Da es weder eindeutige Laborwerte, noch klinische Zeichen gibt, welche auf das Bestehen einer nekrotisierenden Fasziitis hinweisen, ist es schwierig eine Frühdiagnose zu stellen, da die nekrotisierende Fasziitis nur schwer von anderen Weichteilinfektionen zu unterscheiden ist. Um jedoch ein Fortschreiten der Infektion zu verhindern ist es nötig frühzeitig therapeutische Maßnahmen zu setzen, wobei das radikale chirurgische Débridement eine zentrale Rolle spielt.\r\nMaterial und Methoden: Es handelt sich um eine retrospektive Studie, welche am Universitätsklinikum Graz durchgeführt wurde. Untersucht wurden die Daten von PatientInnen mit der Diagnose nekrotisierender Fasziitis, welche in den Jahren 1999-2009 am LKH behandelt wurden. Insgesamt konnten 60 PatientInnen in die Studie eingeschlossen werden. Anhand der erhobenen Daten wurden folgende Parameter evaluiert: Alter, Geschlecht, Komorbiditäten, Lokalisation, Mikrobiologie, Laborwerte, Dauer des stationären Aufenthaltes und Therapie.\r\nResultate: Unter den 60 PatientInnen waren 45 Männer und 15 Frauen, wobei das mittlere Alter aller PatientInnen bei 60 Jahren lag. Die Mortalität lag bei 22%, wobei Männer (n=11) häufiger verstarben als Frauen (n=2). Die Extremitäten waren in 28 Fällen betroffen. Bei den Komorbiditäten waren kardiale Erkrankungen (57%) und Diabetes mellitus (55%) am häufigsten anzutreffen. In der Mehrzahl der Fälle (n=58) war mehr als eine Komorbidität nachzuweisen. Bei 30 PatientInnen (50%) kam es zu einer polymikrobiellen Infektion, wobei aerobe gram-positive Kokken am häufigsten isoliert wurden. Behandelt wurden die PatientInnen mittels radikalem Débridement, Antibiotika und begleitender HBO-Therapie. Nach Beherrschung der Infektion kam die Unterdruck-Therapie mit nachfolgender Spalthauttransplantation zum Einsatz. Im Falle einer Progredienz der Infektion wurden Amputationen vorgenommen. Es konnte kein statistisch signifikanter Unterschied zwischen überlebenden und verstorbenen PatientInnen im Hinblick auf die therapeutischen Maßnahmen gefunden werden.\r\niv\r\nSchlussfolgerung: Essentiell für das Überleben der PatientInnen ist die frühest mögliche Durchführung eines radikalen chirurgischen Débridement und die Gabe von Breitbandantibiotika, da nur so das Fortschreiten des infektiöses Prozesses verhindert werden kann. Engmaschige chirurgische Folgeeingriffe sind in der Regel erforderlich, um eine Infektionsbeherrschung zu ermöglichen. Hüfttotalendoprothesen- Operation unterzogen, füllten sowohl präoperativ als auch 6 Wochen und 3 Monate postoperativ einen Fragebogen, welcher die Adult Attachment Scale (AAS), den Fragebogen zur sozialen Unterstützung (F- SOZU- K7), die Hospital Anxiety and Depression Scale (HADS) und den SF- 12 enthielt, aus. Weiters wurde zu denselben Zeitpunkten der Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) bei Knie- und der Harris Hip Score (HHS) bei Hüftpatienten erhoben.\r\nResultate:\r\nInsgesamt 46 Kniepatienten, davon 34 (74%) Frauen, mit einem Durchschnittsalter von 69,4 Jahren und 29 Hüftpatienten, davon 16 (55,2%) weiblichen Geschlechts, mit einem Durchschnittsalter von 66,7 Jahren schlossen die Studie ab.\r\nDie Korrelationsanalysen zwischen den präoperativ erhobenen psychosozialen Scores und den, zu den drei Zeitpunkten, gemessenen klinischen Scores ergaben bei den Kniepatienten signifikante Zusammenhänge zwischen AAS- Nähe und dem präoperativen WOMAC- Steifigkeitsscore (p= 0,034), HADS- Depression und präoperativem WOMAC- Steifigkeits- (p= 0,022) und Gesamtscore (p= 0,025), HADS- Angst und 3 Monate postoperativem WOMAC- Steifigkeitsscore (p= 0,027) und zwischen der psychischen Summenskala des SF- 12 und dem 3 Monate postoperativen Schmerz- (p= 0,043) und Funktionsscore (p= 0,041) des WOMAC.\r\nFür die Hüftpatienten konnten signifikante Zusammenhänge zwischen den präoperativen psychosozialen Scores AAS- Angst und dem 6 Wochen\r\n6\r\npostoperativen HHS- Funktionsscore (p= 0,046) und dem 3 Monate postoperativen HHS- Funktions- (p= 0,008) und Gesamtscore (p= 0,003), zwischen HADS- Depression und dem 6 Wochen postoperativen HHS- Schmerz- (p= 0,025) und Gesamtscore (p= 0,041), sowie dem 3 Monate postoperativen HHS- Funktionsscore (p= 0,031) gezeigt werden. Weitere Korrelationen zwischen HADS- Angst und dem 3 Monate postoperativen HHS- Schmerzscore (p= 0,020), F-SOZU-K 7 und dem 6 Wochen postoperativen ROM in Grad (p= 0,008) und zwischen der körperliche Summenskala des SF- 12 und dem 6 Wochen postoperativen HHS- Schmerzscore (p= 0,030) konnten nachgewiesen werden.