Der H2FPEF-Score, welcher auf klinischen Daten sowie Echokardiographie basiert, wurde für die Vorhersage eines HFpEF eingeführt. Die Belastungsuntersuchung kann eine frühe Linksherzinsuffizienz aufdecken, die in Ruhe nicht erkennbar ist. Unser Ziel war es, den diagnostischen Wert des H2FPEF-Scores zur Vorhersage eines erhöhten PAWP bei Belastung und seinen Zusammenhang mit Überleben, Belastbarkeit und Dyspnoe zu untersuchen.\r\nMethoden: Wir analysierten retrospektiv PatientInnen mit einem Ruhe mPAP <25 mmHg und mindestens einem Jahr Nachbeobachtungszeit, welche zwischen März 2008 und Dezember 2021 in unserer PH-Klinik eine symptomenbegrenzte Belastungsrechtsherzkatheteruntersuchung hatten. Um die Vorhersagekraft des H2FPEF Scores für einen erhöhten PAWP bei Belastung und das Überleben zu analysieren, wurden Receiver-operating-curve Analysen durchgeführt, für die Assoziation mit dem Überleben Cox-Regressionen, und Assoziationen mit Belastungsparametern sowie des Dyspnoe Indexes Pearson Korrelationen.\r\nErgebnisse: Wir schlossen in die Studie n=177 PatientInnen (n=109 weiblich, mittleres Alter 66 (IQR55–74) Jahre, mPAP 19 (IQR16–22) mmHg, PAWP 8 (IQR6–11) mmHg) ein. Der H2FPEF-Score konnte einen erhöhten PAWP unter Belastung, (AUC:0,694 (95%CI:0,575–0,814, p=0.003) vorhersagen, nicht aber einen erhöhten PAWP in Ruhe (AUC:0,650 (95% CI:0,322–0,978, p=0,309)). Der H2FPEF-Score korrelierte signifikant mit dem peakVO2 (r=-0,222, p=0,023), der Distanz der Gehstrecke (r=-0,426, p<0,001), dem BDI (r=0,269, p=0,001) und dem NT-proBNP (r=0,495, p<0,001). Die Mortalitätsanalysen zeigten, dass der H2FPEF-Score und maximale PAWP >25 mmHg (AUC:0,569 (95%CI:0,439–0,699), p=0,320) bzw. (HR:0,88(0,32–2,43), p=0,807) nicht mit der Mortalität assoziiert waren. Im Gegensatz dazu waren der maximale CO (HR:0,88 (95%CI:0,78–0,99), p=0,041); der TPG/CO slope (HR:1,16 (1,04–1,29), p=0,010), der PAWP/CO slope >2WU (HR:4,07 (1,35–12,32), p=0,013) und der mPAP/CO Slope >3WU (HR:3,99(1,14–13,95), p=0,030) signifikant mit der Mortalität verbunden. Lately, a connection between butyrate-producing bacteria and sleep quality has been discussed. The PROVIT study, as a randomized, double-blind, four-week, multispecies probiotic intervention study, aims at elucidating the potential interconnection between the gut’s metabolome and the molecular clock in individuals with major depressive disorder (MDD). The PROVIT-CLOCK study aimed to analyze changes in core clock gene expression during treatment with probiotic intervention versus placebo in fasting blood and the connection with the serum- and stool-metabolome in patients with MDD (n = 53). In addition to clinical assessments in the PROVIT study, metabolomics analyses with 1H Nuclear Magnetic Resonance (NMR) Spectroscopy (stool and serum) and gene expression (RT-qPCR) analysis of the core clock genes ARNTL, PER3, CLOCK, TIMELESS, NR1D1 in peripheral blood mononuclear cells (PBMCs) of fasting blood, were performed. The gene expression levels of the clock gene CLOCK were significantly altered only in individuals receiving probiotic add-on treatment. TIMELESS and ARNTL gene expression changed significantly over the four-week intervention period in both groups. Various positive and negative correlations between metabolites in serum/stool and core clock gene expression levels were observed. Changing the gut microbiome by probiotic treatment potentially influences CLOCK gene expression. The first cases of the disease were described in 2014, which is why there is not yet much literature available. The high mortality rate makes the analysis of causes of death particularly relevant.\r\nMethods: The study is divided into two parts. The first part consists of a systematic literature research focusing on causes of death from the disease. In the second part, a case report of a patient with anti-IgLON5 disease is presented.\r\nResults: The most common cause of death is “sudden death” with a percentage of 40 %. This is followed by “death due to complications”, which consists mainly of aspiration pneumonia, but also includes trauma from falls. Other causes are respiratory, cardiac and unknown causes. The patient in the case report demonstrates a cardiac cause of death.