Hohes Alter, Komorbiditäten sowie ein vorangeschrittener Krankheitsverlauf bei oftmals spätem Diagnosezeitpunkt limitieren die isolierte chirurgische Therapie, sodass diese häufig nur im Rahmen kombinierter Operationsverfahren (z.B. bei gleichzeitiger Bypass-Operation) Anwendung findet. Lange Zeit stellte daher die alleinige symptomatische medikamentöse Behandlung bei höhergradiger isolierter sekundärer Trikuspidalinsuffizienz, trotz schlechtem Outcome, die einzige Therapieoption dar. Neue Möglichkeiten bietet der interventionelle Therapieansatz, wobei bereits mehrere Device-Systeme Anwendung finden, insbesondere der Transcatheter-Edge-to-Edge-Repair (TEER). Der zentrale Pathomechanismus der funktionellen oder sekundären Trikuspidalinsuffizienz ist die Dilatation des Klappenannulus. Als katheterbasiertes Verfahren zielt deshalb das Cardioband-System, ähnlich der chirurgischen Annuloplastie, auf eine Raffung des Klappenringes, und somit Verringerung des Annulusdurchmessers, ab.
