Die Inzidenz der aSAB beträgt 9.1 pro 100.000 Personen pro Jahr. Die SAB hat eine sehr hohe Mortalitätsrate, bis zu 40% der SAB-PatientInnen versterben innerhalb der ersten 30 Tage nach dem Ereignis. 25 bis 50 % der Überlebenden bleiben schwer behindert. Elektrolytstörungen sind eine sehr häufige Komplikation nach aSAB, welche die Mortalitätsrate erhöht und das Outcome verschlechtert. Methode: Diese Diplomarbeit ist eine systematische Literatursuche, die mit Hilfe von Recherchen in PubMed und dem Literaturstudium anatomischer, pathologischer und klinischer Fachbücher durchgeführt wurde. Von über 150 auf das Thema bezogenen Studien und Artikeln, die in namhaften Fachjournalen veröffentlicht wurden, fanden 90 Eingang in die Diplomarbeit. Die Studien zeigen, dass die häufigste Elektrolytstörung bei SAB-PatientInnen die Hyponatriämie(20%) ist, die meistens durch ein SIADH bedingt ist.Die Therapie der Hyponatriämie infolge eines SIADH erfordert eine Flüssigkeitrestriktion, was zu Vasospasmen führen kann. Mit der Schwere der Hyponatriämie erhöht sich die Mortalität. Hypernatriämie kommt bei annähernd 20% der SAB-PatientInnen vor, steigert die Mortalität und es kommt oft zu kardialen Komplikationen. Störungen des Kaliumhaushalts sind weitere sehr häufig auftretende Elektrolytstörungen. Elektrolytstörungen treten bei SAB-PatientInnen häufig auf. Die Pathogenese ist noch nicht vollständig geklärt. Eine anerkannte Hypothese ist, dass Elektrolytstörungen zu Vasospasmen und anderen Komplikationen wie Hirnödemen führen. Ein optimales Management der Komplikationen der aSAB im klinischen Setting und eine individuell angepasste Narkose während neurochirurgischer oder interventionell radiologischer Eingriffe ist nötig, um die Sterblichkeit dieser schweren Erkrankung zu senken und neurologische Defizite gering zu halten. Aufgrund der zahlreichen Einflussfaktoren auf den klinischen Verlauf, wie Alter der PatientInnen, Komorbiditäten, Schweregrad der aSAB, Zeitpunkt der Diagnose. Ersttherapie und Unterschieden in der Behandlung (neurochirurgisch vs. interventionell angiographisch) ist es schwer, allgemein gültige Aussagen anhand der vorliegenden Studien zu treffen. Hier muss noch im Rahmen größer angelegter Multicenterstudien Forschung geleistet werden, um praktische Leit- und Richtlinien erstellen zu können. Chronic RV pressure overload leads to development of RV dysfunction, which culminates in heart failure. Mechanisms responsible for development of RV dysfunction are poorly investigated. One of the reasons could be the fact that the currently used animal models do not resemble the complexity of RV dysfunction in full and therefore are not satisfactory. The aim of the work is by performing a literature search elucidate some features of RV dysfunction, which are not reflected by existing animal models. Moreover, some aspects of RV pathophysiology, which are less investigated, should be identified. The obtained knowledge should deliver ideas for improvement of our models.\nThe literature search was performed for this work. Results of our group were compared with findings of other groups.\nCurrently, investigations on RV dysfunction are at the level of characterizeing it. Hypoxia induced RV dysfunction model appears as a model with fully compensated pressure overload. Pulmonary artery banding model (PAB) induces pronounced adaptive RV remodeling, which causes moderate dysfunction. Monocrotalin (MCT) and Sugen5416 (SU/HOX) induced models cause severer dysfunction. Although, none of the models demonstrates the phenotype of cardiac decompensation. Investigations using molecular biology approach show contribution of different signaling pathways partly known from the research on the left ventricular dysfunction. Characterization using invasive and non-invasive techniques demonstrates reduced cardiac function in PAB, MCT and SU/HOX models. At the same time, the aspect of RV/PA coupling remains mainly untouched.\nSummary. None of the investigated animal models resembles the clinical picture of cardiac failure. The machanisms of RV dysfunction seem to be partly specific for RV. Due to the widespread use of PSA screening tests, most of the patients are diagnosed at an early stage and are on active surveillance or cured with radical surgery.\r\nIn fact, up to 40% of pre-treated prostate cancer cases progress to castration resistant prostate cancer (CRPC), which is associated with a very high mortality and limited therapeutic options. These patients are no longer sensitive to androgen deprivation therapy and new drugs such as abiraterone and enzalutamide can only postpone the disease. There is an urgent need for predictive and prognostic biomarkers as well as cancer driver genes that contribute to therapy decisions and outcome and provide information about the molecular landscape and heterogeneity of the tumor.\r\nWith the emergence of next-generation sequencing and the analysis of cell-free DNA (cfDNA), referred to as liquid biopsy, a powerful tool for the identification of biomarkers and cancer driver genes has become available. Since cfDNA is released into the circulation as nucleosome-protected DNA fragments, our group established a method based on this finding, called Nucleosome Position Mapping (NPM). Using this approach, the gene expression status can be inferred from cfDNA in order to identify cancer driver genes in predisposed regions. Therefore, a high coverage sequencing of cfDNA and data analysis using a bioinformatic gene prediction algorithm is essential.\r\nWe were able to demonstrate differences and similarities regarding copy number alterations from tissue biopsies vs. liquid biopsies from CRPC patients. Furthermore, we could show the potential of liquid biopsy for disease monitoring and identification of therapy resistance. The accuracy of the NPM algorithm could not be assessed with high confidence.
