Das an Patient und Patientin orientierte Pflegesystem Primary Nursing, entwickelt von Marie Manthey, gleicht die Schwierigkeiten der genannten Pflegesysteme aus. Es umfasst die vier Kernelemente Verantwortungs- bzw. Entscheidungsübernahme, direkte Kommunikation und Fallmethode und führt zu einer Erhöhung der Berufszufriedenheit sowie Optimierung der Arbeitsbedingungen. Bisher wurde das Pflegesystem Primary Nursing in der österreichischen geriatrischen Pflege kaum eingesetzt.\r\nZiel: Das Ziel des Praxisprojektes ist die Evaluierung des Pflegesystems Primary Nursing aus der Perspektive der Mitarbeitenden in der geriatrischen stationären Langzeitpflege.\r\nMethode: Für das Praxisprojekt wird ein quasiexperimentelles Forschungsdesign, das Single-Group-Pretest-Posttest, gewählt. Um Veränderungen nach Implementierung des Pflegesystems Primary Nursing zu messen, werden Perspektiven der Angehörigen der Berufsgruppen der Gesundheits- und Krankenpflege, der Pflegehilfe, der Medizin und der Therapie auf einer geriatrischen Station einer österreichischen Krankenanstalt untersucht. Die Zeitspanne zwischen den zwei Erhebungszeitpunkten beträgt fünf Monate. Zur Erhebung der Daten werden folgende drei standardisierte Instrumente eingesetzt: Fragebogen Primary Nursing Pflegende, Fragebogen Primary Nursing MitarbeiterInnen und das Instrument zur Erfassung von Pflegesystemen (IzEP©).\r\nResultate: Für diplomierte Gesundheits- und Krankenpflegekräfte sowie die Pflegehelfer und -helferinnen werden positive Veränderungen im Bereich Arbeitsabwechslung und im individuellen Einsatz von Sachkenntnissen und Fähigkeiten festgestellt. Nach Implementierung des Pflegesystems Primary Nursing ist die Arbeitszufriedenheit der diplomierten Kräfte konstant geblieben. Daraus entwickeln sich die Kompetenzen. Besonders im Gesundheitsbereich ist es wichtig, Kompetenzen der verschiedenen Gesundheitsberufe klar abzugrenzen, um bei einer Kostenminimierung ein optimales Outcome zu erreichen. \r\nInternational Council of Nursing (ICN) hat dazu im Rahmen einer Reform, ein Kompetenzbild der Registered Nurse entwickelt. Die Nurse Practioners setzen diese, auf sie abgeänderten Kompetenzen in den Niederlanden, erfolgreich ein. In Österreich werden die Kompetenzen für die/den diplomierte/n (dipl.) Gesundheits- und Krankenschwester/-pfleger im Gesundheits- und Krankenpflegegesetz (GuKG) geregelt.\r\nWenn man diese drei Berufskompetenzen miteinander vergleicht, kann man feststellen, dass die Regelungen der Aufgabenbereiche in Österreich sehr offen gehalten werden. Es können dadurch Krankheiten verhindert und die Gesundheit gefördert werden. Es gibt verschiedene Definitionen von Gesundheitsberatung, denn der Begriff wird von den Organisationen unterschiedlich betrachtet. Wenn man verschiedene Definitionen miteinander vergleicht, kann man sagen:\r\nGesundheitsberatung bedeutet:\r\nDen Ratsuchenden zu unterstützen, einen physischen, psychischen und sozialen Gesundheitszustand zu erhalten bzw. wiederherzustellen.\r\nWeiters nimmt die Gesundheitsberatung die Funktion als Brücke zwischen Problemsystemen (zB Individuen) und Hilfssystemen (zB sozial: Pflege) ein. \r\nDadurch unterscheidet man verschiedene Beratungskonstellationen. Die Beratungsgespräche können unterschiedlich geführt werden.