Despite its high prevalence, research has not been focused on the topic. Simple, non-invasive detection methods must be evaluated and optimized in order to detect alcohol-related fibrosis in potentially reversible stages. The aim of this study was to determine the presence of fibrosis by non-invasive means in mostly asymptomatic alcoholics undergoing detoxification. Additionally, investigation of alterations in liver stiffness due to abstinence in a follow-up visit 3 months after the respective baseline visit was conducted. Finally, the role of PNPLA3 and SERPINA1 genotypes in fibrosis development was investigated.\r\nMethods\r\n31 consecutive patients admitted for alcohol detoxification were enrolled. FIB-4 index was calculated using laboratory and clinical routine parameters. ELF-test was carried out on Advia Centaur XP (Siemens Healthcare Diagnostics, Vienna, Austria). SERPINA1 and PNPLA3 genotypes were analyzed in a routine laboratory test by 5'-exonuclease assays (TaqMan). VCTE was performed in patients after a fasting period of at least two hours with FibroScan® 502 Touch (Echosens, Paris, France). Statistical analyses were performed using IBM SPSS Statistics version 25 and 26.\r\nResults\r\nAbout 16% of the initial study population showed advanced fibrosis according to VCTE using a threshold of 15 kPa. Using a LS of 15 kPa as reference, for the detection of advanced fibrosis (F3, F4), ELF worked best using 10.5 as a cut-off (TPR = 80%, TNR = 100%, PPV = 100%, NPV = 96,2%). A FIB-4 cut-off value of 1.30 was found to be adequate for ruling out advanced fibrosis (TPR = 60%, TNR = 80,8%, PPV = 37,5%, NPV = 91,3%). FIB-4 increased, whilst ELF score and LS dropped after a period of 3 months. ALT (p = 0,020) and MCV (p = 0,016) significantly decreased following abstinence.\r\nConclusion\r\nIntroducing asymptomatic alcoholics to a liver clinic could be beneficial for their treatment. ELF and FIB-4 are adequate non-invasive tools for the detection of advanced fibrosis as defined by LS ≥ 15 kPa. The role of PNPLA3 and SERPINA1 genotypes could not adequately be determined in this study. Dieser Wissensmangel beruht am ehesten auf der zu geringen Aufklärung durch behandelnde Ärztinnen und Ärzte sowie anderem medizinischem Fachpersonal. \r\nDie physischen Beschwerden, welche durch die Beckenbodeninsuffizienz hervorgerufen werden, sind sehr vielseitig und können sich in Form von Harn- oder Stuhlinkontinenz, Senkungsbeschwerden und Dyspareunie äußern und führen des Weiteren zusätzlich zu psychischen Begleitsymptomen. \r\nHäufig werden diese aber aus Schamgefühl der Betroffenen gegenüber medizinischem Personal nicht angesprochen. \r\nZiel dieser Arbeit ist es, Grundlagenwissen zum Thema Beckenboden, Beckenbodeninsuffizienz und ihrer Ursachen zusammenzufassen und allem voran aufzuzeigen, wo das Wissensniveau Schwangerer in Bezug auf diese Themen liegt.\r\n\r\nMaterial/Methoden\r\nZu Beginn wurde eine Literaturrecherche zu den Punkten Anatomie des Beckenbodens, Beckenbodeninsuffizienz sowie Harn- und Stuhlinkontinenz, Deszensus und Prolaps und auch zu den verschiedenen Therapieoptionen durchgeführt. Verwendet wurden hierfür vor allem medizinische Fachliteratur und wissenschaftliche Publikationen.\r\nZur Analyse des Wissensstandes der Patientinnen wurde ein neuer Fragebogen erstellt, welcher persönliche Fragen zu Alter, Größe, Gewicht, Nikotinabusus, Sportverhalten sowie Fragen zur aktuellen Schwangerschaft und vorangegangenen Geburten beinhaltete. Der zweite Abschnitt des Fragebogens ging auf das allgemeine Wissen der werdenden Mütter zu Veränderungen des Beckenbodens ein.\r\n \r\nErgebnis\r\nDie verwendeten Studien zeigen, dass die meisten Patientinnen nur wenig Wissen zum Thema Beckenboden und Beckenbodeninsuffizienz besitzen und die meisten ihrer Informationen über Schwangerschaft und Beckenboden von ihren Hebammen beziehen.