Das kardio-renale Syndrom beschreibt eine wechselseitige Beeinflussung dieser eng miteinander verbundenen Organe. Dabei führt die Organdysfunktion des einen Organs zu Funktionseinschränkungen des anderen und umgekehrt, also kann entweder Herz oder Niere Ort der primären Schädigung sein. Ziel dieser Literaturarbeit ist es, nach aktuellem Stand der Forschung, die unterschiedlichen Aspekte des kardio-renale Syndroms zu beschreiben. Fokus wird vor allem auf die Pathophysiologie, sowie die Therapie und die prognostische Auswirkung des kardio-renalen Syndroms auf Patientinnen und Patienten mit Herzinsuffizienz gelegt. \r\nMethodik: Die Literaturrecherche im Zuge dieser Arbeit erfolgte via PubMed, außerdem wurden die Portale Web of Science, Google Scholar, sowie Ovid MEDLINE zur Suche nach aktuellen Studienergebnissen verwendet. Relevante wissenschaftliche Artikel, Reviews, und Fachbücher, sowie Studien und Guidelines, die in Zusammenhang mit den Themen kardio-renales Syndrom, Herz- und Niereninsuffizienz stehen, wurden als Quellen herangezogen.\r\nErgebnis: Die Entstehung dieses Syndroms ist komplex, dynamisch und basiert auf unterschiedlichsten Mechanismen der Schädigung. Dazu zählen unter anderem ein erhöhter zentraler Venendruck, eine Überaktivierung des Renin-Angiotensin-Aldosteron-Systems und des sympathischen Nervensystems, oxidativer Stress und Inflammation. Die Therapie des kardio-renalen Syndroms basiert auf den Behandlungsmöglichkeiten der Herz- und Niereninsuffizienz. Dazu zählen die Reduktion von Risikofaktoren und je nach Typ des kardio-renalen Syndroms die Therapiemöglichkeiten der Herzinsuffizienz, ein entsprechendes Flüssigkeitsmanagement und im Rahmen einer fortgeschrittenen Niereninsuffizienz die Nierenersatzbehandlung.\r\nSchlussfolgerung: Bei Patientinnen und Patienten mit Herzinsuffizienz stellt die Verschlechterung der Nierenfunktion einen prognostisch beeinflussenden Faktor dar. Die Nierenfunktionseinschränkung hat ebenso eine signifikante Erhöhung von Morbidität und Mortalität zur Folge. Obwohl es einheitliche Therapien für Herzinsuffizienz und Niereninsuffizienz gibt, stehen einheitlichen Leitlinien für die Therapie des kardio-renalen Syndroms noch nicht zur Verfügung. In den letzten Jahren hat sich zudem die Genauigkeit der kognitiven Selbsteinschätzung von psychiatrischen Patient*innen als relevanter Prädiktor für den Krankheitsverlauf herausgestellt. Derzeit verfügbare Therapien versuchen die Physiologie des Glukosestoffwechsels durch exogene Zufuhr von Insulin möglichst nachzuahmen. Neben der Entwicklung von Insulinen mit unterschiedlicher Pharmakokinetik und Dynamik, zeigten sich im letzten Jahrzehnt große Fortschritte in der Diabetestechnologie. Sowohl betroffene Kinder als auch Erwachsene profitieren in ihrem alltäglichen Leben sehr von kontinuierlichen Glukosemonitoring Sensoren (CGM), Insulinpumpen und zuletzt auch von sogenannten automatisierten Insulinabgabesystemen (Automated Insulin Delivery Systems/ AID). Diese ermöglichen durch Kombination einer Insulinpumpe mit einem CGM-Sensor sowie einem entsprechend programmierten Algorithmus eine bedarfsbedingte Insulinangabe, angepasst an die aktuelle Sensorglukose. In den letzten Jahren kam es zur Entwicklung einer Vielzahl unterschiedlicher Systeme, wodurch deren positiver Einfluss auf das Diabetesmanagement sowie der Vermeidung von akuten und chronischen Komplikationen durch eine Vielzahl von Studien bestätigt wurde. Unabhängig davon sind nach aktueller Recherche keine direkten Vergleiche von kommerziell verfügbaren AID-Systemen verfügbar. Given the evolutionary pressure to optimize energy requirements, it is hypothesized that these oscillations are indeed exploited to enhance its metabolic demand. Investigating this phenomenon from an energetic perspective provides a new outlook on the topic and contributes to the understanding of neural coding. Based on an extensive literature review and the simulation of a simple probabilistic network, energy costs for specific tasks are evaluated. Phase coding of individual neurons is a compelling possible explanation for oscillatory background activity. At the network level, distributed representation of a stimulus over multiple neurons proves highly efficient for probabilistic low-cost neurons, and for the optimization of the representational capacity of a network. An additional role for oscillatory activity was proposed in the temporal segmentation of computations and highly efficient clustered processing. The identification of mechanisms at work in biological neural networks seems to be highly relevant for the development of artificial neural networks. Current neuromorphic approaches are discussed.