\r\nDiskussion:\r\nDiese Studie zeigt, dass psychosoziale Faktoren, wie Angst und Depression, mit dem Outcome von Knie- und Hüfttotalendoprothetik in Zusammenhang stehen.\r\nSchlussfolgerung:\r\nUm subjektiv noch bessere Ergebnisse nach K- und H- TEP erzielen zu können, scheint es sinnvoll zu sein, jene Risikofaktoren im Vorfeld der Operation zu detektieren und wenn möglich zu therapieren.", "authors": [ "Schneider, N" ], "year": 2011, "source": "Humanmedizin; [ Diplomarbeit/Master Thesis ] Medical University of Graz; 2011. pp.71. Cell migration is dependent on the continuous organization of the actin cytoskeleton, which is regulated by members of the small Rho GTPase family (RhoA, Cdc42, Rac) that are also important for the regulation of signal transduction pathways. \r\nHere we provide evidence that defective lipolysis in macrophages lacking adipose triglyceride lipase (ATGL), the major enzyme responsible for triacylglycerol (TG) hydrolysis, favors an anti-inflammatory M2 macrophage phenotype. Our data implicate that insufficient lipolysis influences macrophage polarization and actin polymerization, resulting in impaired macrophage migration. Sustained phosphorylation of focal adhesion kinase (due to inactivation of its phosphatase by elevated levels of reactive oxygen species (ROS)) results in defective Cdc42, Rac1 and RhoA activation and increased and sustained activation of Rac2. Inactivation of Rac1 and Cdc42 lead to defective polarization and directional movement toward the chemoattractants. Furthermore, RhoA inactivation may cause impaired migration and actin polymerization in the presence of chemoattractant. Inhibition of ROS production leads to focal adhesion kinase (FAK) activation transiently and restores the migratory capacity of Atgl-/- macrophages. \r\nMacrophage apoptosis has an important implication in definition of early and late atherosclerosis. We found reduced plaque formation in model low density lipoprotein receptor (Ldlr-/-) transplanted with bone marrow from Atgl-/- mice. To elucidate the reason of less plaque formation, programmed cell death was investigated in primary peritoneal Atgl-/- macrophages. We observed elevated levels of cytosolic Ca2+ and ROS, stimulated cytochrome c release and nuclear localization of apoptosis inducing factor (AIF). Fragmented mitochondria prior to cell death were indicative of the mitochondrial apoptosis pathway being triggered as a consequence of defective lipolysis. Other typical markers of apoptosis, such as externalization of phosphatidylserine in the plasma membrane, caspase 3 and poly (ADP-ribose) polymerase cleavage, were increased in Atgl-/- macrophages. In addition, Ca2+ depletion from ER triggers activation of ATF4/CHOP pathway that defines ER stress induction in Atgl-/- macrophages. An artificial increase of cellular TG levels by incubating wild-type macrophages with very low-density lipoprotein almost completely mimicked the apoptotic phenotype observed in Atgl-/- macrophages. After this, there are the daily and weekly routines and the therapy plan, followed by the concept of therapy and its subdomains that are further subdivided into background and definition of each domain, its execution at the Weidenhof, the staff´s view on the domain and my personal observations. \r\nEnsuing, the intertherapeutic cooperation in the multidisciplinary setting is discussed after which the EDI ¿ 2 results, the results of the Eating Disorder Inventory, a psychological questionnaire, used to describe the psychopathology of anorexia nervosa and bulimia nervosa patients, are listed. In the same chapter there are the case reports of seven female patients treated at the Weidenhof. Sie können zum Beispiel zur vollkommenen Isolation der Betroffenen führen oder zu einer Veränderung des Tagesablaufs. Sie können das Arbeitsleben, das Privatleben aber auch das Familienleben beeinflussen. Die Einstellung von Jugendlichen über Themen wie Ernährung, Bewegung oder Freizeitgestaltung bereitet große Sorgen. Viele leiden bereits schon im Vorschulalter unter Stress und psychischen, psychosomatischen Beschwerden. Psychosoziale Auffälligkeiten und Störungen sind keine Seltenheit mehr. Doch