In 2019, 4.28 million osteoporotic-associated fragility fractures were reported. Due to increased life expectancy, the number of fragility fractures is estimated to reach 5.28 million by 2034. Osteosynthesis of osteoporotic bone presents a particular challenge due to alterations in the mechanical, immunological, and regenerative properties of the bone. Therefore, the requirements of implant materials changed to gain a better osseointegration, osteoconductivity, and osteoinductivity. Biodegradable magnesium (Mg)-based implants, such as ZX00, showed promising results in animal studies and in a first in-men study. Recently, results in osteoporotic rats revealed accelerated degradation of ZX00 compared to healthy rats. Hence, we aimed to investigate the systemic osteoimmunomodulatory changes during osteoporosis development and osteogenic properties of ZX00 in a rat model. Although PD-L1 expression levels correlate with response to ICI treatment, PD-L1 alone often fails to predict treatment response. \r\nIn our retrospective cohort of early-stage NSCLC patients, we compared PD-L1 expression in patients with available initial biopsy samples, samples from primary curative surgical and rebiopsy samples in case of subsequent recurrence. In all cases Ventana PD-L1 (SP263) assay was used. We scored PD-L1 expression into clinically relevant groups defined as 0%, 1-49% and ≥50%. Our main interest was the change of PD-L1 score-group between the different matched tumor samples in NSCLC patients.\r\nIn total 395 patients with stage I-III NSCLC were identified forming our main cohort. 136 (34%) out of 395 patients relapsed. For 87 patients of these patients at least two specimens for PD-L1 expression comparison, namely initial biopsy and rebiopsy were available. In 72 cases PD-L1 expression could be compared between all-time points. Between initial preoperative biopsy and matched surgical specimens, a clinically relevant conversion of PD-L1 expression group was found in 25 patients (34.7%). We could not identify a statistically significant influence of neoadjuvant treatment on PD-L1 alteration (p=0.39). When we compared PD-L1 expression from preoperative samples with rebiopsy samples a clinically relevant change in PD-L1 expression was found in 32 (36.8%) out of 87 patients. Similar to the neoadjuvant therapy, adjuvant treatment was also not associated with a change in the PD-L1 expression group (p=0.16). Overall, 39 patients (54.2%) showed at least one change into a different PD-L1 score group. In fourteen patients (19.4%) PD-L1 expression score group changed twice and in five patients (6.9%) PD-L1 score group was different in every analysed sample.\r\nWe conclude, that there is clinically relevant PD-L1 expression change in NSCLC patients during the course of disease. Xerophthalmie und Xerostomie stellen die Hauptsymptome dar, jedoch sind auch extraglanduläre Manifestationen wie Fatigue, Arthralgien oder neurologische Manifestationen nicht ungewöhnlich. Die Diagnose des Sjögren Syndroms erfordert gemäß den aktuellen ACR/EULAR Leitlinien von 2016 entweder den Nachweis von Ro-Antikörpern, oder eine Lippenspeicheldrüsenbiopsie mit einem Fokus-Score ≥ 1. Der Fokus-Score stellt dabei einen Surrogatorparameter für das Ausmaß der lymphozytären Entzündung in den Speicheldrüsen dar. Ein Fokus ist definiert als Ansammlung von ≥ 50 Lymphozyten, der Fokus-Score gibt die Anzahl der Foci pro 4 mm² Drüsengewebe an. Während Focus-Score Werte ≥ 1 die Diagnose des Sjögren Syndroms (bei vorliegender Klinik) bestätigen, ist weniger über die Bedeutung von Fokus-Score-Werten zwischen 0 und 1 bekannt, also Fälle, bei denen zwar eine für das Sjögren Syndrom typische Infiltration vorliegt, aber nicht in ausreichendem Ausmaß. Obwohl die Prävalenz von Eisenüberladung (IO) in dieser Patientengruppe in einigen Studien untersucht wurde, fehlen einheitliche Richtlinien für das Management. Aufgrund der Heterogenität der Patientengruppe ist es wichtig, potenzielle Risikofaktoren für Eisenüberladung zu identifizieren. Dazu wurde