            "title": "The impact of Nr4a1 on the dissemination of aggressive lymphoma.",
            "abstract": "Aggressive lymphoma are a heterogeneous group and can further be divided into\r\nmany different subgroups like Burkitt lymphoma, Burkitt like lymphoma, mantle cell\r\nlymphoma, and diffuse large B cell lymphoma (DLBCL). In general, aggressive\r\nlymphoma have a poor long-term survival rate. Therefore, researchers are looking for\r\nmolecular mechanisms, that can serve as a target for novel therapies.\r\nIn aggressive lymphomas, NR4A1 possesses tumor suppressive functions. In the\r\nEμMyc transgenic lymphoma mouse model, we observed that loss of Nr4a1\r\naccelerates lymphomagenesis and a higher tumor cell infiltration of bone marrow and\r\nspleen. The aim of this study was to comprehensively study the impact of Nr4a1 loss\r\non lymphoma dissemination. Therefore, we transplanted EμMyc Nr4a1-/- and EμMyc\r\nNr4a1+/+ lymphoma cells into immune-competent as well as immunodeficient mice\r\nand determined lymphoma cell infiltration of the kidney, liver, brain, lung, spleen,\r\nthymus, and lymph node by histological examination.             "title": "Der Einfluss von PECS- Blockaden auf postoperative Schmerzen und Wohlbefinden von Patientinnen nach plastischen Mamma Operationen.",
            "abstract": "Zusammenfassung\r\nZiele: Erhebung über die Zufriedenheit und das Wohlbefinden bei Patientinnen nach Operationen an der Brust, wobei zwei verschiedene Anästhesieverfahren verwendet wurden. Außerdem Bewertung der wichtigsten postoperativen Faktoren wie Schmerz und Übelkeit, des Schmerzmittelverbrauchs im Krankenhaus und des Auftretens neuropathischer Schmerzen oder möglicher Einschränkungen im alltäglichen Leben aufgrund der Operation. Korrelationen wurden ebenfalls gesucht. \r\nStudiendesign: Explorative Umfrage mittels eines Fragebogens unter Frauen nach einer Brustoperation, die an einem Universitätsklinikum in Mitteleuropa durchgeführt wurden. Je nach Anästhesieverfahren, wurden die Patientinnen in zwei Gruppen eingeteilt und die Antworten verglichen.  \r\nPrimäre Zielgröße: Der Einfluss einer zusätzlich zur Allgemeinanästhesie durchgeführten lokalen Anästhesie der Pectoralnerven mittels PECS- Blocks auf die Ausprägung der Zufriedenheit und des Wohlbefindens nach Brustoperationen. \r\nResultate: Insgesamt gaben die befragten Patientinnen (n = 30) eine hohe Zufriedenheit und ein relativ hohes Wohlbefinden an, unabhängig von der verwendeten Anästhesieform. 97% der Patientinnen waren ziemlich oder stark zufrieden. Das Wohlbefinden war bei 60% der Patientinnen ziemlich oder stark ausgeprägt. Wir fanden bei der Analyse der Antworten auf die gestellten Fragen keine signifikanten Unterschiede zwischen den Gruppen. Der Opioidverbrauch war in der Gruppe mit dem PECS- Block hingegen signifikant niedriger. Es bestand eine moderate Korrelation zwischen dem Wohlbefinden, dem körperlichen Unwohlsein und dem Schmerz im Operationsgebiet, die Zufriedenheit korrelierte hingegen mit keinem anderen Faktor. Der zusätzliche PECS- Block korrelierte mit postoperativer Übelkeit und dem Opioidverbrauch. \r\nFazit: Der PECS- Block führte bei den Patientinnen in unserer Befragung zu keiner höheren Zufriedenheit oder höherem Wohlbefinden. Bei der Betrachtung aller Antworten konnte von uns ein geringer Einfluss der Nervenblockade auf die meisten untersuchten Faktoren (z.B. neuropathischer Schmerz, Einschränkungen im alltäglichen Leben) festgestellt werden. Aufgrund der Einfachheit des Verfahrens, dem breiten Anwendungsspektrum und der niedrigen Komplikationsrate ist der PECS- Block ein relativ neuer und effektiver Weg, um die Verwendung von systemisch wirksamen Opioiden zu verringern.",

            "title": "Donor Site Morbidity and Quality of Life After Microvascular Head and Neck Reconstruction with a Chimeric, Thoracodorsal, Perforator-Scapular Flap Based on the Angular Artery (TDAP-Scap-aa flap)",
