EVARs können Langzeitkomplikationen haben, weswegen eine engmaschige Nachsorge erforderlich ist. Der häufigste postoperative Interventionsgrund ist die Entwicklung von Endoleaks. Zu differenzieren sind fünf Typen. Während Typ -I und -III- Endoleaks sofortige Behandlung benötigen, verlaufen Typ- II- Endoleaks, mit der höchsten Inzidenz, meist beninge mit spontaner Regression, können jedoch durch kontinuierliche Sackexpansion zu einer Aneurysmaruptur führen. Trotz seiner geringen Größe treten starke Schmerzen auf, die häufig nachts zunehmen. Charakteristisch für diese Knochenläsion ist das gute Ansprechen dieser Symptomatik auf Acetylsalicylsäure (ASS). Das charakteristische Merkmal des Osteoidosteoms im Röntgen ist eine zentrale runde bis ovale Aufhellung, welche als Nidus bezeichnet wird. Eine Dauertherapie mit ASS stellt aufgrund der Nebenwirkungen eine unzureichende Therapieform dar. Ein offen-chirurgischer Therapieansatz kann durch Entfernung großer Knochenanteile zur Instabilität und langer Rekonvaleszenz führen und birgt zahlreiche Risiken.
