Auch wenn die Behandlung von Tumorschmerzen eine medizinische Herausforderung darstellt, so erscheint ein adäquates Schmerzmanagement entscheidend, um das Leid der Betroffenen bestmöglich zu lindern. Zur Verbesserung der neuropathischen Tumorkomponente kommen die beiden Antikonvulsiva Pregabalin und Gabapentin zum Einsatz. Zum Einsatz der beiden Wirkstoffe bei Palliativpatient*innen liegen genauso begrenzt Daten vor wie Informationen zum Kurzzeiteffekt dieser Antikonvulsiva. Dieser Prozess wird zumeist von einer Entzündung des angrenzenden Myokards begleitet, wodurch der Begriff Peri-(Myo-)karditis Verwendung findet. Die zeitliche Einteilung erfolgt in akut, rezidivierend, persistierend und chronisch. Bei thorakalen Beschwerden ist ein breites Spektrum an Differentialdiagnosen zu berücksichtigen. Jedoch besteht die Gefahr, dass die Symptome falsch interpretiert werden, da der Fokus zumeist auf häufig auftretenden kardialen Ereignissen liegt. Daher wurde ein Schwerpunkt der Diplomarbeit auf die differentialdiagnostische Unterscheidung der Peri- (Myo-)karditis von ischämischen Ereignissen, wie einem Myokardinfarkt oder einer restriktiven Kardiomyopathie gelegt. Diese Diplomarbeit befasst sich ebenso mit der Klinik, den Möglichkeiten der Diagnostik, den Komplikationen und der Therapie.\r\nMethoden: Im Zuge des Verfassens der Diplomarbeit wurde eine detaillierte Literaturrecherche durchgeführt, um die oft verkannte Diagnose Peri-(Myo-)karditis zu beschreiben und Unterscheidungsmerkmale zu den differentialdiagnostisch möglichen Erkrankungen herauszuarbeiten. Die Literaturrecherche erfolgte via PubMed sowie den Portalen Google Scholar und Ovid MEDLINE, um aktuelle Studienergebnisse vergleichen zu können. Als weitere Quellen sind Fachbücher, Guidelines, wissenschaftliche Reviews und Studien zu nennen, die sich mit (peri-)kardialen Erkrankungen befassen.\r\nErgebnisse: Trotz einiger Merkmale, die sowohl bei der Peri-(Myo-)karditis als auch bei den weiteren Differentialdiagnosen vorgefunden werden können, ist eine Unterscheidung möglich. So sind im Rahmen der akuten Erkrankung vier Hauptdiagnosekriterien feststellbar, die mithilfe des Elektrokardiogramms, der Auskultation und der Echokardiographie detektiert werden können. Es ist vor allem in Bezug auf die Diagnostik zu erwähnen, dass die Perikarditis in einigen Fällen klinisch inapparent verbleiben kann. Doch können als Folge der Herzbeutelentzündung schwerwiegende Komplikationen auftreten, zu denen der Perikarderguss, die Herzbeuteltamponade, eine Pericarditis constrictiva und die tuberkulöse Perikarditis zählen. Therapeutisch ist die invasive von der medikamentösen Vorgehensweise zu unterscheiden.\r\nKonklusion: Obwohl die Peri-(Myo-)karditis eine seltenere Differentialdiagnose bei thorakalen Schmerzen darstellt, sollte sie dennoch im Rahmen der Diagnostik bedacht werden, da schwerwiegende Komplikationen die Folge sein können. Der akute Myokardinfarkt (AMI) kann durch eine Eingefäß- oder Mehrgefäßerkrankung bedingt sein und resultiert in Hypoperfusion und Myokardhypoxie. Die prognostischen Auswirkungen der Lokalisation der „culprit lesion“ sowie die positiven Ergebnisse einer vollständigen akuten Revaskularisierung werden immer noch diskutiert. In dieser Arbeit werden unter anderem die prognostischen Auswirkungen des initialen Gefäßstatus und dessen Interventionsergebnis analysiert und interpretiert.\r\nMethoden\r\nDiese Studie ist eine prospektive Single-Center-Registerstudie, die Daten von Mai 2019 bis April 2023 inkludiert. 558 Patienten/Patientinnen (28,5% wbl.) wurden in dieser Zeit mit Vasopressor-pflichtigem Kreislaufschock im Herzkatheterlabor des LKH Graz vorstellig. Die Daten wurden in einem Prüfbogen (CRF) gesammelt und hinsichtlich intrahospitaler Mortalität analysiert und statistisch ausgewertet.\r\nErgebnisse\r\nPersonen mit Mehrgefäßerkrankung haben verglichen mit Eingefäßerkrankung ein signifikant höheres Risiko intrahospital zu versterben ([RR]: 0,727 (95%-KI: 0,539 ; 0,981); x2=5,042; p=0,025). Bei initial identifizierter LAD als „culprit lesion“ ist das intrahospitale Mortalitätsrisiko verglichen mit den anderen Gefäßen signifikant erhöht ([RR]: 0,782 (95%-KI: 0,629 ; 0,973); x2=4,799; p=0,028), hingegen kann bei identifizierter RCA als „culprit lesion“ ein signifikant geringeres intrahospitales Mortalitätsrisiko nachgewiesen werden ([RR]: 1,320 (95%-KI:1,019 ; 1,711); x2= 4,863; p= 0,027).\r\nErgänzend konnte eine signifikante entgegengesetzte Assoziation zwischen akuter vollständiger Revaskularisierung und intrahospitaler Mortalität aufgezeichnet werden ([RR]=1,559 (95%-KI: 1,209 ; 2,010); x2=13,441; p<0,001). Eine unvollständige Revaskularisierung geht mit einer erhöhten intrahospitalen Mortalitätsrate einher.\r\nConclusio\r\nDie ohnehin ernüchternde Prognose des CS kann unter anderem von initialem Gefäßstatus und Interventionsergebnis positiv und negativ beeinflusst werden.
