Die supradiakondyläre Fraktur weist zwei \r\nAlterspeaks auf, einmal in jungen Jahren mit Polytraumen und dann bei älteren durch \r\nStürze. Durch die steigende Lebenserwartung und die damit verbundene steigende \r\nAktivität bis ins hohe Alter, muss die Versorgung immer höheren Belastungen standhalten. \r\nZudem erschwert die im Alter zunehmende Osteoporoserate die Versorgung. Ziel dieser \r\nStudie ist die Evaluierung, ob die derzeit angewandten Osteosyntheseverfahren immer \r\nnoch zufriedenstellende Ergebnisse liefern und als state of the art weiterhin vertretbar sind. \r\n\r\nMethoden: \r\nFür die Studie wurden retrospektiv Daten von 56 Patient*innen, welche vom 01.01.2009 \r\nbis zum 31.12.2018 an der Universitätsklinik für Orthopädie und Traumatologie der \r\nMedizinischen Universität Graz aufgrund einer distalen Humerus-C-Fraktur mittels \r\nPlattenosteosynthese operativ versorgt wurden, erhoben. Darüber hinaus wurden 16\r\nPatient*innen für eine einmalige klinische und radiologische Nachuntersuchung zur \r\nErhebung des funktionellen und subjektiven Outcomes, sowie zur Erfassung etwaiger \r\nKomplikationen, einberufen. Weitere fünf Proband*innen nahmen im Rahmen von \r\nFragebögen teil. Drei davon retournierten zusätzlich Videos/Fotos des\r\nBewegungsausmaßes ihrer Ellenbogengelenke. Die Datenauswertung erfolgte mittels \r\ndeskriptiver Statistik. Ergänzend wurden die Ergebnisse mit bereits bestehenden Studien \r\nverglichen und diskutiert. \r\n\r\nErgebnisse: \r\nDas mittlere Alter zum Zeitpunkt der Verletzung betrug 62,6 Jahre, etwa 21 % der \r\nOperierten waren bereits verstorben. Die Nachuntersuchung erfolgte im Schnitt nach ca.\r\nneun Jahren. \r\nBei den 19 komplett nachuntersuchten Patient*innen wurde im Durchschnitt ein Mayo\r\nElbow Performance Score (MEPS) von 83,2 Punkten ermittelt. 74 % der Patient*innen \r\nhatten nach Cassebaum ein gutes oder sehr gutes Ergebnis, 53 % nach Jupiter. Der \r\nFlexion-Extension Gesamtbogen betrug 97° und der Pronation-Supination Gesamtbogen \r\n140°. Der QuickDASH-Score betrug im Durchschnitt 23,3. \r\nIV\r\nBei 36 % der 56 Studienteilnehmer*innen kam es laut den MEDOCS-Daten im \r\npostoperativen Verlauf zu Komplikationen, bei der Betrachtung von 20 der 21 \r\nnachuntersuchten Patient*innen sogar bei 65 %, ein Patient wurde aufgrund mangelnder \r\nRöntgenbilder nicht berücksichtigt. Als häufigste Komplikation trat bei sechs von 13\r\nProbanden eine Arthrose auf.\r\n\r\nSchlussfolgerung: \r\nTrotz des hohen Durchschnittsalters ergab die Untersuchung zufriedenstellende \r\nErgebnisse. Es kann somit weiterhin an der Versorgung mit doppelter Plattenosteosynthese \r\nbei distalen Humerusfrakturen festgehalten werden. Bei manchen Patient*innen treten auch überschießende Lokalreaktionen („Large Local Reactions“ = LLRs) von über 10 cm Durchmesser, welche über 24 Stunden persistieren, auf. Die Prävalenz dieser LLRs variiert in der Literatur stark und liegt zwischen 2,4% und 26,4%, was in erster Linie an uneinheitlichen Definitionen für LLRs und heterogenen Studienpopulationen liegt. Hymenopterenstiche führen jedoch bei ca. 3% der europäischen Bevölkerung auch zu systemischen Stichreaktionen (SSRs) mit potentiell lebensbedrohlichen Anaphylaxien. Das Risiko von Patient*innen, die zuvor eine oder mehrere LLRs entwickelt haben, beim nächsten Stich eine SSR zu erleiden, wird intensiv diskutiert. Diese Diskussion wurde in den letzten Jahren insbesondere durch eine Studie von Bilò et al. erneut angestoßen.
