Ziel ist es, die Unterschiede zwischen diesen gerichtsmedizinischen Suiziddaten und den offiziellen Statistiken der Gesamtbevölkerung Steiermark zu ermitteln sowie zeitliche und methodische Veränderungen in den Suizidmustern aufzuzeigen. Die Ergebnisse zeigen, dass die Suizidraten aus den Obduktionsberichten niedriger sind als die in der allgemeinen Statistik. They affect 5% to 7% of Western societies and include diseases like rheumatoid arthritis, psoriasis, inflammatory bowel disease, and multiple sclerosis. These conditions significantly impact patients’ quality of life, causing physical pain, impaired function, and psychological distress. They also pose a substantial socioeconomic burden due to high healthcare costs and reduced productivity. Historically, IMID treatment focused on symptom management. However, the introduction of biological drugs and small molecule inhibitors has revolutionized therapy by targeting specific immune pathways. Biologics, such as TNF-α inhibitors and IL blockers, have shown high efficacy in reducing inflammation and improving patient outcomes. Small molecule drugs like JAK inhibitors offer advantages like oral administration but require careful monitoring due to potential side effects. Emerging technologies are further enhancing treatment precision. Multiomics approaches integrate genomic, transcriptomic, proteomic, metabolomic, and epigenomic data to provide deeper insights into IMID mechanisms. These methods help identify novel biomarkers and therapeutic targets, predict disease progression, and assess treatment response. Big data analytics and comparative methods accelerate drug discovery by analyzing large datasets from biobanks and clinical trials. In silico clinical trialsoffer a cost-effective alternative to traditional trials, reducing development costs and addressing ethical concerns. Targeted therapies focus on specific immune pathways such as NF-κB signaling and the IL-23/IL-17 axis, which play central roles in inflammation across multiple IMIDs. By modulating these pathways, new therapies can address unmet needs in patients who do not respond to existing treatments. Despite these advancements, challenges remain. The heterogeneity of IMIDs complicates the development of universal therapies. High costs limit access to advanced treatments, and integrating new technologies into clinical practice requires significant infrastructure and expertise. Bei 15% der Paare kommt es zum wiederholten Implantationsversagen (RIF). Es handelt sich um ein Zustandsbild multifaktorieller Genese. Ein Einflussfaktor ist die Dysregulation des Immunsystems, bei der u. a. durch ein ungünstiges Verhältnis von Zytokinen eine erfolgreiche Implantation des Embryos verhindert wird. Chronische Entzündungen wie die Infektion mit Helicobacter pylori (H. pylori), einem Bakterium, das den Magen besiedelt und u. a. eine Dysbalance des Immunsystems von RIF-Patientinnen auslöst, nehmen so Einfluss auf die Ausbildung von RIF. They serve as Ca2+ buffers, initiate apoptotic cell death, and are a central hub for cellular metabolism. The uptake of Ca2+ into the mitochondrial matrix is highly regulated by the mitochondrial calcium uniporter complex (MCUC), a multiprotein complex in the inner mitochondrial membrane. The MCUC consists of the pore-forming protein MCU, the dominant negative subunit MCUb, its essential regulator EMRE, the so-called gatekeepers MICU1 and 2, the assembly factor MCUR1, and, under certain conditions like cancer or aging, the uncoupling proteins UCP2/3. Opening of the MCUC is facilitated by local Ca2+ hotspots, which lead to Ca2+ binding to the EF-hand domains of the MICUs and a subsequent conformational change. In this work, I studied the translation of cytosolic Ca2+ signals from different sources, including the endoplasmatic reticulum (ER) and extracellular space via the store-operated Ca2+ entry (SOCE), to the mitochondrial matrix. I used different concentrations of the IP3-generating agent histamine and the antipsychotic drug risperidone, which exhibits antihistaminic side effects, and correlated the changes in cytosolic Ca2+ signals with those in the mitochondrial matrix. Hohes Alter, Komorbiditäten sowie ein vorangeschrittener Krankheitsverlauf bei oftmals spätem Diagnosezeitpunkt limitieren die isolierte chirurgische Therapie, sodass diese häufig nur im Rahmen kombinierter Operationsverfahren (z.B. bei gleichzeitiger Bypass-Operation) Anwendung findet. Lange Zeit stellte daher die alleinige symptomatische medikamentöse Behandlung bei höhergradiger isolierter sekundärer Trikuspidalinsuffizienz, trotz schlechtem Outcome, die einzige Therapieoption dar. Although they are rare, these diseases are clinically important as affected patients can present with severe clinical symptoms like jaundice, pruritus as well as further complications like chronic liver disease, potentially requiring a liver transplant as ultimate therapy. Cholestatic pruritus can greatly reduce quality of life. With new treatments like ASBT inhibitors now available, it’s important to know how common this condition is in order to understand how many patients could benefit from these new options. Throughout this wide group of syndromes, clinical presentation can vary between types of genetic mutations, age groups, and triggering events such as infections, pregnancy or drug medications, possibly triggering the manifestation of symptoms in patients with a genetic predisposition. Methods: We retrospectively analyzed patients with a suspected hereditary