            "abstract": "Background: Extensive defects in the head and neck area often require the use of advanced free flap reconstruction techniques. In such cases, where both soft and hard tissues are part of the defect, the thoracodorsal perforator-scapular free flap based on the angular artery (TDAP-Scap-aa flap) has recently been described as a novel microvascular treatment option in the head and neck reconstruction. However, postoperative donor site morbidity and/or quality of life assessments associated with this novel reconstruction technique have not been investigated yet.\r\n\r\nMaterials and Methods: In this retrospective study, the quality of life and donor site morbidity were postoperatively evaluated over a five-year period (2016–2020) using the standardized SF-36 and the DASH questionnaires in 20 selected cases (n = 20). The postoperative quality of life and donor site morbidity were assessed approximately one year (10–14 months) after extensive head and neck defect reconstruction with the TDAP-Scap-aa flap. The acquired results were compared with the already established data of the healthy German and U.S. norm population (healthy control group). Furthermore, an electronic clinical chart review of the included patients was conducted to collect the demographic data and the harvesting details of the surgery. \r\n\r\nResults: On average, the harvested microvascular free flaps consisted of 7,8 ± 2,1 cm bone and 86 ± 49,8 cm2 soft tissue components. At the donor site (subscapular region) only mild morbidity was observed postoperatively with a mean DASH score of 21,74 ± 7,3 points. This result can be assigned to the second lowest tier in the five-tier category rating of donor site morbidity (very mild to very severe). When comparing the patients’ postoperative quality of life to the healthy German norm population, the observed SF-36 values were within the upper third (> 66 %) of the established norm values in almost all quality of life subcategories.\r\n\r\nConclusions: The actual impact of microvascular flap harvest on the patients’ quality of life is hardly assessable due to various confounders that occur or may occur within the clinical routine or the preoperative environment. However, the mild donor site morbidity and the observed quality of life located in the upper third of the healthy norm population indicates only minor postoperative impairment when using the novel TDAP-Scap-aa free flap. This flap is therefore suitable for the treatment of advanced combined soft and hard tissue head and neck defects.",

            "title": "Lumbopelvic Fixation And Its Limitations After Unstable Pelvic Ring Injury",
            "abstract": "Background: Unstable pelvic ring fractures have a low incidence and are therefore rarely reported in the literature, especially regarding the relatively new method of lumboplevic fixation. After presenting the anatomy of the region and the most common classifications and therapies for pelvic ring injuries, the aim of the retrospective study described in this thesis is to evaluate the outcome of this surgical treatment within a larger study group and prolonged follow-up of at least 12 months. Methods: All patients that received lumbopelvic fixation at the Department of Orthopaedics and Trauma, Medical University Graz from January 2013 until October 2019, due to unstable pelvic ring fractures caused by trauma were retrospectively included. Patients had to be at least 18 years old and the fracture had to be classified as C-type fracture according to the AO/OTA classification. Fifty-three patients met the inclusion criteria. The author of this diploma thesis reviewed all medical records and original radiographic data retrospectively in a nonblinded fashion.