So besteht ein Verletzungsrisiko des Nervus cutaneus femoris lateralis beim direkt anterioren von 0-83% (5) und beim anterolateralen Zugangsweg zum Hüftgelenk von 0-27% (6,7). Alle drei Nerven zeigen sich durch operative Zugänge zur Lendenwirbelsäule gefährdet (1–4). Zur Risikominimierung empfehlen diverse Autor*innen genaue Kenntnisse des regulären Verlaufs und von Varianten dieser Nerven (1,2,4). Um zu diesem Verständnis beizutragen, wurden die Austrittshöhen und etwaige Stammbildungen der Nervi iliohypogastricus, ilioinguinalis et cutaneus femoris lateralis untersucht. Bei letzterem erfolgte die Vermessung des Durchtrittspunkts durch die Fascia lata in Bezug auf die Spina iliaca anterior superior. Until now, the identification and management of TOS create a highly difficult challenge. According to the Cochrane Library 2017, it is proven that an injection of Dexamethasone perineural prolongs the duration of the local anesthetic used for brachial plexus nerve blocks. However, the assumption that Dexamethasone perineural leads to long-term pain relief in this nerve compression syndrome is not yet scientifically proven but highly presumable. This pilot trial aims to demonstrate that a scalene nerve block with Dexamethasone perineural contributes to significant pain relief in patients with neurogenic TOS. The primary objectives are to measure the extent and the duration of the pain relief. Kinder mit chronisch entzündlichen Darmerkrankungen (CED) haben ein sechsmal höheres Risiko venöse Thromboembolien zu erleiden als gleichaltrige gesunde Kinder. Anorganische Polyphosphate (PolyP) sind negativ geladene Polymere bestehend aus unterschiedlich langen Phosphatketten, welche zu diesem erhöhten Risiko beitragen könnten. PolyP kann sowohl von Bakterien, mit einer durchschnittlichen Kettenlänge von 500-1000 Monophosphaten, als auch von Thrombozyten, mit einer Kettenlänge von 70-120 Monophosphaten, produziert werden. Im Blut aktiviert PolyP das Gerinnungssystem, indem es über FXII mit dem Kallikrein-Kinin-System interagiert, wodurch das stark entzündungsfördernde Bradykinin freigesetzt wird. Andererseits entfaltet exogenes PolyP im Darmlumen eine remissionsinduzierende Wirkung und verbessert unter anderem die Barrierefunktion, Inflammation und Fibrosierung über Upregulation von Heat-Shock-Protein-27. Mesalazin, die Goldstandardmedikation zur Remissionsinduktion sowie Erhaltung, hemmt die bakterielle PolyP-Synthese, wodurch dessen antiinflammatorische Eigenschaften weiter erklärt werden könnten. Wenn bakterielles oder thrombozytäres PolyP in die Blutzirkulation gelangt, könnten Thrombosen oder vermehrte Entzündungen die Folge sein. Chronically ill patients are particularly frequently affected. Sarcopenia is one of the most common concomitant diseases of liver cirrhosis. This work aims to analyse whether there is a correlation between sarcopenia and mortality in patients with liver cirrhosis. In addition, it is examined whether complications of liver cirrhosis occur more frequently in patients with sarcopenia.\nMaterial and methods: The data of 118 patients with liver cirrhosis and 60 patients without cirrhosis (control group) from the Department of Gastroenterology and Hepatology were analyzed retrospectively. Ursprünglich wurde angenommen, dass Metformin hauptsächlich über die Leber wirkt, um den Blutzuckerspiegel zu senken. Neuere Forschung legt jedoch nahe, dass auch der Magen-Darm-Trakt, das Darmmikrobiom und Immunzellen beteiligt sind. Auf molekularer Ebene wirkt Metformin durch die Aktivierung der AMPK (Adenosinmonophosphat- aktivierte Proteinkinase), wodurch die Glukoseproduktion in der Leber reduziert wird.
