            "abstract": "Durch autoimmun bedingte Destruktion von Beta-Zellen des Pankreas leiden Menschen mit Diabetes Mellitus Typ 1 (DM-1) unter einem absoluten Insulinmangel. Derzeit verfügbare Therapien versuchen die Physiologie des Glukosestoffwechsels durch exogene Zufuhr von Insulin möglichst nachzuahmen. Neben der Entwicklung von Insulinen mit unterschiedlicher Pharmakokinetik und Dynamik, zeigten sich im letzten Jahrzehnt große Fortschritte in der Diabetestechnologie. Sowohl betroffene Kinder als auch Erwachsene profitieren in ihrem alltäglichen Leben sehr von kontinuierlichen Glukosemonitoring Sensoren (CGM), Insulinpumpen und zuletzt auch von sogenannten automatisierten Insulinabgabesystemen (Automated Insulin Delivery Systems/ AID). Diese ermöglichen durch Kombination einer Insulinpumpe mit einem CGM-Sensor sowie einem entsprechend programmierten Algorithmus eine bedarfsbedingte Insulinangabe, angepasst an die aktuelle Sensorglukose. In den letzten Jahren kam es zur Entwicklung einer Vielzahl unterschiedlicher Systeme, wodurch deren positiver Einfluss auf das Diabetesmanagement sowie der Vermeidung von akuten und chronischen Komplikationen durch eine Vielzahl von Studien bestätigt wurde. Unabhängig davon sind nach aktueller Recherche keine direkten Vergleiche von kommerziell verfügbaren AID-Systemen verfügbar. Aufgrund dessen versuchte diese systematische Übersichtsarbeit, in Form einer Metaanalyse, derzeit verfügbare AID- Systeme anhand deren Effektivität (Zeit im Zielbereich; 70-180 mg/dL) und Sicherheit (Zeit unter dem Zielbereich; <70 mg/dL) zu vergleichen.",

            "title": "Relevance of Rhythmic Depolarizations for Cortical Information Processing - Energetic Considerations",
            "abstract": "Rhythmic depolarizations are observed in the human brain at a variety of temporal and spatial scales. Given the evolutionary pressure to optimize energy requirements, it is hypothesized that these oscillations are indeed exploited to enhance its metabolic demand. Investigating this phenomenon from an energetic perspective provides a new outlook on the topic and contributes to the understanding of neural coding. Based on an extensive literature review and the simulation of a simple probabilistic network, energy costs for specific tasks are evaluated. Phase coding of individual neurons is a compelling possible explanation for oscillatory background activity. At the network level, distributed representation of a stimulus over multiple neurons proves highly efficient for probabilistic low-cost neurons, and for the optimization of the representational capacity of a network. An additional role for oscillatory activity was proposed in the temporal segmentation of computations and highly efficient clustered processing. The identification of mechanisms at work in biological neural networks seems to be highly relevant for the development of artificial neural networks. Current neuromorphic approaches are discussed. This thesis links fundamental oscillation-based calculations in biological and artificial networks.",

In selected cases of femoral neck fractures and post-traumatic hip osteoarthritis with osteosynthesis, the anatomy may allow treatment with a short-stem THA, which has several potential advantages over a straight-stem THA. These include a more atraumatic approach to the hip, good possibilities to reconstruct the hip geometry, and less foreign material. The aim of this study is to provide evidence that the implantation of a short-stem THA is a potential alternative for young patients with athletic aspirations.",

            "abstract": "Background and Objective: In clinical studies on chronic heart failure, inhibitors of the sodium-glucose cotransporter 2 (SGLT2i) have shown improved prognosis compared to placebo. The EMMY study confirmed this relationship after an acute myocardial infarction (AMI) and demonstrated a significant reduction in N-terminal pro-brain natriuretic peptide (NT-proBNP) with empagliflozin. The aim of this thesis is to expand the evidence on the involvement of the right ventricle and systolic and diastolic myocardial dysfunction.\nMaterials and Methods: Echocardiography loops of a group of patients after acute myocardial infarction (n = 219), who received either standard treatment and placebo or standard treatment and empagliflozin as part of the EMMY study, were triple blind evaluated at three time points (week 0, 6, and 26) regarding right ventricular dimensions and performance. The obtained values were statistically compared both over time and between the groups.",