\r\nDie Ergebnisse unserer Befragung sind ähnlich und zeigen, dass die Frauen kaum Kenntnisse über Gravidität und Beckenbodeninsuffizienz besitzen und diesbezüglich auch wenig Informationen erhalten haben. In der von uns durchgeführten Studie beantworteten zwischen 5 – 28% die allgemeinen Wissensfragen mit „ich weiß es nicht“, und nur eine Teilnehmerin gab an, dass sie ihr Wissen als „sehr gut“ einschätzte, wohingegen 8% sich schlecht informiert fühlten. Immerhin 46% der Befragten empfanden ihren Informationsstand als ausreichend, wobei diese Angabe in Relation zu den von denselben Frauen abgegebenen Antworten als fraglich zu betrachten ist.\r\nDer Informationsmangel in Bezug auf die physischen Folgen von Schwangerschaft und Geburt ist mit großer Wahrscheinlichkeit darauf zurückzuführen, dass 49% der Teilnehmerinnen angaben, keine Informationen zum Thema Beckenboden, -veränderungen / -insuffizienz und -training erhalten zu haben.\r\n\r\nFazit\r\nUnsere Fragebogenstudie verdeutlicht, dass fast die Hälfte der Teilnehmerinnen nicht oder nicht ausreichend über den Zusammenhang zwischen Schwangerschaft, (vaginalen) Entbindungen und Beckenbodeninsuffizienz sowie Präventivmaßnahmen aufgeklärt worden sind. The primary hypothesis was that sponsored or dependent studies performed better than independent clinical studies. Secondly, the thesis aimed to detect a possible difference in revision rates between different types of short-stems and surgical approaches. The third goal was to retrospectively analyse the first series of short-stem prostheses implanted at the Medical University of Graz in terms of migration rates. \r\nMethods: Three databases were comprehensively searched and studies were included if the study provided data on revision rate and was published between 2006 and 2020. The formula “revision per 100 component years” was used in order to compare different studies irrespective of number of hips implanted and different follow-up periods. For group analysis, studies were divided into independent and dependent clinical trials and the Australian National Arthroplasty Register could be used for comparison regarding revision rates. Additionally, short-stems were subdivided, depending on their femoral neck resection, into neck sparing (NS), neck retaining (NR) and neck harming (NH) stems. Einzel-Bild-Röntgen-Analyse (EBRA) was used to analyse the migration rate of the Optimys® short-stem.\r\nResults: Data were analysed from sixty-six studies with seventy cohorts. After one decade, the calculated median revision rate from all clinical trials was 4.2%, which is slightly less compared to 5.0% in the Australian register. Dependent data showed lower revisions rates in comparison to independent data, without reaching the criteria of significance. NR femoral stems revealed the worst outcome, while NH and NS prostheses performed comparably well. The posterior approach in combination with short-stem hip replacement is associated with longer implant survival. At 3 years postoperatively, the mean axial subsidence of the first 49 Optimys® short-stems was 3.0 mm. Up to now, no stem had to be revised. \r\nConclusion: Calculated long-term NS and NH revision rates are acceptable and appear to be superior to NR rates. No publication bias could be detected regarding implant survival. There is a need for more clinical trials and joint registries providing sufficient data on short-stem hip prostheses. The migration analysis observed a migration pattern with initially pronounced subsidence followed by a subsequent settling. Ziel dieser Arbeit ist es zu überprüfen, ob die festgelegten Maßnahmen für Leistenbruchoperationen ausreichend sind, um postoperative eine angemessene Schmerzreduktion zu erreichen.