will 3D printed in-house for comparison with the external manufactured ones and for clinical usage. Furthermore, the medical personnel can be supported in real-time during surgery by an interactive navigation, where Augmented Reality integrates computer-generated objects into the operation field. The application reduces surgical complications and ensures a successful treatment result by pre- and intraoperative simulations. Hence, the number of necessary corrective surgeries and the operation time needed can be reduced, and a higher patient survival rate can be achieved. In addition, the combination with Virtual Reality glasses allows the virtual training of operations during medical education. The objective of the project is the establishment of an open source tool as basis for further research and developments for the participating universities. Currently, comparable tools are protected by strict licensing conditions and not accessible without restrictions for academic research. Existing software for clinical practice is not functionally stable enough and has too high error rates. Additionally, the usage imposes high financial costs, which makes a widespread application not possible.