fest steht, dass gesunde Kinder und Jugendliche Herausforderungen besser bewältigen können, als solche mit gesundheitlichen Defiziten. Schulische Gesundheitsförderung meint nicht nur Informationen über Gesundheitsthemen, sondern auch die Gestaltung des Settings Schule. Gesundheit sollte als bedeutende Komponente unseres Alltages gesehen werden. Gerade zum Alltag von Kindern und Jugendlichen gehört die Schule nun mal dazu. Unterschiedlichste Strategien werden eingesetzt um Gesundheitsfördernde Maßnahmen zu initiieren. Als bedeutender Faktor kann hierbei auch die Familie gesehen werden, da Interventionen deutlich effektvoller wirken, je mehr die Familie des jeweiligen Individuums inkludiert ist. Bispectral index (BIS) and state/response entropy \n(SE/RE) index have been widely used to estimate depth of anesthesia and sedation. In adults, independent of age, adequate and safe depth of anesthesia for surgery is usually assumed when these indices are between 40 and 60. Since its introduction in 1996, the Bispectral IndexTM (BISTM) has gained increasing popularity as a monitoring tool for the depth of anesthesia. Many reports on the value of BIS have been published in the literature paradoxically changing or inaccurately indicating the depth of anesthesia.\r\nMethods: Fifty American Society of Anesthesiologists (ASA) classification I-II patients, aged 18-59 years, undergoing general surgery on the lower limb were recruited in the study. We recruited two groups, one group was maintained at BIS around 25 and the other group of 25 patients was maintained at BIS around 50. Exclusion criteria was body mass index <18 or >26 kg m-2 and subjects with medical conditions that could affect the level of consciousness such as stroke, stupor or dementia, or patients on treatment with cardiovascular or sedative/hypnotic drugs that might affect BIS monitoring. BIS and Entropy electrodes were placed on patients forehead. For monitoring the level of analgesia we use BIS variability (BIS standard deviation) and Entropy bias (response entropy-state entropy).\r\nResults: There were no significant differences between the demographic data of the 2 groups. Our results indicate no significant differences with increasing degrees of noxious stimulation starting from 10 mA till 80 mA. The inducible cyclooxygenase-2 (Cox-2) is a candidate inflammatory marker in human osteosarcoma and a rate-limiting enzyme in PG biosynthesis. 15-deoxy-d12,14-PGJ2 (15d-PGJ2), a metabolite of PGD2 and PPAR¿ ligand, exerts a panel of different biological activities via receptor-dependent and receptor-independent mechanisms. The present study aimed at investigating the intracellular redox status and signalling cascades leading to Cox-2 induction in human MG-63 osteosarcoma cells. 15d-PGJ2 induced an accumulation of reactive oxygen species (ROS) that in turn led to upregulation of Cox-2 via different routes in a PPAR¿- and PGD2-receptor-independent manner. First, phosphorylation of p38 MAPK directly enhanced Cox-2 expression by promoting mRNA stability. Second, 15d-PGJ2 induced activation of EGF receptors and downstream activation of Cox-2 via phosphorylation of p42/44 MAPK. Functional activity of Cox-2 expression was tested by measurements of PGE2 and PGF2¿. Glutathione precursor molecules reversed enhanced ROS levels and Cox-2 expression. The synthetic compound 9,10-dihydro-15d-PGJ2 lacking the ¿¿ß-unsaturated carbonyl group in the cyclopentenone ring did not exhibit cellular responses observed with 15d-PGJ2. Thus, it is concluded that the electrophilic carbon atom of 15d-PGJ2 is responsible for alterations in the intracellular redox status and Cox-2 expression.\r\nThe electrophilic character of 15d-PGJ2 was also related to the induction of apoptotic markers such as caspase-3 activation and PARP cleavage. Apoptosis was regulated independently of MAPK activation, PPAR¿, and PGD2 receptors. Both, death receptor- and mitochondria-dependent pathways might be involved in 15d-PGJ2-induced programmed cell death, as procaspase-8 cleavage was upregulated and expression levels of the pro-apoptotic Bcl-2 family member Puma were increased. Furthermore, 15d-PGJ2 mediated the elevation of the intracellular GSH content and expression of heme oxygenase-1 (HO-1), both events that might act in a cytoprotective manner preventing cytotoxic effects of 15d-PGJ2.