\r\nMethoden\r\nEingeschlossen in die Studie wurden 951 PatientInnen, bei denen im Zeitraum vom 04.01.2013 bis zum 29.12.2017 ein operativer Verschluss des Leistenbruches durchgeführt wurde. Für die Studie relevanten Patientenparameter wurden aus verschiedenen Dokumentationseinheiten der KAGes-Applikation „Medocs“ entnommen, zusammengeführt und statistisch ausgewertet. Die Prävalenz von klinisch relevanten Schmerzwerten ( ≥ 4 / 10), der Effekt einer intraoperativen Regionalanästhesie auf den postoperativen Schmerz und die Prävalenz von postoperativer Übelkeit und Erbrechen (PONV) wurden mit verschiedenen statistischen Methoden analysiert.\r\nErgebnisse\r\nInsgesamt litten 15,8% der PatientInnen an Schmerzen ≥ 4 / 10 innerhalb des Beobachtungszeitraumes (Operationstag bis 3. postoperativer Tag). Die Anzahl an PatientInnen mit klinisch relevanten Schmerzen war am Tag der Operation am höchsten (Operationstag: 15,8%, 1. postoperative Tag: 6,6%, 2. postoperative Tag 3,9%, 3. postoperative Tag: 4,9%). Durch den Einsatz einer intraoperativen Regionalanästhesie konnten postoperativ Schmerzen reduziert werden. (Effektstärke r der Tage im zeitlichen Verlauf: Operationstag: r=0,07, 1. postoperative Tag: r=0,31, 2. postoperative Tag: r=0,30, 3. postoperative Tag: r=0,40) Es konnten keine signifikanten Unterschiede bei der Häufigkeit von behandlungsbedürftigen Schmerzen in Relation zu einer erhaltenen / nicht erhaltenen intraoperativen Regionalanästhesie gefunden werden. (Signifikanzwert p der Tage im zeitlichen Verlauf: Operationstag: p=0,571, 1. postoperative Tag: p=0,583, 2. postoperative Tag: p=0,602, 3. postoperative Tag: p=0,443) Die PONV-Prävalenz mit und ohne intraoperativ erhaltener Regionalanästhesie lag bei 11,5%.\r\nSchlussfolgerung:\r\nDas festgelegte standardisierte schmerztherapeutische Vorgehen bei Leistenbruchoperationen an der Universitätsklinik für Kinder- und Jugendheilkunde Graz ist gut und der zu erwartenden Schmerzen angemessen. Recent theories reveal that hypoglycemic events in diabetes is involved in the development and occurrence of cardiovascular disease. For instance, in the ACCORD study, an intensive glucose lowering therapy regimen in type 2 diabetes was associated with a higher risk of cardiovascular disease and mortality. In this thesis a review of existing literature regarding the influence of hypoglycemia on the cardiovascular metabolism and data of an experimental trial, DIAPLATE, will be presented.\r\nMethods: In the depicted study, 14 subjects with T2DM (10 men and 4 women, age 55 ± 7 years, HbA1c 51 ± 7 mmol/mol) without established cardiovascular comorbidity and treated with metformin monotherapy were included to a monocentric, open, single arm and mechanistic study. The acute and prolonged (up to one week) effects of a stepwise hyperinsulinemic-hypoglycemic clamp experiment (63 mg/dL and 45 mg/dL, for 30 minutes respectively) on markers of platelet aggregation, coagulation, endothelial function as well as inflammation were investigated.\r\nResults: A significant increase in PAC1posCD62Ppos, PAC1posCD63pos and PAC1posCD62PposCD63pos positive platelets, PAI-1, fibrinogen, factor VIII, von Willebrand factor and D-Dimer as well as in VCAM and ICAM was observed. These effects were observed during the experiment but especially one week after the investigation. Ein Problem dieser in Fachkreisen als sinnvoll eingestuften Maßnahmen ist, dass sich viele Angehörige zu wenig auf ihre Rolle vorbereitet fühlen und zu wenig Informationen bezüglich Pflege erhalten. Schriftliches Informationsmaterial ist eine gute Möglichkeit, um die Angehörigen in dieser Hinsicht zu unterstützen. \r\nZiel: Ziel dieser Arbeit ist es, publizierte Informationen und Handlungsempfehlungen zu pflegerischen Maßnahmen bei beatmeten Patientinnen und Patienten zu bewerten und daraus Informationsmaterial für Angehörige zu erstellen. \r\nMethoden: Für diese Übersichtsarbeit wurde eine systematische Literaturrecherche nach Leitlinien und Reviews in elektronischen Datenbanken durchgeführt. Zusätzlich