Schenkelhalsfrakturen gehören zu den häufigsten Frakturtypen bei Erwachse-nen, wobei besonders ältere Personen von den potenziell gravierenden Konsequenzen betrof-fen sind. Diese reichen von eingeschränkter Mobilität über Pflegebedürftigkeit bis hin zum Tod. Daher ist eine adäquate Therapiewahl essenziell, um das Risiko von Komplikationen zu reduzieren und eine effiziente Rehabilitation der Patient*innen zu fördern. Unsere retrospek-tive Untersuchung zielt darauf ab, die Eignung der Osteosynthese bei der Frakturversorgung zu evaluieren.",

            "title": "Arbeit 4.0\r\nAuswirkungen der Digitalisierung auf das Schlafverhalten",
            "abstract": "Ein Großteil der Beschäftigten ist in ihrem Arbeitsumfeld zunehmend mit digitalisierten Prozessen und neuen Kommunikationstechnologien konfrontiert. Diese Entwicklung begann bereits vor Jahrzehnten, wurde aber in den letzten Jahren, insbesondere bedingt durch die Covid19-Pandemie noch einmal beschleunigt. Während die gesellschaftlichen und ökonomischen Vorteile der Digitalisierung klar auf der Hand liegen, ist es auch wichtig, die Auswirkung der immer stärkeren Nutzung digitaler Technologien auf die Gesundheit der Beschäftigten zu untersuchen. Insbesondere die immer stärker verschwommenen Grenzen (Entgrenzung) zwischen Beruflichem und Privatem könnte negative Konsequenzen auf die mentale Gesundheit haben. Nachdem mehrere internationale Studien bereits auf mögliche nachteilige Auswirkungen zu vermehrter Digitalisierung auf das Stressempfinden aber auch auf die Schlafqualität der Arbeitnehmer*innen hinweisen, ist die Studienlage im deutschsprachigen Raum, insbesondere in Österreich, noch weniger dicht. Das von der Arbeiterkammer Steiermark geförderte Projekt zur Arbeit 4.0 sollte diese und andere Aspekte nun wissenschaftlich untersuchen. In der vorliegenden Arbeit wurden Daten aus dieser Erhebung analysiert. In dieser Stichprobe konnte gezeigt werden, dass Teilnehmer*innen mit höherer Digitalisierung in ihrem Arbeitsalltag mehr von Schlafproblemen (Einschlafstörungen, Durchschlafstörungen, frühes Erwachen) betroffen sind als jene mit niedrigerer Digitalisierung. Das wurde auch schon in Vorgängerstudien beschrieben.",

            "title": "Consequences of intestinal CGI-58/ATGL deficiency and ATGL overexpression on gut lipid homeostasis",
            "abstract": "Systemic lipid levels are mainly determined by dietary lipid absorption and lipoprotein secretion by enterocytes of the small intestine (SI) as well as by rapid chylomicron (CM) clearance by the liver. Excessive triglycerides (TGs), originating from apical (diet) or basolateral (circulation) lipid uptake, are transiently stored in form of cytosolic lipid droplets (cLDs). Mobilization of this intracellular lipid pool is believed to sustain peripheral lipid supply in interprandial periods, however, the underlying mechanism(s) and enzyme(s) involved are still elusive. As mice lacking adipose TG lipase (ATGL) in the gut displayed massive accumulation of cLDs, we hypothesize that the enzymatic pathway accountable for the catabolism of intestinal cLDs involves ATGL and its coactivator comparative gene identification-58 (CGI-58).\nTo prevent systemic effects of CGI-58/ATGL deficiency, we generated mice lacking both proteins exclusively in the SI (Cgi-58/Atgl iDKO). Loss of intestinal CGI-58/ATGL resulted in increased cLD accumulation within enterocytes, independent of the diet. Alimentary lipids failed to accumulate in the SI of iDKO mice in the early phase of absorption, but got incorporated into cLDs 2 h post-gavage. These findings together with persistent cLD accumulation after restriction to endogenous lipids indicated the existence of a secretion/reuptake cycle in enterocytes. In line, accumulation of intravenously applied lipids in the proximal SI of iDKO mice highlighted the role of intestinal CGI-58/ATGL in the hydrolysis of a basolaterally-derived lipid pool.\nTo further investigate the contribution of solely ATGL to intestinal and whole-body lipid homeostasis, we also generated mice overexpressing ATGL (Atgl iTg) specifically in the SI. Previous studies in Atgl iKO mice revealed a role of ATGL in cholesterol absorption rather than in dietary TG metabolism. In line, Atgl iTg mice displayed accelerated cholesterol absorption, which was mainly mediated by the activation of PPARα. Although Atgl iTg mice displayed slightly impaired secretion of CM-TGs and –cholesterol, ATGL seems to play a neglible role in intestinal lipoprotein metabolism.\nBoth studies demonstrated that lipid hydolysis in the SI is by far more complex than described in previous models, highlighting the necessity to modify the current model of intestinal lipid metabolism. We could identify CGI-58 and ATGL as critical players in the catabolism of a basolaterally-derived intestinal lipid storage pool rather than alimentary lipids. However, the enzyme responsible for the catabolism of dietary lipids still remains elusive. Identification of potential lipid hydrolases in the SI and future studies in this field should lead to the identification of new drug targets to treat elevated lipid levels in blood and intestine.",

