Weltweit ist die kombinierte hormonelle orale Verhütungspille (KOK) die am zweithäufigsten verwendete Form der reversiblen Empfängnisverhütung und hat von allen modernen Methoden die größte geografische Verbreitung. Mit einem Pearl Index von etwa 0,3 haben alle KOK-Präparate eine vergleichbare Wirksamkeit und sind daher hochwirksam zur Verhinderung einer (ungewollten) Schwangerschaft. Derzeit gibt es drei Arten oraler Verhütungspillen: die kombinierten Östrogen-Progesteron-Pillen, die reinen Progesteron-Pillen und Pillen zur Dauer- und Langzeitanwendung. Diese variieren in der Dosis und Art des Östrogens, der Dosis und Art des Gestagens, dem Verabreichungsregime und dem Verabreichungsweg. Blood loss is the most common cause of early death in trauma patients, while traumatic brain injury (TBI) is the most frequent origin of delayed death and morbidity due to injuries to brain and its encasing structures. Ab 28 + 0 bis 36 + 6 SSW spricht man von Frühgeburten. Im Jahr 2022 kamen in Österreich insgesamt 5.647 Frühgeborene zur Welt. Die Planung des Geburtsmodus bei extremen Frühgeburten ist äußerst komplex und stellt sowohl das medizinische Personal als auch die Eltern vor schwierige Entscheidungen. So besteht ein Verletzungsrisiko des Nervus cutaneus femoris lateralis beim direkt anterioren von 0-83% (5) und beim anterolateralen Zugangsweg zum Hüftgelenk von 0-27% (6,7). Alle drei Nerven zeigen sich durch operative Zugänge zur Lendenwirbelsäule gefährdet (1–4). Zur Risikominimierung empfehlen diverse Autor*innen genaue Kenntnisse des regulären Verlaufs und von Varianten dieser Nerven (1,2,4). Um zu diesem Verständnis beizutragen, wurden die Austrittshöhen und etwaige Stammbildungen der Nervi iliohypogastricus, ilioinguinalis et cutaneus femoris lateralis untersucht. Bei letzterem erfolgte die Vermessung des Durchtrittspunkts durch die Fascia lata in Bezug auf die Spina iliaca anterior superior. Until now, the identification and management of TOS create a highly difficult challenge. According to the Cochrane Library 2017, it is proven that an injection of Dexamethasone perineural prolongs the duration of the local anesthetic used for brachial plexus nerve blocks. However, the assumption that Dexamethasone perineural leads to long-term pain relief in this nerve compression syndrome is not yet scientifically proven but highly presumable. This pilot trial aims to demonstrate that a scalene nerve block with Dexamethasone perineural contributes to significant pain relief in patients with neurogenic TOS. The primary objectives are to measure the extent and the duration of the pain relief.