in dieser retrospektiven Studie die Transfusionsbelastung von 219 Patient*innen im Alter von 0 bis 29 Jahren, bei denen eine maligne Erkrankung diagnostiziert wurde und die ihre zytotoxische Therapie (Chemotherapie und/oder Bestrahlung) von Jänner 2016 bis Juni 2022 in der Abteilung für Pädiatrische Hämato-/Onkologie des Universitätsklinikums Graz abgeschlossen haben, analysiert. Demographische und krankheitsbezogene Variablen wie Alter, Geschlecht, Wachstum, Diagnose, Therapiedauer, Therapieintensität und Komplikationen wurden im Zusammenhang mit den Transfusionsparametern analysiert. Das durchschnittliche Alter bei der Diagnosestellung lag bei 103 Monaten (0-350) und die mediane Behandlungsdauer betrug 340 Tage (23-1502). Insgesamt erhielten 135 Patient*innen (87,7 %) Transfusionen von Erythrozytenkonzentraten. Der IO wurde anhand etablierter Grenzwerte aus der Literatur bestimmt (≥10 Erythrozytenkonzentrate, ≥100 ml/kg Körpergewicht, ≥1000 ml/m2 Körperoberfläche und geschätzte hepatische Eisenbelastung ≥1,6 mgFe/g Trockengewicht der Leber). Die Kohorte erhielt im Median 11 Erythrozytenkonzentrate (1–99). Das kumulative Transfusionsvolumen betrug im Median 2824 ml (133–24.122), das Volumen bezogen auf das Körpergewicht 110,9 ml/kg (4,4–493,3) und auf die Körperoberfläche 2993,5 ml/m2 (165,8–12.586,2). Die durchschnittliche hepatische Eisenbelastung betrug 2,0 mg Fe/g (0,1–17,1). Mehr als 10 Erythrozytenkonzentrate erhielten 62,2 % der Patient*innen, 55,6 % erhielten mehr als 100 ml/kg Körpergewicht, 83,0 % mehr als 1000 ml/m2 Körperoberfläche und 58,5 % wiesen eine geschätzte hepatische Eisenüberladung von ≥1,6 mg Fe/g auf. Als Risikopatient*innen für Eisenüberladung wurden Kinder unter 12 Jahren sowie Patient*innen, die an akuten Leukämien, Knochentumoren, embryonalen Tumoren und hochmalignen Gliomen erkrankt sind, identifiziert. Außerdem ging eine höhere Transfusionsbelastung mit längerer Therapiedauer, intensiveren Therapieintensitätsleveln sowie gehäuftem Auftreten von potenziell IO-bedingten Komplikationen einher. Diese Risikogruppen sollten in Zukunft auf Eisenüberladung gescreent und anschließend monitorisiert sowie bei Bedarf therapiert werden. The choice of therapy, the prognosis, and the outcome of plexus injuries – particularly regarding the functionality and usability of the affected extremity in everyday life – depend on several factors. These include the exact location of the nerve lesion in the brachial plexus (spinal root, trunci, divisions, cords or terminal branches), the affected arm function (shoulder function, elbow flexion, motor and sensory deficits in the forearm and hand), the form of injury (stretch, rupture, avulsion – neurapraxia, axonotmesis or neurotmesis), the mechanism of injury (compression, stretch or laceration of the nerve) and the mode of reconstruction (neurolysis, direct nerve repair or intra-/extraplexial nerve transfers). Brachial plexus reconstruction yields a good functional outcome in children and young adults, however, data and literature on the outcome after surgical reconstruction of brachial plexus injuries in patients over 40 years are scarce. This in turn has led to an increase in revision operations. As a result, revision arthroplasty of the knee joint is becoming increasingly important. This is reflected in the increasing number of total knee arthroplasty (TKA) revisions every year. It can be concluded from this that consideration must be given to the optimal components, both for the benefit of the patient and from an economic point of view (high costs per TKA). Today, there are various methods of implant fixation for knee prosthesis revisions. Initial short and medium-term results suggest that the use of metaphyseal components in revision arthroplasty can be a satisfactory method. Metaphyseal sleeves in addition to stem anchoring have proven particularly effective in the management of bone defects. The use of metaphyseal sleeves distributes the biomechanical forces in the proximal part of the tibia and in the distal femur.
