The gut microbiome is also \r\nsuspected to play a role in sleep regulation, and recent studies suggest that certain \r\nprobiotics, prebiotics, synbiotics, and fecal microbiome transplantation can improve sleep \r\nquality. \r\nWe aimed to assess the relationship between gut-microbiota composition, psychiatric \r\ndisorders, and sleep quality in this cross-sectional, cross-disorder study. We recruited 103 \r\nparticipants, 63 patients with psychiatric disorders (major depressive disorder [n = 31], \r\nbipolar disorder [n = 13], psychotic disorder [n = 19]) along with 40 healthy controls. \r\nSleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI). The fecal \r\nmicrobiome was analyzed using 16S rRNA sequencing, and groups were compared based \r\non alpha and beta diversity metrics, as well as differentially abundant species and genera. \r\nA transdiagnostic decrease in alpha diversity and differences in beta diversity indices were \r\nobserved in psychiatric patients, compared to controls. Correlation analysis of diversity \r\nmetrics and PSQI score showed no significance in the patient and control groups. \r\nHowever, three species, Ellagibacter isourolithinifaciens, Senegalimassilia faecalis, and \r\nuncultured Blautia sp., and two genera, Senegalimassilia and uncultured Muribaculaceae \r\ngenus, were differentially abundant in psychiatric patients with good sleep quality (PSQI \r\n>8), compared to poor-sleep quality patients (PSQI ≤8). Das Ziel dieser Arbeit besteht darin herauszufinden, ob es während der ersten Welle von COVID-19 Zusammenhänge zwischen den empfundenen Einschrän- kungen und sozioökonomischen Faktoren bei Personen in Österreich mit einem höheren Al- ter als 60 Jahre gab.\r\nMethodik\r\nIn einer vom Institut für Sozialmedizin, und Epidemiologie der Medizinischen Universität Graz in Auftrag gegebenen Querschnittsstudie wurden Personen online und am Telefon nach ihrem subjektiven Empfinden der Einschränkungen und nach diversen sozioökonomischen Faktoren befragt. Die Nettostichprobe umfasst 557 Personen. Anhand dieser Daten wird eine deskriptive Datenanalyse und daraufhin anschließend eine Poisson-Regression durchge- führt, bei dem ein Modell (A) ohne Kontrollvariablen mit einem Modell (B) unter Herein- nahme der Kontrollvariablen verglichen wird.\r\nErgebnis\r\nWährend Modell A einen Zusammenhang zwischen Einschränkungen und sozioökonomi- schen Prädiktoren nicht besser erklärt als ein Intercept-Only-Modell (p=0,421), ist das Mo- dell B dazu in der Lage (p=0,000). Die Kontrollvariablen „gesundheitliche Einschränkungen in den letzten sechs Monaten“ (p=0,000) und die „Häufigkeit des Sporttreibens“ (p=0,000) weisen jeweils einen signifikanten Zusammenhang mit den Einschränkungen auf. Alle an- deren Variablen sind in beiden Modellen nicht statistisch signifikant.\r\nSchlussfolgerungen\r\nWährend der ersten Welle von COVID-19 im Frühjahr 2020 gab es bei der Altersgruppe der über 60-Jährigen keinen signifikanten Zusammenhang zwischen Einschränkungen und so- zioökonomischen Faktoren. Gründe dafür können die im Vergleich mit späteren Lockdowns kurze Zeit drastischer Einschränkungen und die Qualität der gesamtgesellschaftlichen Her- ausforderung sein. Insbesondere am Beginn der Pandemie war dieses Gefühl der Herausfor- derung am stärksten ausgeprägt und im Gegensatz zu späteren Zeitpunkten resultierte das nicht in einer Pandemiemüdigkeit, sondern in einer Art „Pandemietrotz“. Häufig werden diese Tumore erst im fortgeschrittenen Stadium, wenn bereits Fernabsiedelungen vorliegen, diagnostiziert. „Oligometastasierung“ d.h. eine bis wenige Fernmetastasen stehen möglicherweise mit einer speziellen, weniger aggressiven Tumorbiologie in Verbindung. Moderne System- und Lokaltherapien gestatten in vielen derartigen Fällen längerfristige Tumorkontrolle. Die Literatur gibt erste Hinweise dafür, dass die Überlebensrate im oligometastatischen Stadium durch chirurgische Therapie des Primärtumors und der Fernmetastasen verbessert werden kann.