Among these conditions, hepatocellular carcinoma (HCC) stands out as the most common form of liver cancer, accounting for approximately 70% of liver cancer cases. Reliable in vitro models needed to study liver biology and disease are currently limited. Primary human hepatocytes rapidly lose function and have limited proliferative capacity in vitro, which restricts their use for long-term metabolic, toxicological, and signaling studies. Liver organoids have emerged as a powerful alternative to primary cells and immortalized cell lines, as they provide a stable model that better preserves hepatic functionality and architecture, enabling disease modeling, drug testing, and gene editing. In this study, we aimed to isolate and characterize patient derived liver- and HCC organoid lines. After the organoid isolation, the organoids were then pursued to differentiate them to a more hepatic state using a special medium. We further characterized the organoid lines using different stainings and immunohistochemistry, using antibodies against cholangiocyte markers such as Ck7, CkAE1-3, and hepatocyte markers like HepPar1, HSP70, glypican 3, to establish cellular identities and differentiation status. Protein and gene expression analysis by western blot and qPCR were performed to confirm the expression of liver function-related and HCC-associated markers. Liver organoids showed increased expression of hepatic markers under differentiation conditions, including albumin, AFP, Ck19, and HNF4α, as well as metabolic genes such as cytochrome P450 isoforms. In contrast ductal marker Sox9 was downregulated, while EpCAM and Ck19 were unexpectedly upregulated in the differentiated state. HCC-derived organoids displayed similar morphological and molecular liver-like phenotype. Upon development, these liver and tumor organoids consistently showed a shift toward a more mature hepatic phenotype, demonstrating their suitability as physiologically relevant in vitro liver models.
