Diese Merkmale sind einer enormen Anreicherung an Mitochondrien mit defekter OXPHOS-Kette zuzuschreiben. Onkozytäre Veränderungen werden in der Routinediagnostik nicht zuverlässig als solche erkannt, weshalb es immunhistochemischer mitochondrialer Färbungen bedarf. Mittels eines pan-mitochondrialen Markers (Prohibitin) wurde in der Vorstudie (1) nicht nur die Erkennung von onkozytären Veränderungen verbessert, sondern auch die Existenz von „mitochondrion-rich-non-oncocytic“ (MRNO) Tumoren beschrieben. In einer späteren Studie wurde Prohibitin um NDUFS4 als Marker der intakten OXPHOS-Kette, ergänzt und die neue Markerkombination erfolgreich zur Detektion von onkozytären Veränderungen an papillären Schilddrüsentumoren angewandt (2). Eine Konzepterarbeitung für Präventionsprogramme im Kinderschutz.", "abstract": "Einleitung\r\nIn der Literatur tauchen Misshandlungssyndrome wie das Shaken Baby Syndrome nach wie vor selten auf und es bedarf laut rezenten Publikationen an Aufklärung und Prävention. Regelmäßig wird in der Presse über den Tod durch Schütteln eines Säuglings berichtet. Diese Diplomarbeit soll untersuchen, ob die Bevölkerung über Traumata bei Neugeborenen und Säuglingen durch Schütteln informiert ist und ob das klinische Bild des Shaken Baby Syndrome bzw. Schütteltrauma bekannt ist. Darüber hinaus soll erhoben werden, ob dies durch Unkenntnis geschieht und ob diese Syndrome im Sinne eines Tabuthemas nicht ausreichend aufgeklärt sind.\r\n\r\nMethoden\r\nEs wurde ein Online-Fragebogen via LimeSurvey mit insgesamt 41 Fragen erstellt. Nach der Erhebung persönlicher Informationen wurde die Aufzeichnung eines Demonstrationsvideos abgespielt, in dem eine Säuglingspuppe geschüttelt wird. Die ProbandInnen sollten daraufhin ihre persönliche Einschätzung zu möglichen Folgen abgeben. Anschließend wurden 27 Fragen zur Thematik des Schütteltraumas gestellt, um den Wissensstand bzw. die Einstufung der Gefährlichkeit des Schüttelns abbilden zu können. Ebenso erhoben wurde die Kenntnis im Umgang mit Schreibabys und deren Schreiverhalten.\r\n\r\nErgebnisse\r\nVon August bis September 2022 nahmen 319 ProbandInnen am Fragebogen teil. Die Mehrheit der TeilnehmerInnen war weiblich (n = 246; 77,1 %), verheiratet (n = 177; 56 %) und hatte eigene Kinder (n = 244; 76,5 %). Die überwiegende Mehrheit (n = 313; 97,5 %) wusste, dass Schütteln zu tödlichen Verletzungen führen kann. Die Personen (n = 6; 1,8 %), die eine Todesfolge ausschlossen, schätzten zeitgleich die Verletzungsschwere erheblich niedriger ein.\r\nDie TeilnehmerInnen, die eigene Kinder besitzen, erhielten weder vor (n = 208; 85,2 %) noch nach der Geburt (n = 210; 86,1 %) Informationen über das Schütteltrauma, auch nicht im Rahmen von Mutter-Kind-Pass-Untersuchungen (n = 216; 88,5 %). Anlaufstellen bei Überforderungssituationen waren den meisten Teilnehmern unbekannt (n = 197; 80,7 %). Große Unsicherheit zeigte sich zudem auch bei der Auswahl subjektiv korrekter bzw. falscher Verhaltensweisen bei anhaltend schreienden Kindern mit zunehmender elterlicher Überforderung und Erschöpfung.\r\n\r\nSchlussfolgerungen\r\nBis auf wenige Ausnahmen ist es in der Bevölkerung bekannt, dass Schütteln von Neugeborenen und Säuglingen zu schweren Verletzungen führen kann. Strategien zur Vermeidung von Aggressionsverhalten bei Erschöpfung oder Überforderung sind den Betreuungspersonen allerdings nur unzureichend bekannt. Eine besonders große Herausforderung der kommenden Jahre wird die Versorgung in ländlichen Gebieten darstellen. Zu diesem Thema wurden bislang nur wenige Studien mit kleiner Stichprobengröße durchgeführt, die sich mit den Wünschen und Berufsziele von Studierenden, Schüler*innen und jungen Absolvent*innen im medizinischen Bereich befassen. Die gewonnenen Daten sollen genutzt werden, um mit Verantwortlichen in Krankenhäusern, Politiker*innen und anderen Stakeholdern in den Dialog zu treten und aktiv Veränderungen in der medizinischen und pflegerischen Versorgung in Österreich zu implementieren, um dem weiter wachsenden Personalmangel entgegenzuwirken.