wurde eine Handsuche über eine Metasuchmaschine und über Referenzlisten durchgeführt. Die Limitationen bei der Suche beschränkten sich auf die Sprachen Englisch und Deutsch und auf Artikel der letzten zehn Jahre. Die eingeschlossenen Artikel wurden von zwei Forscherinnen unabhängig voneinander bewertet. \r\nErgebnisse: Insgesamt konnten drei Leitlinien und drei Reviews eingeschlossen werden. Hinsichtlich ihrer Qualität wurden die Leitlinien durchgehend als moderat und die Reviews sehr positiv bewertet. Die Artikel lieferten Empfehlungen zu allgemeinen Anforderungen bei Heimbeatmung, zur benötigten Ausstattung bei invasiver und bei nichtinvasiver Beatmung, zum Sekretmanagement, zum Tracheostomamanagement und Kanülenwechsel, zu möglichen Komplikationen sowie zum Notfallmanagement. \r\nSchlussfolgerung: Es konnten einige „starke Empfehlungen“ und sehr wichtige Empfehlungen mit niedrigerem Evidenzgrad sowie Expertinnen- und Expertenmeinungen identifiziert werden. The main causes for a compression are accidents which lead to skull fractures and expansive procedures such as tumors. \r\nBesides conservative therapy such as steroids and intracranial approaches, endoscopic decompression is a well-established method. In the international literature opinions on which parameters influence the visual outcome of the patients differ a lot. The objective of this study was to analyze several parameters and draw conclusions which may help to improve and adapt treatment options for future cases.\r\n\r\nMaterial and methods: In this study, data from patients at the Department of Otorhinolaryngology at the Medical University of Graz were collected who underwent an optic nerve decompression between 1 June 2001 and 18 November 2018. In total, 62 patients fulfilled the inclusion criteria and 32 underwent an endoscopic decompression. They were considered for the main analysis. Retrospectively collected data were statistically analyzed to compare parameters that favor visual improvement after surgery. \r\n\r\nResults: The use of steroids did not show a benefit in visual outcome. On the contrary, in the group of patients with no steroid treatment more patients (66.67%) improved in vision as in the group with steroid treatment (31.25%) (p=0.049). \r\nPatients with an injury of the nerve proven in radiological imaging before surgery had a higher recovery rate (66.67%) in comparison to the group with a normal optic nerve (28.57% improved) in their scans (p=0.033). \r\n\r\nConclusions: Considering the success rate of vision recovery (50%) after endoscopic decompression surgery, the establishment of a standard guideline for optic nerve compressions would be an important future target to improve the success rate. As a conclusion, the radiological findings as indicator for surgery might help to avoid non-beneficial surgeries. Moreover, the use of steroids should be considered more carefully and investigated with further studies analyzing their feasibility.\r\nSurprisingly, no strong correlation was observed regarding the timing of surgery. Therefore, basics of cancer development, the immune system and the possibilities in cancer therapy were investigated. The focus of this thesis is a systematic review of T-cell therapy, including latest developments and current studies.\r\n\r\nThe method used for this work is a systematic literature review. The literature research includes books, various publications from journals and current treatment guidelines. Therefore, PubMed and ClinicalTrials.gov were selected as databases. Wherever possible, current literature was used, in particular in the chapters of the different forms of T-cell therapies.