Die Berufsgruppe der Pflege spielt eine entscheidende Rolle in der Vorbeugung und Behandlung von IAD. Pflegekräfte sind für eine adäquate Hautpflege verantwortlich, um das Auftreten einer IAD zu verhindern. Besteht diese bereits, setzen sie geeignete Interventionen ein und tragen Verantwortung für die Verhinderung eines Wiederauftretens. In der Literatur wird ein Mangel an Wissen zum Thema IAD bei Pflegepersonen beschrieben, was sich auf die pflegerische Betreuung sowie auf die Gesundheit der Patient*innen negativ auswirken kann. Zur Erhebung des Wissens von Pflegepersonen zum Thema IAD steht derzeit das deutschsprachige Instrument „KNOW-IAD“ zur Verfügung (Durakovic et al., 2024). Es liegt jedoch noch keine Testung dieses Instrumentes auf dessen psychometrischen Eigenschaften im deutschsprachigen Raum (Österreich, Deutschland und Schweiz) vor. Eine psychometrische Testung ist von Bedeutung, um Aussagen über die Qualität des Instrumentes (Polit and Beck, 2021) und seine potenziellen Anwendungsgebiete in der pflegerischen Praxis treffen zu können.\r\nZiel: Ziel ist es, einige psychometrische Eigenschaften (Interne Konsistenz, Diskriminante Validität, Itemvalidität) des deutschsprachigen Fragebogens „KNOW-IAD“ im Setting Krankenhaus zu erheben. \r\nMethode: Das Design einer deskriptiven Querschnittsstudie wurde herangezogen und die Erhebung fand zwischen April und Juni 2024 statt. Der Fragebogen „KNOW-IAD“ beinhaltet 18 Items, welche in drei Domänen unterteilt sind (Ätiologie und Risiko, Klassifizierung und Diagnosestellung, Prävention und Management). Die Erhebung erfolgte mittels „Lime Survey“ und fand in Österreich, Deutschland und der Schweiz statt. Zur Berechnung der Internen Konsistenz wurde Cronbach’s alpha verwendet. Die Diskriminante Validität wurde mit einer Hypothesentestung ermittelt, und für die Itemvalidität wurden der „Difficulty Index“ und der „Discrimination Index“ berechnet.\r\nErgebnisse: An der Erhebung nahmen 521 Personen teil, davon waren 403 Personen aus Österreich, 77 aus Deutschland und 41 aus der Schweiz. Der Cronbach’s alpha für die erste Domäne (Ätiologie und Risiko) betrug 0,349. In der zweiten Domäne (Klassifizierung und Diagnosestellung) wurde ein Wert von 0,612 erreicht. Die letzte Domäne (Prävention und Management) wies einen Wert von 0,403 auf. Im Rahmen der Hypothesentestung zeigte sich, dass Pflegepersonen mit Fort- und Weiterbildungen (p<0,001), Zusatzqualifikationen (p<0,001) und Kenntnissen über das GLOBIAD Instrument (p<0,001) signifikant mehr Wissen aufwiesen als Pflegepersonen ohne diese Aspekte. Die Hypothesen zur Berufserfahrung (p=0,798) und zum Ausbildungsstand (p=0,750) konnten nicht bestätigt werden. Der „Difficulty Index“ ergab, dass vier der 18 Items des Fragebogens zu einfach (>0,80) und zwei Items zu schwer (<0,20) waren. Beim „Discrimination Index“ wiesen vier Items einen Wert unter 0,20 auf und konnten somit nicht adäquat zwischen eher „besseren“ und eher „schlechteren“ Teilnehmer*innen unterscheiden.\r\nSchlussfolgerung: Dies ist die erste Studie, die den „KNOW-IAD“ im deutschsprachigen Raum untersucht. Die große Stichprobengröße verdeutlicht das Interesse an diesem Themengebiet. Der Fragebogen erreichte in der psychometrischen Testung jedoch nur mäßige Ergebnisse. Ziel ist es, das körperliche, emotionale und soziale Wohlbefinden der Patient*innen zu stärken. Die Bandbreite der Angebote umfasst Gemeinschaftsaktivitäten wie Sportgruppen, Kunstprojekte bis hin zu ehrenamtlichen Tätigkeiten wie beispielsweise in Vereinen. Besonders bei chronischen Erkrankungen, Einsamkeit und psychischen Belastungen wird ein ganzheitlicher Ansatz gefördert, der das Gesundheitssystem gleichzeitig entlastet. Aus immunologischer Sicht ist eine besondere Modulation des Immunsystems notwendig, um die Entwicklung des semiallogenen Fetus zu ermöglichen. Der Aufbau einer adäquaten Immuntoleranz ist essenziell für den physiologischen Verlauf der Schwangerschaft. Ist diese notwendige immunologische Adaptation gestört, so können Schwangerschaftspathologien wie Präeklampsie, intrauterine Wachstumsrestriktion oder auch Abort die Folge sein. Frauen mit spontanen und rezidivierenden Aborten verspüren einen besonders großen Leidensdruck was die adäquate Diagnostik und Therapie umso wichtiger macht. Diese Arbeit soll die immunologischen Aspekte die zu spontanen und habituellen Aborten führen genauer beleuchten, sowie mögliche Therapieansätze beschreiben.