\r\nInzwischen haben sich fünf verschiedene Beratungsansätze herauskristallisiert:\r\n¿Psychoanalytischer Beratungsansatz\r\n¿Verhaltenstherapeutischer Beratungsansatz\r\n¿Humanistischer Beratungsansatz\r\n¿Familientherapeutischer/Systemischer Beratungsansatz\r\n¿Lösungsorientierter Beratungsansatz\r\nDer Beratungsprozess jedoch bleibt in jedem Beratungsansatz gleich. Um auch die Gesundheitsberatung in der Pflege zu vereinheitlichen, wurden verschiedene Checklisten entwickelt. Diese werden in Altersgruppen unterteilt, um eine altersspezifische Beratung zu ermöglichen.\r\nBesonders wichtig, um für die Praxis gut vorbereitet zu sein, ist die Ausbildung. Die Ausbildung im Unterrichtsgegenstand Gesundheitsberatung wurde von der World Health Organisation (WHO) festgelegt. Das Ausbildungscurriculum ist jedoch sehr offen gehalten und die Lehrenden sollen selbst die Wichtigkeit dieses Themas einschätzen. The Pinnacle¿ cup is a third-generation, porous-coated, modular acetabular component.\r\nThe clinical examination consisted of three standardized questionnaires: the UCLA activity score, the Harris Hip Score and the 36 item short- form. The clinical data of the latest follow up examination were encouraging, showing remarkable results in all three scores. Mean HHS was 95.6 points, mean UCLA was 6.9 points and the SF-36 scores in the subgroups bodily pain, mental health and physical functioning were excellent. Compared to the clinical scores of the second-generation Duraloc predecessor, the clinical results of the present group were superior. \r\nFor the radiological evaluation EBRA (Einzel-Bild-Röntgen-Analyse) digital software was applied. Mean total-migration was 0.74 mm after 3 months, 1.15 mm after 12 months and 1.59 mm after 36 months. The results demonstrate progressive migration of the Pinnacle¿ acetabular component in the first 3 months after surgery, but thereafter a rapid decrease in migration speed occurs indicating that secondary stabilisation has obviously occurred. \r\nNo revision surgery due to aseptic loosening had to be performed. Our findings add to the accumulating evidence that revision surgery due to aseptical loosening in hemispheric porous-coated cups is a rare complication. Bei der Meningokokkenerkrankung gibt es mehrere Serotypen, wobei der Serotyp B, gefolgt vom Serotyp C, in Österreich am häufigsten vorkommt. Die klinische Symptomatik ist weit gefächert und reicht von geringen Symptomen bis hin zu schwerer und oftmals noch immer letaler Sepsis. Im Rahmen meiner Diplomarbeit wurden die in den letzten Jahren an der Kinderabteilung des LKH Leoben beobachteten invasiven Meningokokken - Erkrankungen bzgl. Alter bei Erkrankung, Laborparameter, klinischer Symptomatik, Krankheitsverlauf und Outcome analysiert. Weiters erhoben wurde wie sich der Verlauf bei bestimmten Serotypen, in bestimmten Altersgruppen, und je nach Jahreszeit unterscheidet. Anschließend wurden die Resultate mit publizierten Ergebnissen verglichen.\r\nGeschlechtsspezifisch gibt es keinen Unterschied, jedoch ist die Erkrankung jahreszeitlich abhängig. So kommen die Infektionen vor allem in den Wintermonaten vor. Im jugendlichen Alter kam es zu einer Häufung von Erkrankungen, im Kleinkindesalter kam die Infektion nur vereinzelt vor. 80% der Patienten hatten präklinisch keine Antibiotikatherapie erhalten, obwohl bei 50% die richtige Verdachtsdiagnose gestellt worden war. Das Outcome war bei 90% sehr gut, während 10% an der Erkrankung verstarben. Dies entspricht dem internationalen Durchschnitt. Die Aufteilung der Serotypen war folgendermaßen: 50% erkrankten durch den Serotyp B, 35% durch den Serotyp C und bei 15% konnte kein Serotyp nachgewiesen werden.