\r\n\r\nThe results include a systemic analysis of the different forms of T-cell therapy in various tumor entities. The possibilities of tumor therapies are discussed and a short excursion into the history of T-cell therapy is given. Results from pre-clinical and clinical studies using T-cell therapies in treatment of different forms of cancer are discussed. In addition, side effects and limitations for each form of T-cell therapy are shown. The manufacturing process of T-cell therapy products is described as well. The influence of various factors, such as the tumor microenvironment or the properties of the used T-cells, are explored. Future perspectives of T-cell therapy in form of new approaches, which might enable a widespread use, are presented.\r\nResults of T-cell therapy are compared to results of standard cancer therapies, such as chemotherapy or radiation therapy. Possible combinations of T-cell therapies with other forms of cancer treatments and their results in clinical studies are shown. Already approved T-cell therapies are presented and current ongoing clinical trials are described.\r\n\r\nDue to advances in immunotherapy research, T-cell therapies have already successfully induced tumor regression or even cure in patients in clinical studies. Ziel dieser Studie war es, PatientInnen-, Behandlungs- und Komplikations-spezifische Daten retrospektiv zu analysieren und Risikofaktoren zu ermitteln, welche die Entwicklung postoperativer Komplikationen begünstigen. Des Weiteren sollte veranschaulicht werden, dass die objektive und systematische Erfassung postoperativer Komplikationen durch die Clavien-Dindo-Klassifikation einen Vergleich unterschiedlicher Studienergebnisse ermöglicht.\r\n\r\nMethodik\r\nIm Rahmen dieser retrospektiven Studie wurden 543 PatientInnen untersucht, die sich zwischen Juli 2005 und Dezember 2017 einer freien Lappenplastik an der klinischen Abteilung für Plastische, Ästhetische und Rekonstruktive Chirurgie der Medizinischen Universität Graz unterzogen haben. Ausgeschlossen wurden freie Lappenplastiken die eine Fallzahl <15 aufwiesen. Sämtliche Komplikationen und potentielle Risikofaktoren wurden aus dem Datenverwaltungsprogramm openMEDOCS von einem Observer erhoben. Die erhobenen Komplikationen wurden mit Hilfe der Clavien-Dindo-Klassifikation klassifiziert.\r\n\r\nErgebnisse\r\nInsgesamt entwickelten 357 PatientInnen (65,75%) postoperative Komplikationen. 30,53% der erfassten Komplikationen wurden als Abweichung vom normalen postoperativen Verlauf ohne Notwendigkeit einer zusätzlichen pharmakologischen oder chirurgischen Intervention (Grade I), 1,68% als zusätzliche antibiotische Therapie (Grade II), 7,56% als chirurgische Intervention in Lokalanästhesie (Grade IIIa) und 57,98% in Allgemeinanästhesie (Grade IIIb) klassifiziert. 2,24% der PatientInnen mit postoperativen Komplikationen entwickelten eine Lungenarterienembolie und mussten intensivmedizinisch behandelt werden (Grade IVa). Es konnten keine postoperativen Komplikationen der Grade IVb und V erfasst werden.\r\nEs konnten 13 potentielle Risikofaktoren nachgewiesen werden: Männliches Geschlecht, arterielle Hypertonie, Nikotinabusus, Alkoholabusus, verlängerte Operationsdauer, erhöhte intraoperative Flüssigkeitsbilanz, erniedrigtes präoperatives Albumin, erniedrigtes präoperatives Kreatinin, erniedrigtes präoperatives Hämoglobin, erhöhtes präoperatives Hämatokrit, erhöhte präoperative APTT, erhöhte präoperative Thrombozyten und erhöhtes intraoperatives Laktat.\r\n\r\nSchlussfolgerung\r\nTrotz der Gesamtkomplikationsrate von 65,75% handelt es sich bei freien Lappenplastiken um eine sichere Behandlungsmethode. Im Vorfeld muss den PatientInnen vermittelt werden, dass nicht interventionspflichtige postoperativen Komplikationen häufig auftreten und das Revisionen in Lokal- oder Allgemeinanästhesie notwendig sein könnten. Jedoch ist die Wahrscheinlichkeit lebensbedrohlicher Komplikationen, die eine intensivmedizinische Behandlung erfordern oder einen tödlichen Verlauf aufweisen äußerst gering. Der stetige Altersanstieg der Bevölkerung bedingt zusätzlich, dass derartige