\r\n\r\nMethoden\r\nBei dieser Diplomarbeit handelt es sich um einen Literaturreview. Der Inhalt wurde aus verschiedenen Studien, Fachbüchern, Reviews und Journalen unter Zuhilfenahme von Internetdatenbanken wie unter anderem Pubmed und Google Scholar zusammengestellt. Um einen Überblick über die aktuell empfohlenen Therapierichtlinien zu erhalten, wurden die aktuell gültigen Leitlinien, sowie Artikel aus Journalen und Studien herangezogen. \r\n\r\nDiskussion\r\nDie Studien zu den aktuell in Forschung befindlichen Therapieoptionen sind heterogen in ihren Einschlusskriterien und aufgrund von zu geringer Stichprobengröße wenig repräsentativ. Es sind grundsätzlich mehr Studien in hoher Qualität zu jeder der genannten Therapieansätze nötig, um wirklich wissenschaftlich fundierte Daten zu den einzelnen Möglichkeiten zu erhalten. While CB is considered as a potential early marker for detecting developmental disorders, no clinical application has been widely implemented to date. A comprehensive study on typical babbling development is essential to establish a foundation for identifying differences in infants with atypical language development. In this study, the vocalizations of a typically developing female infant were analyzed, based on 599 minutes of home audio-video recordings taken between 4 and 12 months of age. A total of 2,852 vocalizations were classified into eight canonical babbling subtypes by differentiating between single and multiple syllable vocalizations, conforming and non-conforming to the target language German, and rapid versus slow formant transitions. For each month and week, the CB ratio (CBR), the babbling length ratio, and the proportion of each babbling subtype relative to the total number or length of vocalizations were calculated. The analysis revealed a typical CB onset (CBO) at nine months of age and showed that babbling length analysis provided no additional insights beyond the CBR. From 32 to 46 weeks of age, multiple syllable vocalizations consistently outnumbered single syllable ones. At the age of 37 weeks conforming to target language began to be frequently observed. This study suggests that there may be distinct steps in an infant's babbling development, each with a specific onset. Es fehlen jedoch Biomarker, die das Behandlungsergebnis\r\nvorhersagen können. Ziel der Studie ist es, einen Biomarker aus dem Differentialblutbild zu\r\nidentifizieren, welcher vor der Therapie im Routinelabor erhobenen wurde und mit dem das\r\nTherapieansprechen oder die Behandlungspersistenz assoziiert ist.\r\nMethoden:\r\nDiese retrospektive, monozentrische Studie wurde an der Medizinischen Universität Graz\r\ndurchgeführt. Eingeschlossen wurden biologika-naive CED-Patient*innen, die zwischen\r\n1999 und 2020 eine Anti-TNF-Antikörper-Therapie begonnen haben und mindestens einer\r\nder untersuchten Werte aus dem Differenzialblutbild zu Therapiebeginn oder nach drei\r\nMonaten verfügbar war. Es wurden Therapiepersistenz, Therapieansprechen und\r\nTherapieversagen bewertet. Klinische Remission wurde definiert als ein Harvey-Bradshaw-\r\nIndex von <5 für Morbus Crohn und einen partial adapted Mayo-Score von ≤1 für Colitis\r\nulcerosa. CRP-Remission war definiert mit <5mg/l.\r\nErgebnisse:\r\nInsgesamt wurden 268 Patient*innen (Alter: Median [IQR] 33 [24, 43] Jahre, 47 % weiblich,\r\nmediane Krankheitsdauer: 51 [11, 135] Monate) in die Analyse eingeschlossen. Von diesen\r\nlitten 159 (59 %) an Morbus Crohn, 90 (34 %) an Colitis Ulcerosa und 19 (7 %) an einer\r\nnicht klassifizierbaren CED (IBDU). Die meisten Patient*innen erhielten Infliximab (71%)\r\noder Adalimumab (27%). Die Therapiepersistenz betrug nach drei Monaten 78 %, nach\r\nzwölf Monaten 60 % und am Ende des follow-up 30%. Die meisten der analysierten\r\nBehandlungsendpunkte waren nicht mit den Werten des Differentialblutbildes zu Beginn der\r\nTherapie assoziiert. Bei Patient*innen mit Morbus Crohn war aber die CRP-Remission nach\r\ndrei Monaten mit einer niedrigeren Thrombozyten und einer höheren Lymphozyten Anzahl\r\nsowie mit einem niedrigeren Verhältnis von neutrophilen Granulozyten zu Lymphozyten\r\nund Thrombozyten zu Lymphozyten zu Therapiebeginn verbunden. Die Kaplan-Meier-\r\nAnalyse zeigte, dass CED-Patient*innen mit höheren Ausgangswerten der eosinophilen\r\nGranulozyten nach 12 Monaten Behandlung höhere Therapieversagensraten aufwiesen.\r\n5\r\nSchlussfolgerung:\r\nBei CED-Patient*innen sind die Parameter des Differentialblutbildes nur begrenzt geeignet,\r\num den Erfolg der folgenden Anti-TNF-Antikörper-Therapie vorherzusagen.
