Sowohl die Allgemeinanästhesie als auch die Regionalanästhesie sind mit spezifischen Vor- und Nachteilen verbunden, die je nach den unterschiedlichen Komorbiditäten der Patientinnen und Patienten sorgfältig berücksichtigt werden müssen. Dieses Thema besitzt eine hohe klinische Relevanz aufgrund der großen Anzahl an jährlich durchgeführten elektiven Routineoperationen, insbesondere im Bereich orthopädisch-prothetischer, transurethraler und gynäkologischer Operationen. Ziel dieser Arbeit war es, den Einfluss präexistenter Komorbiditäten auf die Wahl des Anästhesieverfahrens zu untersuchen und damit zu einem besseren Verständnis des Wahlprozesses in der klinischen Praxis beizutragen.",

Diese Standards werden regelmäßig international diskutiert und fallweise in Frage gestellt. Daraus ergab sich das Ziel, die eigenen Ergebnisdaten zu überprüfen und mit den Qualitätsstandards der ESVS abzugleichen.",

Over the past decades, the prevalence of adiposity has increased exponentially worldwide, but also physical inactivity is a key driver, making MASLD a growing global health issue. This is not only because more cases are being identified in younger patients, but also due to its connection to rising rates of type 2 diabetes mellitus (T2DM) and cardiovascular complications. Bile salts play an important role in the human body, not only by helping digestion, absorbing dietary fats and fat-soluble vitamins, they also act as hormone-like signals coordinating the whole-body-lipid balance. The current thesis analyzed bile acid profiles in overweight versus control male and female adult patients.",

            "title": "Hidden Symptoms in Multiple Sclerosis: Cognitive Impairment and Fatigue.",
            "abstract": "The majority of people with multiple sclerosis (pwMS) suffer from \"hidden\" symptoms, with fatigue (78%) and cognitive impairment (65%) being the most commonly reported. These symptoms significantly affect patients' quality of life, working ability, and social integration. They are among the most debilitating features of MS, and more profound understanding could improve the management of these symptoms. Therefore, this thesis investigated three key areas: (1) the relationship between cognition and physical disability, (2) the application of an iPad®-based cognitive assessment in a clinical setting, and (3) psychological and brain magnetic resonance imaging (MRI) factors potentially influencing fatigue. The first study examined if cognition serves as a potential prognostic marker for physical impairment and disease progression in pwMS. The findings suggest that cognitive function, particularly information processing speed, is a crucial marker for the evolution of MS and could serve as an independent prognostic marker for physical impairment and progression due to shared underlying brain pathology. This highlights the importance of integrating neuropsychological assessments into standard clinical care to better monitor MS progression. The second study evaluated a self-administered iPad®-based processing speed test (PST) for use in clinical routine settings. The results showed that the PST demonstrates high feasibility and strong validity, correlates well with the established Symbol Digit Modalities Test (SDMT) and other cognitive assessments, as well as with MRI parameters and psychological factors, such as level of depression, anxiety, and fatigue. However, the study found that the commonly used threshold of -1.5 standard deviations for the PST is not appropriate in Europe, as it identified fewer cases of impaired processing speed compared to the SDMT. The results underscore the importance of comprehensive neuropsychological assessment, but also suggest that the PST could be a valuable tool for resource-efficient cognitive screening in pwMS. In the third study, we examined how psychological factors and brain MRI metrics are associated with fatigue in pwMS and healthy controls (HC). We found that depression and self-efficacy significantly affect fatigue severity in both pwMS and HC. Additionally, white matter integrity (a microstructural MRI metric) along the cortico-striato-thalamo-cortical loop explained incremental variance of fatigue only in pwMS. Overall, findings of this thesis underlined that cognitive impairment and fatigue are common in MS. They are not isolated features but closely related to other MS-symptoms, adding information about progression of the disease. Furthermore, our research critically evaluated a more efficient cognitive assessment in clinical routine. Moreover, we found that not only neurobiological changes lead to fatigue, but malleable psychological factors influence perceived fatigue severity, providing crucial information for the development of targeted treatment.",