            "title": "Biomechanical properties of Gastrocnemius muscle and  Achilles tendon",
            "abstract": "Spastic cerebral palsy, which can be divided into unilateral and bilateral subtypes, is a common neurodevelopmental disorder in childhood. However, the majority of studies that have examined the muscle-tendon properties of the lower limb muscles in individuals with spastic cerebral palsy have not distinguished between the two subtypes. One of the most significant factors influencing the functionality of spastic muscles is their morphological characteristics. Therefore, different muscle-tendon pathologies may require different treatment strategies. Accordingly, one aim of this thesis was to conduct a comparative analysis of the muscle and tendon characteristics of children with unilateral or bilateral spastic cerebral palsy. The use of valid and reliable measurement methods is critical in assessing potential differences at the musculotendinous level. Therefore, in a study performed beforehand, a 3D ultrasound approach was developed for the assessment of the gastrocnemius medialis muscle belly, muscle-tendon unit, and Achilles tendon lengths. This approach was evaluated with regard to two key aspects: firstly, its validity in comparison to magnetic resonance imaging, and secondly, its reliability for static and dynamic length measurements.\r\nIn order to evaluate the reliability and validity of the 3D ultrasound approach, two ultrasound assessment sessions and one magnetic resonance imaging examination were conducted. The tissue lengths were assessed at a fixed ankle joint position by combining 3D motion capture techniques, 2D ultrasound and vector algebra. Subsequently, the aforementioned measurements were then compared to those obtained from magnetic resonance imaging. The intra- and inter-rater reliability of the static and dynamic length assessment was evaluated using various statistical parameters including standard errors of measurement, coefficients of variation, intraclass correlation coefficients, and minimal detectable changes. A total of 33 ambulatory children were included to assess the muscle-tendon properties of the spastic cerebral palsy subtypes. Of these, 15 had unilateral and 18 had bilateral spastic cerebral palsy. Isometric muscle strength, ankle joint range of motion, and muscle-tendon properties of the gastrocnemius medialis muscle-tendon unit (e.g., muscle volume, tissue lengths and lengthening behaviour) were assessed using isokinetic dynamometry, 3D motion capture, and ultrasound, respectively. Independent t-tests or Mann-Whitney-U-tests were used to test for group differences with a level of significance set at α = 0.05. \r\nA comparison of the 3D ultrasound approach with magnetic resonance imaging revealed a slight underestimation of tissue lengths, with a difference of approximately 1.1%. Moreover, the approach demonstrated excellent intra- as well as inter-rater reliability, as indicated by high intraclass correlation coefficients (≥ 0.94), small standard errors of measurement (≤ 1.3 mm), and good values for minimal detectable change (≤ 3.6 mm). No significant differences were observed in any of the assessed parameters between children with unilateral or bilateral spastic cerebral palsy (p > 0.05, d < 0.57).\r\nThe proposed 3D ultrasound approach was found to be reliable and valid for the assessment of the gastrocnemius medialis muscle belly, muscle-tendon unit, and Achilles tendon lengths, as well as the lengthening behaviour of these tissues. It has been demonstrated that the approach can serve as a valuable measurement technique for examining both static and dynamic muscle-tendon characteristics. The functional and morphological properties of the gastrocnemius medialis muscle-tendon unit seem to be similarly developed in children with unilateral or bilateral spastic cerebral palsy.             "abstract": "Peripheral Arterial Disease (PAD) is a condition characterized by insufficient oxygen supply due to arterial vessel alterations in the extremities. From a certain stage, this leads to pain when walking, forcing patients to stop walking and thus affecting mobility and quality of life. Evidence-based guidelines recommend lifestyle changes, exercise therapy, and angioplasty for PAD management. Therefore, this study introduces a telehealth service, \"Keep pace!\", developed to address the needs of PAD patients in stage IIa and IIb, and evaluates the walking distances, patient knowledge about PAD and patient satisfaction. The 12-week telehealth program imparts their knowledge and enables participants to engage in structured ambulatory walking training using a new specially designed mobile app by instruction and monitoring progress. The geolocation tracking function of the mobile app allowed for self-paced 6-minute walk tests. Initially, nurses provided supervision via telephone calls for a period of eight weeks, after which participants were encouraged to exercise independently. App-based reminder and knowledge updates happened regularly over the whole study.",

