The disease causes a wide variety of symptoms and has a mortality rate similar to measles, with as much as 89,000 deaths per year.\r\nB. pseudomallei spreads intracellularly and can invade phagocytic and non-phagocytic cells alike. It possesses a Type 3 Secretion System-3 (T3SS-3), which is essential for successful invasion of the host cell and spread. It delivers effector proteins into the host cytosol, which are able to subvert the immune response. Therefore, investigation of the interactions between T3SS-3 proteins and the host are relevant for future treatment options. Many mechanisms of the pathogen regarding infection, host evasion and resistance are not fully understood yet. Hence new tools for the investigations of the molecular basis of these mechanisms in B. pseudomallei are needed. A promising new way is the incorporation of non-canonical amino acids (ncAAs) into proteins. More than 150 different ncAAs have been introduced into proteins using the amber stop codon and a corresponding tRNA/aminoacyl-tRNA synthetase pair. These ncAAs, which are amino acids not used in nature to build proteins, can have fluorescent, cross-linking or redox-active functions. Their divers physical, chemical and biological properties have been exploited for a variety of biological studies and biotechnical applications already in many organisms including E. coli, yeast and mammalian cells. Therefore, providing a powerful tool for studying protein-protein interactions, host-pathogen interaction and many others. Diagnosis is often problematic, due to the rather unspecific symptoms and diagnostic tools with mediocre performance. Up to now, the current diagnostic standard methods are the indirect hemagglutination assay (IHA) and culture, which are both hampered by their low sensitivity (60%). Therefore, there is a great need for the development of new, standardizable and sensitive diagnostic tools. The aim of this work was the development of an improved version of a protein microarray and a point-of-care test for serological testing of melioidosis. The here described protein microarray as well as the lateral flow assay (LFA) are based on a multiplex detection of serum antibodies against B. pseudomallei proteins. On the protein microarray the immune response against up to 400 antigens can be analyzed in parallel, whereas the whole procedure is highly standardizable, which makes it particularly attractive for routine diagnostic. IgG-detection is based on a horse radish peroxidase labeled goat anti-human IgG detection antibody. Based on the data obtained from the improved microarray version and the screening of more than 400 sera, new antigen candidates for the LFA were chosen, to improve its performance. On the LFA, four antigens can be spotted and an individual’s immune response is detected by colloidal gold conjugated secondary anti-human IgG antibodies. The LFA is not only a cost effective, but also a very simple to use point-of-care test, which makes it a perfect tool for an initial diagnosis at the patients home but also for epidemiological studies. Their expression is essential for a healthy placental and fetal development, as some studies indicate a crucial role of the PSGs in the regulation of the immune response, placental angiogenesis, and platelet activation. Proteomics data from preliminary experiments suggest that these pregnancy-specific glycoproteins accumulate in maternal platelets over the course of gestation. Therefore, the aim of this master thesis is to test whether PSG11 is selectively taken up by platelets and whether platelet priming is a crucial process during human pregnancy. Placental PSG11 from healthy first and third trimester as well as from preeclamptic tissue was characterized using immunohistochemistry, Western blot, and qPCR analysis. Platelets were isolated from whole blood samples either from healthy pregnant or non-pregnant women or male donors. Characterization of PSG11 in platelets was performed on protein level using immunocytochemistry, Western blot analysis and ELISA. Also, platelets from non-pregnant women were incubated with blood plasma of pregnant women in their third trimester of gestation, recombinant PSG11 or trophoblast-derived PSG11 and subsequently analyzed on protein level. Immunohistochemistry for PSG11 revealed positive signals in the villous syncytiotrophoblast of placental tissue from the first and third trimester of gestation as well as from preeclampsia patients. These results were verified on gene expression level via qPCR as well as on protein level via Western blot analysis. Our data showed an accumulation of PSG11 in platelets isolated from women in their third trimester of gestation compared to control groups, which were not pregnant. Furthermore, PSG11 was also detected in platelets from non-pregnant women after incubation with