Operationen vermehrt notwendig werden. In dieser Arbeit haben wir uns mit dem radiologischen und klinischen Outcome anhand dreier Totalendoprothesendesigns (DePuy Attune, DePuy Sigma, Zimmer NexGen) innerhalb eines Zentrums beschäftigt und deren Ergebnisse miteinander verglichen. Es galt die Hypothese zu bestätigen, wonach das entwicklungstechnisch jüngste System, die DePuy Attune-TEP, signifikant bessere Ergebnisse - sowohl klinisch als auch radiologisch - aufzeigt. \r\n\r\nZielsetzung \r\nZiel dieser Studie war es, anhand des Vergleichs verschiedener Designs monozentrisch zu ermitteln, inwieweit das exakte radiologische Alignement das klinische Outcome für Patientinnen und Patienten beeinflusst. Des Weiteren lag unser Augenmerk auf der Langzeitbeobachtung der Ganzbeinachse (GBA), um etwaige Veränderung oder statistische Korrelationen feststellen zu können. \r\n\r\nMethoden\r\nEs handelt sich hierbei um eine retrospektive, klinisch-radiologische Nachuntersuchung von 223 Knietotalendoprothesen im AUVA - UKH Steiermark | Standort Graz, welche zwischen 2008 und 2018 implantiert wurden.\r\nNeben der Vermessung der prä- und postoperativen Ganzbeinachse aller Patientinnen und Patienten wurden 45 im Single-Surgeon Prinzip und Implantat gematcht und klinisch evaluiert. Hierbei wurde der WOMAC Index, der Knie Society Score (KSS) sowie ein Ganzbeinröntgen angefertigt, eine ideale radiologische Ausrichtung von 0° Achsabweichung wurde definiert. Eine statistische Interpretation der Ganzbeinachse im Langzeit follow-up wurde ermittelt.\r\n\r\n\r\nErgebnisse \r\nIn der radiologischen Untersuchung konnte keine Signifikanz zugunsten eines TEP-Modells festgestellt werden. Während hierbei die DePuy Sigma-TEP die besten Werte aufzeigte (0,12° Valgus), zeigten sich alle drei Totalendoprothesendesigns im Langzeit – follow up der GBA annähernd gleichwertig, wenngleich das Zimmer NexGen Modell im Schnitt 2,1° Achsenabweichung vom postoperativen Ausgangszustand aufwies.\r\nIn der klinischen Nachkontrolle wies die Zimmer NexGen-TEP sowohl im Zuge des KSS (86,14 Prozentpunkte) als auch beim WOMAC Index (3,07) vergleichend mit den beiden anderen Totalendoprothesentypen signifikant überlegene Resultate auf.\r\n\r\nDiskussion\r\nRein radiologisch zeigten sich keine signifikanten Unterschiede zwischen den einzelnen Designs. Im Langzeit – follow up der GBA lieferte die Zimmer NexGen-TEP die größte Abweichung vom direkt postoperativ gemessenen Wert. Auch dies erscheint aufgrund diverser Prozesse im Langzeitverlauf nachvollziehbar.\r\nDie Philosophie, Totalendoprothesen strikt achsengerecht und entgegen eines vermehrten Bewegungsumfanges einzubauen, konnte im Rahmen dieser Studienuntersuchung in Frage gestellt werden und könnte für das Konzept der kinematischen Ausrichtung sprechen. They harbor genetic aberrations, immune and stromal cells as well as microbes. During tumor development, the host-microbiota equilibrium is altered leading to a pro-inflammatory resident microbiota which could drive cancer progression. Given the central importance of microbiome in health and disease, elucidating mechanisms that maintain the microbiome homeostasis at cutaneous (skin) and mucosal (GI) surfaces as well as factors leading to microbiome disturbance (dysbiosis) and ultimately to chronic inflammation will improve our understanding of cancer initiation and progression. \r\nSkin harbors a considerable number of microbes that play an important role in health and disease. Nevertheless, their contribution to skin tumorigenesis has not been well studied. Therefore, we comparatively assessed the microbial community compositions in human keratinocyte skin tumors, AK, SCC and basal cell carcinoma (BCC). Our study shows that skin tumors are characterized by markedly different microbial community structures. AK and SCC, unlike BCC exhibit a significantly increased bacterial