\r\nResults: Thirty-nine out of 53 patients noted a disturbance. A disturbance was defined as: Pain in the region of the instrumentation, lack in range of motion or complications that led to hardware removal. Median length until disturbance was 236 days. In total, 81% of patients reached bony consolidation, within a median time of 267 days. Around 70% of patients requested hardware removal, due to disturbances or complications. In total, 18 patients had postoperative complications with 4 of them developing infection and 6 implant loosening or breakage. The group “No hardware removal” was older (mean=57), than the group with hardware removal (mean=41). Furthermore, 12 out of the 14 patients who did not experience disturbance by the implants were overweighted and/or older than 50 years. Conclusion: A CT-scan and hardware removal surgery may be scheduled in advance 12 months after lumbopelvic fixation surgery, since most of the patients will have achieved full bony consolidation and will have a disturbance up to this point. This strategy should especially apply to young and healthy-weighted patients.",

            "title": "Association of diabetes-specific markers and functional capacity in people with type 1 diabetes",
            "abstract": "Background:\r\n\r\nType 1 diabetes (T1D) is a chronic condition that implicates serious short-term and long-term complications, above all microvascular and macrovascular disease, which contribute to the major morbidity and mortality associated with T1D. Physical activity poses an important cornerstone in the prevention of cardiovascular disease. However, even though it is well established that physical exercise is an effective therapeutic tool, it is not entirely clear whether and how diabetes-specific markers, especially HbA1c, affect functional capacity in people with T1D. Therefore, the objective of this thesis was to focus on the relations between diabetes-specific markers (HbA1c, diabetes duration, C-peptide, anthropometric and demographic measurements) and functional capacity as well as cardiorespiratory markers during cardiopulmonary exercise (CPX) testing.\r\n\r\nMethods:\r\n\r\nData was collected at the Medical University of Graz from 15 individuals with T1D participating in a randomized controlled cross-over trial investigating two different ultra-long-acting basal insulins’ performance around multiple spontaneous 60-minute exercise sessions, during which CPX tests were performed. The laboratory and cardiorespiratory markers obtained from the CPX tests were analyzed for correlations as well as between-group differences regarding the first lactate turn point (LTP1), the second lactate turn point (LTP2) and maximum exertion depending on the participants’ HbA1c, diabetes duration, C-peptide status, age, body mass index (BMI) and sex.\r\n\r\nResults:\r\n\r\nNo significant correlations were found between HbA1c and maximum oxygen uptake (VO2max), neither for absolute VO2max (“good” HbA1c [≤ 7.5%] group: R = 0.40, p = 0.33, “poor” HbA1c [> 7.5%] group: R = 0.15, p = 0.75) nor relative VO2max (“good” HbA1c: R = 0.24, p = 0.565, “poor” HbA1c: R = 0.10, p = 0.828). Also between-group comparisons (“good” vs. “poor” HbA1c) for mean absolute VO2max (2.71 ± 0.93 vs. 2.82 ± 0.86 L/min, p > 0.999) and relative VO2max (37 ± 9 vs. 34 ± 10 ml/min/kg, p = 0.532) showed no significant differences. Significantly higher relative VO2max values were observed in people with residual pancreatic beta cell function (positive C-peptide) vs. people with negative C-peptide (41 ± 10 vs. 31 ± 5 ml/min/kg, p = 0.032*). Also younger (≤ 41 years) individuals showed significantly higher relative VO2max compared to older (> 41 years) subjects (41 ± 9 vs. 31 ± 6 ml/min/kg, p = 0.030*) as well as correlations for absolute VO2max (R = 0.66, p = 0.074) and relative VO2max (R = 0.84, p = 0.008**). Diabetes duration and BMI did not demonstrate significant influence on absolute and relative VO2max. \r\n\r\nConclusion:\r\n\r\nIn this study, blood sugar control had no influence on functional capacity. Participants with residual pancreatic beta cell function and younger participants achieved higher VO2max. Diabetes duration and BMI were no significant performance predictors.",

