Aufgrund ihres neuartigen Wirkmechanismus, welcher in erster Linie auf einer Aktivierung der intrinsischen Anti-Tumorzell-Aktivität von T-Zellen beruht, ergibt sich ein völlig neues Nebenwirkungsspektrum im Vergleich zu konventionellen Tumortherapien. Eine dieser Nebenwirkungen ist die Immun-Checkpoint-Inhibitor-vermittelte Hypophysitis (ICIH). Diese tritt vor allem vermehrt unter Therapie mit CTLA-4-Inhibitoren auf. Während es bei PD-1-/PD-L1-Inhibitoren hauptsächlich zu einer isolierten zentralen Nebenniereninsuffizienz (AI) kommt, verursacht ein CTLA-4-ICIH häufig multiple Hormondefizite, überwiegend im Bereich der Adenohypophyse. Da es sich bei der Hypophysitis um eine potenziell lebensbedrohliche Komplikation der ICI-Therapie handelt, sind ein rechtzeitiges Erkennen und eine entsprechende therapeutische Intervention von herausragender Bedeutung.