            "title": "Überversorgung im Bereich der niedergelassenen Allgemeinmedizin in Österreich - wichtigste Maßnahmen und Möglichkeiten der Reduktion",
            "abstract": "Einleitung: In dieser Arbeit wurde der Frage der Überversorgung aus Sicht der niedergelassenen österreichischen Allgemeinmediziner*innen nachgegangen. Dabei versteht man unter Überversorgung medizinische Leistungen, die nicht indiziert sind oder keinen gesicherten Nutzen für die Patient*innen haben. \r\nMethode: Es wurden zwei offene Fragen als Teil eines Online-Fragebogens an sämtliche (insgesamt 4001) österreichische allgemeinmedizinische Ordinationen gesendet, deren Kontaktdaten im Internet frei zugänglich waren. Es erfolgte eine qualitative Auswertung der beiden offenen Fragen. In der ersten Frage wurde nach den wichtigsten überversorgenden und überdiagnostizierenden Maßnahmen im österreichischen Gesundheitssystem gefragt. Diese Frage wurde induktiv ausgewertet, indem die Antworten kategorisiert und dementsprechend zugeordnet wurden. Die zweite Frage behandelte die wichtigsten Maßnahmen, die helfen würden, um Überversorgung zu reduzieren. Diese Frage wurde deduktiv, angelehnt an das „Behaviour Change Wheel“, ausgewertet.  \r\nErgebnisse: Insgesamt wurde der Fragebogen von 331 Allgemeinmediziner*innen beantwortet. Der Großteil der Teilnehmer*innen war zwischen 40 und 49, bzw. 50-59 Jahre alt, wobei die Verteilung weiblich-männlich annähernd gleich groß war. Die erste Frage wurde 134-mal beantwortet. Den folgenden fünf Oberkategorien konnten die meisten Antworten zugeordnet werden: „Bildgebende Maßnahmen“ erhielt insgesamt 80 Antworten und wurde am häufigsten genannt. Darauf folgten die Kategorien „Labor“ mit 44 und „Medikamente“ mit 32 Nennungen. Der Oberkategorie „Untersuchungen“ konnten 29 Antworten zugeordnet werden und der Kategorie „Freier Zugang zu medizinischen Maßnahmen“ 19. \r\nDie zweite Frage wurde insgesamt 129-mal beantwortet. Folgenden Kategorien konnten die meisten Antworten zugeordnet werden; „Bildung“ wurde 31-mal, „Befähigung“ 26-mal und „Leitlinien“ 25-mal. „Kommunikation/Marketing“ 23-mal und „Ausbildung“ 15-mal genannt. \r\nDiskussion: Die in der Befragung genannten überversorgenden Maßnahmen ähneln den aus der Literatur bekannten Ergebnissen aus früheren Erhebungen in verschiedenen Ländern, so wurden auch dort häufig bildgebende Verfahren, wie auch Laboruntersuchungen genannt. Auch bei den aus Sicht der österreichischen Allgemeinmediziner*innen hilfreichen Maßnahmen zur Reduktion von Überversorgung finden sich in der durchgeführten Befragung ähnliche Ansätze, wie in anderen Ländern, wenngleich auch unterschiedliche Herangehensweisen zu erkennen sind. In der österreichischen Befragung wurden als die wichtigsten Maßnahmen die Bildung und die Befähigung gesehen. Im Gegensatz zu den USA, wo finanzielle Gründe als starker Faktor für überversorgende Maßnahmen angeführt wurden, war dieser Punkt in unserer Befragung kein Thema. In diesen Ergebnissen spiegeln sich auch die verschiedenen Gesundheitssysteme wider und so braucht es in verschiedenen Ländern mit verschiedenen Gesundheitssystemen unterschiedliche Ansätze, um eine Veränderung der Verhaltensweisen in Bezug auf Überversorgung zu bewirken.",