            "abstract": "Malaria ist eine der häufigsten parasitären Infektionskrankheiten in den tropischen Regionen \r\nweltweit. 2019 erkrankten 229 Millionen und 409 000 Menschen verstarben an der Infektion. \r\nDie Erreger der Malaria sind die Plasmodien. Sie werden durch die Anopheles Mücke auf \r\nden Menschen übertragen, wo sie sich weiterentwickeln und zu einer Vielzahl an \r\nSymptomen führen können. Für eine erfolgreiche Behandlung ist die richtige Diagnostik von \r\nentscheidender Bedeutung. Die therapeutischen Ansätze sind vielseitig. Ziel dieser Arbeit \r\nist es, den aktuellen Forschungsstand der medikamentösen Therapie und Impfprophylaxe in \r\nBezug auf die Malariaerkrankung darzustellen. Für diese Arbeit wurde anhand von \r\npharmakologischer und mikrobiologischer Literatur sowie Online-Datenbanken eine \r\nLiteraturrecherche durchgeführt. Trotz zahlreicher Medikamente, die die Plasmodien \r\nbekämpfen, ist die Prophylaxe weiterhin der wichtigste Infektionsschutz. Um das Risiko \r\neiner Infektion und eines schweren Verlaufs zu reduzieren, werden neue Medikamente und \r\nImpfstoffe entwickelt. Ausgewählte, sich in Entwicklung befindliche Impfstoffe, wurden in \r\nBezug auf ihre Effektivität verglichen und bewertet. Der Impfstoff RTS,S/AS01 wurde als \r\nausreichend effektiv und sicher bewertet, um von der WHO als Impfprophylaxe empfohlen \r\nzu werden. R21/Matrix-M, ein sich in der Entwicklung befindender Impfstoff zeigt ebenfalls \r\nerste vielversprechende Ergebnisse, die durch weitere Studien bestätigt werden müssen.\r\nAktuell gibt es jedoch keinen Impfstoff, der eine ausreichende Effektivität aufweist, um \r\neinen flächendeckenden Schutz bieten zu können. Ob Impfstoffe zukünftig in der \r\nBekämpfung der Malaria eine wesentliche Rolle spielen werden, kann zum derzeitigen Stand \r\nnoch nicht abgeschätzt werden.",

            "title": "Postoperative pain after arthroscopic anterior cruciate ligament reconstruction.",
            "abstract": "Introduction:\r\nAnterior knee pain (AKP) is a very common complication after anterior cruciate ligament reconstruction (ACLR). Although AKP is a well-known complication, pain after ACLR has never been specifically classified or localized. The aim of this study was to classify and localize postoperative pain after arthroscopic ACLR and to investigate its influence on the subjective functional outcome and quality of life, taking the psychological state of the patients into account.\r\n\r\nMaterial and Methods:\r\nThis is a prospective observational study of 103 patients (mean age 32 years) undergoing arthroscopic ACLR with a mean follow-up of 3.1 years. All subjects underwent surgery at the University hospital of Graz (Austria) between March 2010 and August 2019. The data were collected once postoperatively via a self-designed electronic Case Report Form (eCRF) at cross-section. \r\n\r\nResults:\r\nKnee pain occurred mainly in the medial knee joint (61.2%) and in the popliteal fossa (43.7%). Medial knee pain was more common in men (67.1% vs. 44.1%). Popliteal knee pain was more common in men (51.3% vs. 22.2%) and younger patients. Patients with medial knee pain and popliteal fossa pain had significantly worse subjective functional outcome scores (IKDC, TAS, Lysholm score), lower SF-36 scores and more kinesiophobia compared to the rest of the study population. Negative correlations between IKDC and Lysholm score with the NRS scale were not mediated by psychological parameters (FESS, TSK, PCS).\r\n\r\nDiscussion:\r\nPain sensations after arthroscopic ACLR are a chronic condition in the medial knee joint and the popliteal fossa. Even with low pain intensity, these pain sensations have a significant impact on function and quality of life. The origin and prevalence of these pain sensations should be further addressed in future studies to further improve the quality of arthroscopic ACLR.",