cause of cholestasis who underwent genetic testing at our university hospital throughout the last 20 years. Clinical data, laboratory findings, and genetic variants (e.g. mutations, SNPs) were reviewed to evaluate genotype-phenotype correlations, clinical presentation by age, and potential disease triggers. Results: From medical records, we identified 88 patients, 64 children and 24 adults, with a possible hereditary cause of cholestasis. Only 53 patients presented with a clear cholestatic phenotype (such as PFIC, BRIC, ICP or LPAC). The remaining 35 patients had evidence of cholestasis or mutations but without a final clinical classification at the time of research. Genetic variants were identified in 50 out of 88 patients, most frequently in the ABCB11 and ABCB4 gene. 38 patients had mutations without a clear clinical diagnosis. The clinical presentation varied by age. Pediatric cases (predominantly male (44 out of 64)) were more often associated with ABCB11 variants and presented with jaundice, whereas adult patients (predominantly female (16 out of 24)) more often carried ABCB4 variants and presented with biliary symptoms or gallstone disease. Simply determination of zygosity status of a variant (i.e. homo- or heterozygote variants) cannot predict clinical appearance since remaining function of the protein a critical contributing factor. Pruritus was documented in only approximately 6% of cases at the time of diagnosis but in around 22% during disease course, clustering with ABCB11 variants in children and ABCB4 or ATP8B1 variants in adults. Treatment was mainly symptomatic, with UDCA prescribed in 32 patients and in only two cases a specific antipruritic therapy was documented. Methods: Therefore, this thesis comprises three studies that aimed to (1) characterize the symptoms, burdens, and impacts of Long-COVID, (2) investigate structural and functional brain changes associated with cognitive impairment - one of the most frequently reported symptoms - and (3) explore the effectiveness of a tablet-based training program as a potential intervention for cognitive and psychological deficits. Results: The findings of this thesis underscore (1) the substantial impact of Long-COVID on patients’ personal and professional lives, including a broad range of symptoms, high levels of psychological distress, and work-related consequences such as loss of income or inability to work. In addition, (2) significant functional brain alterations were observed between patients with and without cognitive impairment following COVID-19, especially in large-scale brain networks such as the default mode network (DMN), which plays an important role in various mental processes. Finally, (3) the findings of this thesis provide partial support for the use of tablet-based training programs to alleviate subjective cognitive complaints, reduce depressive symptoms, and support memory performance in affected individuals. Conclusion: These findings highlight the diverse clinical presentation of Long-COVID and its significant impact on those affected. Moreover, they underscore the need for a deeper understanding of the neuronal mechanisms underlying its cognitive and psychological symptoms. Diese führen zu degenerativen Modifikationen, insbesondere zur Arteriosklerose, die wiederum meist die Hauptursache für die epidemiologisch bedeutsamsten Entitäten wie koronare Herzkrankheit, arterielle Hypertonie, Myokardinfarkt und ischämischen Apoplex ist.\r\nZu Beginn der Arbeit wird der Alterungsprozess und sein Zusammenhang mit oxidativem Stress erläutert sowie die biochemischen Grundlagen der Radikale und Antioxidantien erklärt. Der Hauptteil untersucht, ob eine präventive oder therapeutische Supplementation von Vitamin C, Vitamin E oder Carotinoiden die Entstehung oder die Progredienz solch degenerativer Veränderungen beeinflussen kann. Dafür wurde eine Literaturrecherche mithilfe mehrerer Nachschlagewerke und der Datenbanken PubMed und UpToDate durchgeführt, wobei systematische Reviews, Meta-Analysen sowie pro- und retrospektive Studien analysiert wurden. Als primäres Recherchekriterium wurde die isolierte Wirkung der genannten Antioxidantien auf kardiovaskuläre Parameter gewählt.\r\nInsgesamt fehlen eindeutige Studienergebnisse, die eine evidenzbasierte Empfehlung für oder gegen die isolierte Supplementation der genannten Antioxidantien erlauben. Weitere Studien sind erforderlich, da antioxidative Substanzen weiterhin großes Potenzial für die Gesundheitsprävention aufweisen und insbesondere in der Altersmedizin lebensqualitätssteigernde Effekte haben könnten. In die Untersuchungen sollten sowohl der individuelle Gesundheitszustand als auch Einflussfaktoren wie Ernährungsgewohnheiten und Lebensstil miteinbezogen werden, um Daten zu Dosierung, potenziellen Nebenwirkungen und Wechselwirkungen unterschiedlicher Nährstoffe zu sammeln. In this systematic review, we analyzed 26 studies from the past 15 years focused on injection-based carcinogenesis in small animal models. Our aim was to identify standardized practices in animal and cell line selection, injection methods, volumes and cell concentrations. The findings highlight the variability in methodologies, underscoring the need for consistency to streamline future research and enhance translational relevance. Our review concludes that adopting precise and minimally invasive approaches, such as endoscopic injection, in anatomically scalable models like rats, could bridge the gap between experimental designs and human CRC pathology.