biomass. Markedly, Staphylococcus aureus was overabundant in the hyperkeratotic regions of SCC. Further, skin tumors showed an altered expression of antimicrobial peptides (AMPs), wherein human ß-defensin-2 (hBD-2) was elevated in SCC. Interestingly, this AMP is known to induce proliferation of squamous epithelia. To that end, we challenged SCC cells with S. aureus in vitro and observed specific induction of hBD-2 expression, which also promoted the growth of SCC cells. Thus, our studies demonstrate that a microbial community composition possibly favored due to the alteration of the microbial habitat (niche) perpetuates cancer development. In the case of SCC, increased S. aureus likely promotes tumor cell growth via specific induction of hBD-2, thus perpetuating the neoplastic process.\r\nAnother organ system wherein the microbiota contributes to cancer development is the GI tract. Fusobacterium species typically colonizing the oral cavity but also the GI tract, have shown to be increased in GI tumors and can promote their growth. Part of this thesis validated an increased abundance of tumor-promoting Fusobacterium and F. nucleatum in adenomatous polyps and CRC compared to normal tissue. Interestingly we also observed a decreased expression of FadA from adenomas to carcinoma suggesting a possible down-regulation of this microbial colonization factor. \r\nAlso, another aspect of this thesis aimed at addressing critical issues in fungal compartment of the microbiota which includes fungal DNA extraction and PCR amplification using the internal transcribed spacer (ITS) regions from human skin FFPE specimens. The tools used for microbial community analysis are typically developed for bacterial components and often fall short if applied to fungal data. Hence, we first aimed at attaining an ideal method to isolate fungal DNA from human skin FFPE, which represents the typical specimens for histopathology. We further sought to optimize PCR amplification of fungal ribosomal DNA using the ITS1 and -2 regions which are highly variable sequences used to distinguish fungal species. We showed that mechanical cell disruption during DNA isolation significantly lowered the DNA yield leading to a significant decrease in signal-to-noise ratio in ITS PCR. Possible underlying reasons may be alterations of the airway microbiome. The aim of this prospective cohort study was to investigate the cardiopulmonary performance capacity and the airway microbiome of adolescent and adult patients after corrected EA and to compare the findings to healthy age- and sex-matched controls.\r\n\r\nMethods: All EA patients treated between 1980 and 2010 were invited to a prospective study consisting of clinical examination, conventional spirometry and exhausting bicycle spiroergometry. The airway microbiome was determined from deep induced sputum by 16S rRNA gene sequencing. The results were compared to a healthy age- and sex matched control group. \r\n\r\nResults: Nineteen EA patients with a mean age of 24.7 years and 19 age- and sex-matched controls were included. EA patients showed a significantly lower muscle mass compared to the control group. Spirometry revealed a significantly lower VCmax and higher rates of restrictive ventilation disorders of EA patients; significantly lower relative performance capacity and a significantly lower peak VO2 were found in spiroergometry. Alpha- and beta-diversity of the airway microbiome did not differ significantly between the two groups. LEfSe analysis revealed significantly enriched species of Prevotella uncultured, Streptococcus anginosus, Prevotella 7 Prevotella enoeca and Mogibacterium timidum in the EA patients.\r\n\r\nConclusion: In the long-term outcome, EA patients frequently suffer from restrictive ventilation disorders and impaired cardiopulmonary function associated with discreet alterations of the airway microbiome.