            "title": "Adipositas und Fettstoffwechselstörungen im Kindes- und Jugendalter",
            "abstract": "Erkrankungen des kardiovaskulären Formenkreises treten typischerweise im Erwachsenenalter auf. Der Grundstein dieser häufig auftretenden Erkrankungen, die maßgeblich an der Morbidität und Mortalität unserer Gesellschaft beteiligt sind, wird schon in der frühen Kindheit gelegt. Beeinflusst von multiplen Faktoren sind es vorallem ein erhöhter Body-Mass-Index (BMI) und das Vorhandensein einer Dyslipidämie, sowie die mit Adipositas vergesellschafteten Risikofaktoren wie arterielle Hypertonie und Störungen den Glukosestoffwechsels im Kindesalter, welche die Erwachsenensterblichkeit stark beeinflussen. (1,2)\r\nÜbergewicht stellt als relevanter Risikofaktor mittlerweile eine der größten Herausforderung für den Public Health Bereich in Europa dar. Die Ergebnisse aktueller Studien bestätigen einen alarmierenden Anstieg der Prävalenz von Übergewicht und Adipositas, insbesondere bei Kindern und Jugendlichen. So stieg die Häufigkeit seit 1976 um mindestens die Hälfte an. Eine rezent publizierte Studie des Robert Koch-Instituts (RKI) an Kindern und Jugendlichen in Deutschland zeigte, dass bei rund 15% der Kinder und Jugendlichen der BMI über 25 kg/m2 liegt und sie damit definitionsgemäß als übergewichtig gelten. An Adipositas und den damit einhergehenden Auswirkungen auf die Gesundheit leiden rund 6%. Die Einflussfaktoren der Adipositas im Kindes und Jugendalter sind breit gefächert, hierzu zählen vorallem die prä- und postnatale Prägung, adipogene Umwelteinflüsse, Schlafverhalten und genetische Faktoren.\r\n\r\nFragestellung:\r\nDas Ziel dieser Diplomarbeit ist ein Überblick über die rezente wissenschaftliche Lage zum Thema Adipositas und Fettstoffwechselstörungen im Kindes- und Jugendalter, mit Augenmerk auf die Begleiterkrankungen und der Betrachtung als Risikofaktor für kardiovaskuläre Erkrankungen, sowie präventive Maßnahmen.\r\n\r\nMaterial und Methoden:\r\nDiese Diplomarbeit wurde als Literaturarbeit verfasst, die Literaturrecherche erfolgte mit Hilfe elektronischer Datenbanken wie PubMed®, Google Scholar®, UpToDate®, Cochrane Library® und Thieme-eRef®. Als relevante Suchbegriffe wurden verwendet: „Adipositas“, „juvenile Hyperlipidämie“, „Arteriosklerose“, „juvenile obesity“, „children“, „childhood“, „overweight“, „weight gain“, „hyperlipidemia“, „familial hypercholesterinemia“, “arteriosclerosis”, „comorbidity obesity“. Über die elektronische Bibliothek der\r\nMedizinischen Universität Graz standen Bücher und Journals in elektronischer Form zur Verfügung. Das Literaturverzeichnis wurde mit dem Literaturverwaltungsprogramm „Proquest Refworks®“ erstellt.\r\n\r\nErgebnisse:\r\nAdipositas im Kindes- und Jugendalter führt zu körperlichen Veränderungen und Erkrankungen, die in der Regel nur im Erwachsenenalter vorkommen. Mehrere Studien belegen die Tatsache, dass bei adipösen Kindern bereits erste Veränderungen der Gefäßwände aufzuweisen sind. Oft ist es nicht nur die Adipositas allein, sondern eine Kumulation mehrerer Risikofaktoren wie Dyslipidämie, Bluthochdruck und Diabetes Mellitus. Übergewichtige Kinder haben ein erhöhtes Risiko für Komorbiditäten und Folgeerkrankungen.\r\nHäufig entwickeln sich aus übergewichtigen und adipösen Kindern übergewichtige Erwachsene mit hohem Risiko für akute und chronische Erkrankungen. Durch den frühen Beginn der Schädigungen werden Folgeerkrankungen immer früher tragend und Krankheiten der Erwachsenendomäne werden zur Problematik junger Erwachsener.\r\n\r\nSchlussfolgerung:\r\nZwischen kindlichen Fettstoffwechselstörungen, Adipositas und Arteriosklerose besteht ein klarer Zusammenhang, der schwerwiegende Folgen für die Volksgesundheit nach sich zieht. Die Beeinflussung kardiovaskulärer Risikofaktoren ist daher eine Möglichkeit zur erfolgversprechenden Prävention von kardiovaskulären Folgeerkrankungen im späteren Erwachsenenalter.",