Ursprünglich wurden diese für die alternative Behandlung von chronischen Schmerzen konzipiert, stehen jedoch mittlerweile im Fokus vieler anderer Bereiche. Da chronische Kreuzschmerzen den größten Anteil an chronischem Schmerzleiden verursachen und mit sozioökonomischen Herausforderungen einhergehen, sind die Auswirkungen von MBI auf chronische Kreuzschmerzpatient*innen von besonderem Interesse. Ziel der vorliegenden Arbeit ist es, diese Auswirkungen zu beleuchten und MBI als Behandlungsalternativen bei chronischen Kreuzschmerzpatient*innen zu diskutieren.",

            "abstract": "Einleitung: Der Schlaganfall ist weltweit die führende Ursache dauerhafter Behinderungen im Erwachsenenalter. Mithilfe der mechanischen Thrombektomie kann im Sinne einer endovaskulären Schlaganfalltherapie eine Rekanalisationsrate von rund 90 % beim akuten intrakraniellen Großgefäßverschluss erreicht werden, was einen beachtlichen therapeutischen Fortschritt bedeutet. Der klinische Outcome wird überwiegend mittels „modified Rankin Scale“ (mRS) bewertet. Zusätzlich zu diesem klinischen Beeinträchtigungsgrad soll in dieser Arbeit ein besonderes Augenmerk auf die Lebensqualität und das kognitive Outcome nach Thrombektomie bei Patient*innen mit schwerem Schlaganfall gelegt werden.",

Manche Patienten lehnen dies trotz Empfehlung ab. In der vorliegenden Arbeit wurde untersucht, wie sich das klinische Outcome bei Patienten mit Blasensteinen verhält, bei denen eine isolierte transurethrale Blasensteinentfernung ohne operative Behandlung der Prostata durchgeführt wurde.",

While invasive blood pressure monitoring is the in-hospital gold standard, its prehospital use remains debated. Time delays until definitive in-hospital treatment are feared and research evaluating whether it improves blood pressure control compared to non-invasive blood pressure monitoring has not yet been performed. Therefore, this thesis aimed to evaluate both potential negative effects (delays in care) and potential positive effects (improved blood pressure control) of invasive blood pressure monitoring in patients with acute brain injury.",

            "abstract": "Lower extremity artery disease (LEAD) affects over 200 million people worldwide. The advanced stage of this disease is referred to as chronic limb threatening ischemia (CLTI) and is associated with increased morbidity and mortality. This is reflected in a one-year mortality rate of approximately one quarter among CLTI patients, while amputation rates reach about 30% within one year. Despite the significant socioeconomic impact of LEAD, it remains largely underrepresented compared to other cardiovascular research fields. Cardiac biomarkers such as troponin T and N-terminal pro-brain natriuretic peptide (NT-proBNP) are well-studied and firmly established in the cardiology setting. However, their significance in the context of LEAD is largely unexplored. Only a few studies have investigated cardiac biomarkers in LEAD patients, demonstrating an association between LEAD and elevated cardiac biomarker levels. The aim of this study was to assess differences in cardiac biomarkers between the stages of LEAD and to evaluate the predictive value of cardiac biomarkers in patients with LEAD.",

            "abstract": "Zur Beurteilung der körperlichen Leistungsfähigkeit von Patient:innen wird dem Goldstandard entsprechend oft die maximale Sauerstoffaufnahme (VO2max) verwendet. Um sie zu erheben, ist es notwendig, eine aufwändige Untersuchung auf einem Ergometer mit einer Atemmaske durchzuführen, insofern keine Kontraindikationen gegeben sind. Wir untersuchten daher den Zusammenhang der VO2max mit einem einfacher zu erlangendem Testparameter: der maximalen Gehstrecke des 6-Minuten-Gehtests, um eine Grundlage zu schaffen, mit diesem in der klinischen Praxis eine gute Aussage über die körperliche Verfassung von Patient:innen treffen zu können.",