\r\nDiscussion: The strikingly frequent occurrence of “sudden death” raises the question of why so many patients experience it and what underlying causes exist. The assumption is that unrecognised cardiac causes are often responsible for “sudden death”. Human serum albumin (HSA) is the most abundant protein in blood and \r\nvarious other body fluids, such as cerebrospinal fluid (CSF). According to its redox state in terms of cysteine-34 (Cys-34), HSA can serve as an indicator of oxidative stress. In this diploma thesis, we aimed to analyze the redox state of HSA in serum and CSF of MS patients in comparison to controls, and to further determine potential associations with disease activity and severity. This should serve as a pilot study in the search for MS biomarkers.\r\n\r\nMethods\r\nWe performed high performance liquid chromatography (HPLC) on paired serum and CSF samples of 20 MS patients and 21 symptomatic controls, to determine the percentual allocation of HSA to the fractions human mercaptalbumin (HMA), human \r\nnon-mercaptalbumin 1 (HNA1) and human non-mercaptalbumin 2 (HNA2). HMA is the reduced form of the protein, with a free thiol group on Cys-34. In HNA1, HSA is reversibly oxidized with Cys-34 as disulfide together with another thiol, whereas HNA2 is irreversibly oxidized with Cys-34 as sulfinic or sulfonic acid. We then tested for differences in HSA fraction allocation between groups and between serum and \r\nCSF, as well as for associations of HSA redox status with disease parameters reflecting activity and severity.\r\n\r\nResults\r\nWe did not find an overall significant difference in HSA redox state between MS patients and controls, although CSF HNA2 showed a trend to higher fractions. In CSF, HMA fractions were significantly higher than in serum, while HNA1 and HNA2 fractions were lower. We found significant associations of albumin redox state in serum with physical disability in remission in MS patients, as well as significant associations of albumin redox state in CSF and serum and disease activity. Additionally, there were some significant correlations of HSA redox state in both \r\ncompartments with age, as well as significant correlations of HSA fraction allocation in serum with lactate in CSF.\r\n\r\nConclusion\r\nIn conclusion, our data affirm the involvement of oxidative stress in MS pathophysiology. This study should serve as a basis for further investigation of HSA in MS, particularly in larger cohorts and patients with more advanced disease \r\nstages. Dafür verantwortlich war vermutlich die fehlerhafte Studienmethodik, mit Rekrutierung auf Basis pathologischer Atemtests, also einer Malabsorption – als Erfolgsparameter wurde hingegen die Symptoma-tik überprüft, also die Intoleranz. Mittlerweile ist allerdings bekannt, dass etwa die Hälfte der Patienten mit Malabsorption keine Symptome einer Intoleranz hat und daher vermutlich bei diesen Patient*innen auch kein Therapieeffekt zu erwarten ist. \r\nZiel der Studie: Im Rahmen dieser retrospektiven Studie wurde der Effekt einer Diät bei Patient*innen untersucht, bei denen eine Laktoseintoleranz (LIT) oder Fruktoseintoleranz (FIT) mittels des validierten „adult carbohydrate perception questionnaire, aCPQ“ diagnosti-ziert worden war. \r\nMethodik: 41 konsekutive Patienten (23 LIT, 8 FIT, 10 LIT und FIT, 30 weibl., 11 männl., Alter 36 ± 17 Jahre) erhielten eine schriftliche Diätanleitung. Die Symptome (Durchfall, Windabgänge, Blähungen, Schmerzen und Übelkeit) der Patient*innen wurden mit Hilfe eines modifizierten IBS-SSS Fragebogens vor der Diät und nach 58 ± 22 Tagen Diätdauer erfasst und nach Überprüfung der Normalverteilung mittels t-Test für zwei verbundene Stichproben verglichen. Die Zahlenangaben entsprechen Mittelwert ± Standardabweichung. \r\nErgebnisse: Durch die Diät kam es zu einer Abnahme des VAS-Wertes um 67 ± 37 % vom Ausgangswert vor Diät (p < 0.001).