\r\nMethoden: In dieser retrospektiven Analyse der Daten von erwachsenen Patient*innen mit nichtkleinzelligen Bronchuskarzinomen, die an der Klinischen Abteilung für Thorax- und hyperbare Chirurgie im Zeitraum zwischen 01.01.2003 und 01.01.2022 therapiert wurden, wurde evaluiert, wie sich die Resektion von Fernmetastasen bei Patient*innen im oligometastatischen Stadium im Vergleich zu Patient*innen ohne Fernmetastasen im gleichen T- und N-Stadium auf das Gesamtüberleben und das progressionsfreie Überleben auswirkt. Ausschlusskriterien waren fehlende Resektion des Primärtumors und ≥ 5 Fernmetastasen in ≥ 3 Organsystemen. Es konnte von 45 Patient*innen ein vollständiger Datensatz erhoben werden, wovon sich 19 bei Diagnosestellung im oligometastatischen Stadium befanden. Die Daten wurden durch Recherche im MEDOCS-System beziehungsweise per Abfrage aus dem zentralen Sterberegister erhoben. Die statistische Datenauswertung erfolgte mit „R“.\r\nErgebnisse: In den Analysen wiesen die Patient*innen ohne Fernmetastasen gematcht nach T- und N-Stadium über alle Subgruppen hinweg, außer jener mit fortgeschrittener Lymphknotenbeteiligung, ein besseres Gesamt- und progressionsfreies Überleben auf. Darüber hinaus konnte gezeigt werden, dass Frauen tendenziell eine höhere Überlebenswahrscheinlichkeit haben als Männer und dass Patient*innen mit fortgeschrittener Lymphknotenbeteiligung bzw. großen Primärtumoren bezüglich des OS von einer chirurgischen Therapie stärker profitieren. Die Resultate sind statistisch nicht signifikant.\r\nDiskussion: Die Ergebnisse der vorangegangenen Studien, die signifikante Unterschiede nachweisen konnten, wurden mit dieser Arbeit nicht bestätigt, obwohl in einigen Analysen entsprechende Trends sichtbar wurden. Die geringe Anzahl eingeschlossener Patient*innen und die kleinen Subgruppen schmälern allerdings die Aussagekraft dieser Analyse maßgeblich. Daher bedarf es weiterführender Studien prospektiver und multizentrischer Art, um die Ergebnisse zu untermauern. Unfortunately, the exponential growth of medical data forms an increasing problem, making it almost impossible to manually analyse and extract meaningful information. Deep learning (DL) is capable of detecting features and seeking recurring patterns in high-dimensional datasets. The field of oncology is, therefore, the perfect environment for the integration of DL to detect and characterize cancer cells and uncover molecular mechanisms.\r\n\r\nAim and objective: The aim of this thesis consists of comprehensive research and a summary of novel insights into DL as an augmenting tool in oncology. Hence, creating an overview of developments in the fields of radiomics, computational pathology and multi-omics.\r\n\r\nMethodology: A comprehensive literature search for all published articles regarding the objective of this thesis was performed using PubMed and Google Scholar as search engines. Complimentary information about DL and tumorigenesis was mostly obtained from textbooks and papers.\r\n\r\nResults and discussion: This novel technology marks suspicious lesions, predicts malignancies and monitors aberrant tissue. Furthermore, DL is outstanding at discovering driver oncogenic pathways and identifying cancer vulnerabilities, which could lead to the development of targeted therapy. The strength of this revolutionizing technology to quantify interconnected mechanisms of tumorigenesis and uncover patient-specific characteristics boosts the transition towards precision medicine. The patient-centric approach allows the tailoring of medical treatment to individual variabilities and enables the maximization of treatment response and clinical outcomes while minimizing side effects.\r\n\r\nConclusion: The data-driven characteristics of DL appear to be an excellent tool to augment physicians in different domains of their clinical routine.
