However, outside standardized trials, considerable interscanner and intrascanner variability has to be expected for such measures. So far, the reliability of such brain measures in clinical practice has received relatively little systematic investigation. Our aim thus was to determine the effect of using different MRI protocols at different scanners on the measurement of lesion load and brain volumes metrics in clinical similar cohorts of MS patients of two different centers.\nMethods: \n 174 MS patients from two MS centers (3D MPRAGE MR scans of 111 patients from Graz and 2D SE MR scans of 63 patients from Linz) at baseline and follow-up (after a mean of 3.42+1.58 years) were analyzed by using SIENAX and SIENA. Patients were comparable regarding age, disease duration, Expanded Disability Status Scale, and the proportion of patients on disease-modifying therapy. Variability between 2D and 3D data segmentation was estimated by the difference in brain metrics, percentage brain volume change (PBVC), coefficients of variation and percentage differences in volumes of brain metrics.\nResults: \n Lesion load estimation showed similar T2 hyperintense lesion load but significantly different T1 hypointense (“black hole”) lesion load between the two cohorts (0.35 cm3 in Graz and 0 cm3 in Linz). Results from SIENAX varied significantly for all brain metrics except the global brain volume between the Graz and Linz cohorts (white matter volume 768 cm3 and 854 cm3, grey matter volume 797 cm3 and 706 cm3, respectively). Mean percentage differences showed that despite similar results concerning the global brain volume, the ratios of white and grey matter volumes between the two centers were completely different (-12% in grey and 10% in white matter between the cohorts). SIENA analyses demonstrated that PBVC was significantly lower in Linz cohort than in Graz (-0.19% and -0.39%, respectively). \nConclusions: \n Even in cohorts of MS patients with similar clinical characteristics, obtaining MRI data at different scanners (despite locally standardized protocols) yields significant differences in several lesion and brain volume metrics. From etiological point of view, traumatic, iatrogenic and spontaneous CSF fistulas can be distinguished. One of the most dangerous related complications is bacterial meningitis, thus an accurate and fast diagnosis is important. For this purpose, radiological and laboratory methods are used. However, diagnostics can be quite challenging, since a small defect size is a limitation to radiological methods. Accordingly, nasal discharge analysis to examine CSF specific compounds seems to be a suitable prospective method.\r\nAmong all CSF proteins, ß- trace protein (BTP) is most interesting. It is a protein with impressively high intrathecal fractions, CSF/Serum ratio and shows relatively high concentrations in CSF. However, recommended cut-off values in the literature vary widely between 0.244 and 6 mg/l. The main aim of this diploma thesis is to improve the diagnostic accuracy of determining CSF leakage and new evaluation of existing cut-off values in nasal discharge. A second aim was the retrospective calculation of specificity and sensitivity using NPV and PPV. \r\nMethods\r\nBTP concentrations in ventricular CSF (V-CSF) and lumbar CSF (L-CSF), (n = 24 each), and in nasal discharge from known healthy volunteers (n = 17) with admixture of gradually prediluted (pure, 2, 3 and 4 Fold) V- or L-CSF was measured via nephelometry.\r\nIn order to evaluate BTP as a suitable marker, BTP concentrations in serum (n=17), nasal discharge for determining BTP concentration of healthy population (n = 188) and nasal discharge of operative examined patients (n = 75) were statistically analyzed.\r\nSince some groups revealed to be not normally distributed (evaluated via Kolmogorov-Smirnov test), non-parametric tests were preferred. For determining the significance between groups, Kruskal-Wallis and Mann-Whitney-U-Test were used; for investigation of correlations, Spearman’s correlation coefficient was calculated. BTP cut-off value in nasal discharge was determined via ROC Curve. Sensitivity, specificity, positive and negative predictive values were calculated via cross tables. \r\n\r\nResults\r\nThere was a significant difference (p < 0.001) detectable between the means of V-CSF (10.94 mg/l) and L-CSF (25.69 mg/l) BTP concentrations, as well as serum (0.52 mg/l) and nasal discharge (0.29 mg/l) values. An inverse correlation between prediluted samples was detected via Spearman’s correlation coefficient (p < 0.001). The ROC curve (Figure 16) represented an AUC of 0.765 with a significance of p < 0.001. Additionally, a sensitivity of 100% with a PPV of 75.7% and a specificity of 22.7% with a NPV of 100% could be described (p < 0.001).\r\nConclusion\r\nThe results of this study suggest the acceptance of the cut-off value of 1.3 mg/l.\r\nWe conclude that BTP seems to be a promising laboratory marker of even minor rhinoliquorrhea, which however requires further evaluation with consideration of possible limitations. Certain patient related features like bacterial meningitis, impaired renal function and multiple skull injuries can lead to BTP value changes in the respective sample. Voice overload and vocal misuse can lead to structural changes of the vocal folds (VF) and thus to hoarseness and limited capacity of the voice. These benign VF lesions occur in various forms. In the thesis on hand, five typical lesion types are compared regarding their incidence, their age- and gender dependency and the VF side affected: VF nodules, polyp, cyst, granuloma and Reinke´s edema.\r\n\r\nIn a retrospective cohort study, the data of all patients who had visited the Division of Phoniatrics of the ENT University Hospital Graz over a period of 13 years due to voice complaints and had been diagnosed with one of the five selected lesion types were recorded. Based on video-documented laryngoscopies, a detailed review process was conducted according to strictly defined criteria. Subsequently, the included data was subjected to statistical analysis.\r\n\r\nThe study included 535 lesions, with both genders affected equally often. However, while the included female patients were diagnosed with VF nodules and Reinke’s edema in one third each, more than half of the male patients were affected by VF polyps. The majority of the patients were of working age, while only a third of the diagnoses concerned children, adolescents or elderly. Each of the lesion types examined showed a significant association with gender, and with the exception of VF cysts, all diagnoses were also significantly related to age. Among the unilateral lesions, the right and left VF were affected equally often overall, but the distribution of the affected side was found to be gender-related.\r\n\r\nA comparison of the results with previous studies was complicated by the fact that nomenclature, definition and classification framework of benign VF lesions are still handled inconsistently today. In particular, different perspectives on these structural alterations (endoscopic, operative, histological) lead to sometimes contradictory results. Further research is needed here, but the results of the present study also have practical impact. Although multiple studies have demonstrated the effectiveness of MI in the second or third trimester of pregnancy, the optimal timepoint to afford the newborn maximum protection is yet to be elucidated. The primary aim of this thesis was to model the impact of timing of MI on infant antibody titers at birth in order to determine the optimal timeframe between vaccination and delivery. We conducted secondary analysis on a cohort of mother-infant pairs (n=698) vaccinated between 13.4-37.1 weeks gestation. Data were taken from 4 parent studies on pertussis vaccination during pregnancy. We used natural cubic splines and linear mixed models to model the association of both gestational age at vaccination and the interval between vaccination and delivery, with cord antibody titers. The association was investigated in antibody titers to pertussis antigens included in the vaccines administered during pregnancy. Our results show that term-born infants on average achieve the highest antibody titers at birth if wom-en are vaccinated earlier than the 31st gestational week. When considering both term and preterm de-liveries, our results suggest that an interval of at least 7.5 weeks is required between vaccination and delivery before the highest cord antibody titers are achieved. The models suggest that vaccinating ear-lier than these will also afford the infant equally high titers. Vaccinating in the second and early third trimester affords the infant the highest antibody levels at birth.