pregnant blood plasma. Trotz umfangreicher bis dato durchgeführter Forschungsarbeit gibt es bis heute keine eindeutigen, generellen Empfehlungen für bestimmte Parameter oder Screeningverfahren zur Identifikation von Sportlerinnen und Sportlern mit erhöhtem Verletzungsrisiko. Diesbezüglich wurde im Rahmen dieser Pilotstudie eine Testbatterie entwickelt, um Defizite in den motorischen Grundeigenschaften dynamisches Gleichgewicht, Maximalkraft und Sprungkraft festzustellen. Durchgeführt wurden der Y- Balance- Test (YBT), isometrische Maximalkraftmessungen des Musculus quadriceps femoris und der ischiocruralen Muskulatur sowie der Single Hop for distance und der Cross Hop for distance. An der Studie nahmen 156 Sportlerinnen und Sportler aus den Sportarten Schi Alpin, Handball und Fußball teil. Weiters erfolgte im Rahmen der Durchführung der Tests eine ausführliche Verletzungsanamnese der letzten 12 Monate (zur retrospektiven Analyse). Für die Studie waren Nichtkontaktverletzungen der unteren Extremität von Relevanz. Von 45 Sportlerinnen und Sportlern wurde zudem nach einem Jahr eine weitere anamnestische Erhebung entsprechender Verletzungen durchgeführt (zur prospektiven Analyse). \r\nDie Leistungsparameter der Studienteilnehmerinnen und -teilnehmer wurden einerseits zur Etablierung von Normwerten herangezogen. Andererseits erfolgte anhand von in der Literatur definierten Cutoff- Werten eine Einteilung in zwei Gruppen. Gruppe A repräsentierte jene Sportlerinnen und Sportler, welche die Cutoff- Werte erreichten, Gruppe B repräsentierte jene, welche die Cutoff- Werte nicht erreichen konnten. Folgende Parameter und Cutoff- Werte wurden für die Gruppeneinteilung herangezogen:\r\n1.YBT anteriore Asymmetrie: Gruppe A < 4cm; Gruppe B ≥ 4cm\r\n2.YBT Composite Score: Gruppe A > 94%; Gruppe B ≤ 94%\r\n3.LSI der Maximalkraftmessungen und Hop- Tests: Gruppe A ≥ 90%; Gruppe B < 90%\r\nUm nun festzustellen, ob die Zugehörigkeit zur Gruppe B mit einem höheren Risiko für Verletzungen verbunden ist, wurde zunächst ein Vergleich der Mittelwerte beider Gruppen durchgeführt. Es zeigten sich, mit Ausnahme des Cross Hop for distance bei den Mädchen, keine signifikanten Unterschiede in den Durchschnittswerten zwischen den Gruppen A und B. Weiters erfolgte die Analyse des Zusammenhanges zwischen höheren Seitenasymmetrien bzw. schlechterem Gesamtscore des YBT und Verletzungen. In der retrospektiven Analyse zeigte sich kein signifikanter Zusammenhang mit Nichtkontaktverletzungen der vergangenen 12 Monate. In der prospektiven Analyse zeigte sich lediglich für den Single Hop for distance ein signifikanter Zusammenhang mit Nichtkontaktverletzungen der folgenden 12 Monate. \r\nZusammenfassend kann auch anhand dieser Pilotstudie keine eindeutige Empfehlung für eine Testbatterie bzw. bestimmte Parameter zur Identifikation von Sportlerinnen und Sportlern mit erhöhtem Verletzungsrisiko gegeben werden. Die anteriore Asymmetrie des YBT, der Composite Score des YBT, der LSI der Maximalkraftmessungen des Musculus quadriceps femoris und der ischiocruralen Muskulatur sowie der LSI des Cross Hop for distance können somit in dieser Studienpopulation nicht als Screeningverfahren für Verletzungen herangezogen werden. Der Reducer, ein sanduhrförmiger Stent, stellt eine neue, symptomatische Therapieform für diese Patient:innen dar. Das Ziel dieser Arbeit ist, die Auswirkungen der Reducer-Therapie auf die Angina pectoris Symptomatik durch den Vergleich verschiedener Parameter vor und nach der Implantation zu untersuchen.\r\nMaterial und Methoden: In dieser retrospektiven Datenanalyse wurden die Daten von zehn Patient:innen, die an der Klinischen Abteilung für Kardiologie am LKH Univ. Klinikum Graz einen Reducer implantiert bekommen hatten, vor der Reducer-Implantation, sowie sechs und zwölf Monate nach der Implantation ausgewertet und verglichen. Die zur Auswertung verwendeten Parameter waren die Canadian-Cardiovascular-Society-Klassifikation (CCS), der 6-Minuten-Gehtest, der Seattle Angina Questionnaire (SAQ), der EQ-5D-5L und der EQ-VAS. Es erfolgte ein Vergleich der Variablen zwischen Baseline (vor der Implantation) und 6-Monats-Visite nach der Implantation, sowie zwischen 6- und 12-Monats-Visite. Die statistische Auswertung wurde mit dem Programm IBM SPSS Statistics 29.0 ® durchgeführt. Die Datenanalyse erfolgte mittels deskriptiver Statistik und die möglichen Veränderungen zwischen den drei Zeitpunkten wurden mittels Wilcoxon-Test bei gepaarten Stichproben mit einem Signifikanzniveau von p = 0,05 ermittelt.\r\nErgebnisse: Sechs Monate nach der Reducer-Implantation zeigte sich eine signifikante Reduktion von mindestens einer CCS-Klasse bei sieben Patient:innen (70%). Die mittlere CCS-Klasse betrug zum Zeitpunkt der Baseline 2,6 ± 0,5 (p < 0,05) , zum Zeitpunkt der 6-Monats-Visite 1,7 ± 0,7 (p < 0,05).