Wie gut eine Polymerisationslampe Komposite aushärtet, hängt nicht nur von der Lichtintensität der Lampe ab. Die Literatur vergleicht in unzähligen Studien Halogen und LED-Lampen bei der Polymerisation von Kompositen verschiedenster Art. Vergleicht man diese Studien, so kommt man zum Erkenntnis, dass es keine signifikanten Unterschiede zwischen den einzelnen Lampensystemen gibt. Sehr wohl wird aber in der Li9teratur immer wieder hingewiesen, dass die Vollständigkeit der Polymerisation von der Struktur des Komposits und von der Farbe abhängig ist. So müssen dunklere Komposite, egal ob Halogen oder LED-Lampe, länger als gleichnamige hellere Komposite ausgehärtet werden. Das Gleiche gilt auch für Mikrofüllerkomposite, die durch ihre kleinen Partikel die Strahlung streuen, und deshalb länger der Polymerisation ausgesetzt werden müssen.Beim Kauf eines Lichtpolymerisationsgerätes sollte auch darauf geachtet werden, welche Komposite in der Praxis verwendet werden, und welcher Initiator dem Komposit beigemischt ist. Die beste LED-Lampe mit dem neuesten Soft-Startmodus hilft dem Behandler überhaupt nichts, wenn der ausgestrahlte Wellenlängenbereich nicht mit dem Initiator übereinstimmt. Halogenlampen strahlen ein breites Spektrum an Wellenlängen aus, und können deshalb auch für den Initiator Lucirin verwendet werden. Der größte Nachteil der Halogenlampe ist, dass sie sehr viel Wärme entwickelt, deshalb wird sie in der Literatur vielfach als Ofen bezeichnet, und dem Leser klargemacht, dass LED-Lampen mit derselben Strahlungsintensität Komposite gleich gut aushärten. Zusammenfassend ist zu diesem komplexem Thema zu sagen, dass LED und Halogenlampen mit einer Lichtintensität von 400 bis 1500 mW/cm2 benutzt werden können. Wichtig dabei ist noch, dass Initiatorsystem und ausgestrahlter Wellenlängenbereich übereinstimmen. Die Adhäsivtechnik erzielt eine gute Randschlussqualität im Schmelz, und die zusätzliche Applikation eines Dentinadhäsives führt zu verbesserter marginaler Adaption im Dentin. (Klinische Studie).", "abstract": "Meine Diplomarbeit hat zum Ziel, anhand einer klinischen Studie die Wirkungen und Nebenwirkungen des sogenannten In-Office-Bleaching herauszufiltern. Man versteht darunter die professionelle Methode des Zahnbleichens, die nur vom Zahnarzt durchgeführt werden kann.\r\nIn den beginnenden ausführlichen Behandlung der theoretischen Grundlagen beschäftigt sich die Arbeit mit Zahnverfärbungen und deren Ursachen an sich, mit der Klassifikation und Beschreibung der verschiedenen Bleichsysteme, sowie mit den Wirkungen, Nebenwirkungen und der Prognose des Zähnebleichens.\r\nIm Anschluss daran wird durchgeführte klinische Studie vorgestellt: Bei 17 Probanden wurden nach sorgfältiger Anamnese und professioneller Zahnreinigung, sowie fortlaufender Fotodokumentation des Bleicheffektes bzw. auch der auftretenden Nebenwirkungen, zwei In-Office-Schienenbleichungen der Oberkieferfront für die Dauer von 40 Minuten durchgeführt. Es kam das Produkt Illumin, Professionelles Aufhellungssystem für die Praxis, von der Firma Dentsply Detrey zur Anwendung. Der Effekt wurde anhand des Vitafarbschlüssels an den Referenzzähnen 12 und 13 dokumentiert, die untersuchten Nebenwirkungen (Gingivairritationen und Hypersensibilitäten) wurden nach einem Punktesystem bewertet.\r\nDaraufhin folgt die Verdichtung der Ergebnisse sowie deren statistische Auswertung: Ein deutlicher Aufhellungseffekt an beiden Referenzzähnen konnte nachgewiesen werden. Dazu gehören für viele auch ansprechende weiße Zähne, da diese zu einem Symbol für Jugend geworden sind und ein sympathisches Erscheinungsbild versprechen. Da jedoch viele unterschiedliche Mittel am Markt sind und diese sowie deren Wirkungen für die Patienten längst nicht mehr zu überblicken sind war e das Ziel der Arbeit, ein neues, für die Anwendung zu Hause gedachtes und einfach handhabbares Produkt klinisch auf Wirksamkeit und eventuelle Nebenwirkungen zu testen.\r\n\r\nMaterial und Methode: Im November 2003 wurden bei insgesamt 18 Patienten an zwei aufeinander folgenden Terminen im Abstand von einer Woche die Bleichwirkung sowie die unerwünschten Wirkungen eines carbamidperoxidhaltigen Paint-On-Produkts zur Heimanwendung mittels Farbbestimmung mit dem Vita-Farbschlüssel und Befragung der Probanden ermittelt. Auf Schutzmaßnahmen für die Gingiva wurde verzichtet. Präexistente Zahnhalshypersensibilitäten waren Ausschlusskriterien, Gingivairritationen und der Genussmittelkonsum der Teilnehmer wurden berücksichtigt. Die Daten wurden analysiert und mit Angaben aus Literatur und anderen Studien verglichen.\r\n\r\nErgebnisse: Im Durchschnitt kam es zu einer Aufhellung von 4,2 Vita-Farbstufen, wobei das Ausmaß der Bleichwirkung nicht mit dem Genussmittelkonsum in Relation zu bringen war. In 6 Fällen bestanden posttherapeutisch leichte Gingivairritationen, wovon 2 präexistent waren. In 3 Fällen ließen sich leichte Hypersensibilitäten nachweisen.\r\n\r\nSchlussfolgerung: Die Aufhellungswirkung des Mittels erwies sich als sehr gut, dabei blieb die Nebenwirkungsrate relativ gering und die Folgen sind im Allgemeinen gut beherrschbar. Dem Patienten wird somit ein gut wirksames und einfach anzuwendendes Produkt geboten, das aber- aufgrund der möglichen Nebenwirkungen und des mangelnden Wissens der Patienten über Indikationen und Kontraindikationen - nur unter ärztlicher Aufsicht angewendet werden sollte.