\r\nWeiters konnte eine signifikante Steigerung im Teilbereich Lebensfreude des Seattle Angina Questionnaire veranschaulicht werden. Bei der Baseline-Untersuchung wurden durchschnittlich 49,2 ± 14,94 Punkte von den Teilnehmer:innen vergeben, sechs Monate später verbesserte sich die mittlere vergebene Punktezahl auf 66,7 ± 27,5 Punkte (p < 0,05).\r\nFazit: Diese retrospektive Datenanalyse zeigte bei 70% der Teilnehmer:innen ein Ansprechen auf die Reducer-Therapie, das sich durch eine Reduktion der Angina pectoris in der CCS-Klassifikation und eine Steigerung der Lebensfreude im SAQ darstellen lässt. Dieses Ergebnis entspricht anderen, ähnlich aufgebauten, Studien zur Reducer-Therapie. In an animal study, it could already be proven that the supplementation with the polyamine spermidine led to lower blood pressure levels in mice. This thesis aims to give further insights into the possible cardioprotective effects of spermidine on heart function in patients with hypertension disease assessed by both conventional echocardiographic and strain parameters.\r\nMaterial and Methods: This work was based on retrospective data analysis of echocardiographic loops of 20 patients with arterial hypertension from the SMARTEST trial at four points in time (weeks 0, 8, 12, and 20). The subjects received 4mg spermidine per day and a placebo for each eight weeks with a washout interval of four weeks in between in a crossover design. Standard echocardiographic and strain parameters of visit 1 and visit 4 were statistically evaluated and compared with each other to assess cardiac function.\r\nResults: The results of this study showed no significant changes in left ventricular (LV) function and volumes, right ventricular (RV) function and diameter, left atrial (LA) function and diameter, and diastolic function between visit 1 and visit 4. Regarding strain analyses, a strong positive correlation between LV Endo global longitudinal strain (GLS) and LV AutoStrain (r = 0.651) as well as a strong negative correlation between biplane left ventricular ejection fraction (LVEF) and Endo GLS (r = -0.593) were determined on visit 4.\r\nConclusion: The administration of spermidine in hypertensive patients did not lead to any significant echocardiographic changes in volume, linear, and strain parameters. Nevertheless, this work could show the safety of oral spermidine supplementation on cardiac function in hypertensive patients evaluated by echocardiography. Die Antibiotika-assoziierte Diarrhoe (AAD) ist eine Erkrankung, die im klinischen Alltag häufig zu finden ist. Die Pathomechanismen dafür sind vielfältig und es bedarf weiterer Forschungsarbeit, diese endgültig zu klären. Mit dieser Diplomarbeit soll ein Beitrag zum besseren Verständnis der AAD geleistet werden.\r\nIn der Einleitung finden sich Begriffsdefinitionen und theoretische Grundlagen zum Thema Antibiotika und Diarrhoe, außerdem werden Grundlagen sowie mögliche Einsatzgebiete von Entzündungsmarkern im Stuhl dargestellt.\r\nMaterial und Methoden. Zur Beantwortung der Forschungsfragen „Welche Rolle spielt die fäkale Calprotectinbestimmung bei der Antibiotika-assoziierten Diarrhoe (AAD)?“ und „Welche Rolle spielt die fäkale Calprotectinbestimung nach Antibiotikaeinnahme ohne Diarrhoe?“ wurde eine systematische Literaturrecherche mithilfe der Meta-Plattform „Pubmed“ durchgeführt. Letztlich wurden 21 passende Untersuchungen gefunden, welche in neun Kategorien unterteilt, für die Beantwortung der Forschungsfragen herangezogen werden konnten. \r\nErgebnisse. Die Studien zeigen, dass die fäkale Calprotectinmessung bei der Differenzierung zwischen Clostridioides difficile Infektionen (CDI) und non-CDI AAD, sowie bei der Differenzierung zu anderen Ursachen einer Diarrhoe hilfreich sein kann. Der fäkale Calprotectinwert weist einen Zusammenhang zu auftretenden Komplikationen bei CDI und dem CDI-Schweregrad auf. Eingeschränkt kann der Wert herangezogen werden, wenn es um die Differenzierung von manifester CDI und asymptomatischer Clostridioides difficile (C. diff.) Kolonisation, C. diff. mit oder ohne Toxinproduktion, Prognose von CDI-Rückfällen und dem messbaren Therapieerfolg geht. Die Rolle des fäkalen Calprotectinwertes zur Vorhersagekraft für späteres Therapieansprechen ist fraglich. Ein Zusammenhang von hohen Werten und CDI-bezogenen Todesfällen besteht nicht. Zur Aussagekraft von fäkalen Calprotectinwerten bei Patient*innen nach Antibiotikaeinnahme ohne Diarrhoe finden sich keine Studienergebnisse.\r\nSchlussfolgerung. Fäkale Calprotectinmessungen zeigen für einige Fragestellungen eine hohe Aussagekraft. Bei anderen Fragestellungen zeigt sich hingegen bis jetzt kein eindeutiger Nutzen der Messung. Insgesamt sind weitere prospektive Studien mit großer Teilnehmerzahl notwendig, um die Calprotectin-Bestimmung auch in anderen klinischen Situationen zu etablieren.
