Die Funktion von SIRT4, ein mitochondrial lokalisiertes Sirtuin, ist bisher wenig verstanden, aber scheint eine Rolle in der Regulation der zellulären Fettsäureverwertung und in der Anpassung an zelluläre Stressoren zu spielen. Ziel dieser Arbeit war es, den Einfluss einer Sirt4Überexpression auf den kardialen Lipidmetabolismus und die Entwicklung einer kardialen Hypertrophie unter Bedingungen einer fettreichen Diät im Mausmodell zu untersuchen. Es wurden dafür 6 Wochen alte global Sirt4-überexprimierende transgene Mäuse (TG) und Wildtypmäuse (WT) für 12 Wochen entweder einer fettreichen Diät (high fat diet; HFD) oder einer fettarmen Diät (low fat diet; LFD) unterzogen. Die Quantifizierung der Kardiomyozytengröße erfolgte mittels einer Wheat-Germ-Agglutinin-Färbung, und für die Bestimmung der kardialen Fibrose wurde eine Masson´s-Trichrome-Färbung verwendet. Es wurde der mRNA-Gehalt von Schlüsselenzymen und Transportern im Fettsäure- und Glukosemetabolismus mittels RT-qPCR untersucht. Die HFD führte zu einer signifikanten Zunahme der Kardiomyozytengröße, wobei es keine signifikanten Unterschiede zwischen TG und WT Mäusen gab. Weder eine HFD noch eine Überexpression von Sirt4 in TG Mäusen, war mit einem erhöhten kardialen Fibrosegehalt assoziiert. Die kardiale Triglyceridkonzentration war ebenfalls nicht unterschiedlich zwischen den Gruppen. Der kardiale mRNA-Gehalt von Proteinen der mitochondrialen Fettsäureoxidation und -imports war in WT Mäusen, aber nicht in TG Mäusen, nach der HFD erhöht. In HFD-gefütterten WT-Mäusen kam es zu keiner Veränderung des kardialen Ppar-alpha-mRNA-Gehaltes, wohingegen in TG-Mäusen eine signifikante Erniedrigung des Ppar-alpha-mRNA-Spiegels nach HFD beobachtet wurde. Der mRNA-Gehalt von Glukosetransportern war nach HFD erhöht, wobei es keinen Unterschied zwischen WT und TG Mäusen gab. Davon haben 212 Patient*innen die Kriterien für die Einbeziehung in die retrospektive Studie erfüllt. Die Datenrecherche erfolgte in der elektronischen Datenbank (EyMeD) der Universitäts-Augenklinik Graz. Die erhobenen Parameter waren: Alter zum OP-Zeitpunkt, Geschlecht, betroffenes Auge inkl. Lokalisation des Pterygiums, Typ & Klassifikation des Pterygiums, angewendete OP-Technik, Komplikationen und das Auftreten eines Rezidivs in einem Nachbeobachtungszeitraum von bis zu 12 Monaten. Außerdem wurden vereinzelt die Auswirkungen einer postoperativen Bestrahlung mit Strontium 90 auf die Rezidivrate analysiert.\r\nErgebnisse:\r\n212 Augen von 212 Patient*innen mit einem durchschnittlichen Alter von 60 Jahren (SD: 13,7) wurden in die Studie einbezogen. Davon waren 75 (35%) weiblich und 137 (65%) männlich. In 114 (54%) Fällen war das rechte und in 98 (46%) das linke Auge betroffen; 206 (97%) wiesen eine nasale und 6 (3%) eine temporale Lokalisation auf. Bei 50 (23,58%) Patient*innen kam die SE+VP-Technik, bei 122 (57,55%) die CAGT/BHT, bei 36 (16,98%) die AMT und bei 4 (1,89%) die mSLET zur Anwendung. Insgesamt ist von den 212 Operationen in 51 (24,06 %) Fällen ein klinisch relevantes Rezidiv zu verzeichnen. In der SE+VP-Gruppe ergab sich eine 26%ige; in der CAGT/BHT-Gruppe eine 19,67%ige; in der AMT-Gruppe eine 33,33%ige und in der mSLET-Gruppe eine 50%ige Rezidivrate. Ein gehäuftes Rezidiv-Vorkommen ließ sich in einem Nachbeobachtungszeitraum von bis zu 7 Monaten feststellen. Insgesamt waren von den 212 Operationen bei 12 (5,66 %) Personen Komplikationen (intra- bzw. postoperativ) zu verzeichnen. Bei den 51 Rezidiven lag fast bei der Hälfte, nämlich bei 23 (45,1%) bereits präoperativ ein Rezidiv-Pterygium als Ausgangsbefund vor. Von den 158 präoperativen Primärpterygia sind im Gegensatz nur 28 (17,7%) rezidiviert. Bei reiner Betrachtung der Primärpterygia in der CAGT/BHT-Gruppe konnte sogar eine geringere Rezidivrate von 17% festgestellt werden. Bei den 24 Rezidiven in der CAGT/BHT-Gruppe sind 14 (58,3%) ausschließlich mit Nähten als Fixationsmaterial versorgt worden; bei den restlichen 10 (41,7%) kam zusätzlich Fibrinkleber für die Fixierung des Bindehauttransplantats zur Anwendung. 20 Patient*innen (mit ungünstigeren Ausgangsbedingungen) erhielten postoperativ eine adjuvante Therapie mit Strontium90, davon bekamen 4 (20%) innerhalb eines Jahres ein Rezidiv.\r\nSchlussfolgerung:\r\nAnhand der Ergebnisse der retrospektiven Datenauswertung konnte gezeigt werden, dass der Gold-Standard (CAGT/BHT) der Pterygium-Chirurgie die besten Ergebnisse in Bezug auf die Rezidivraten aufweist; diese konnte mit 19,67% eruiert werden. Sofern präoperativ ein Primärpterygium vorlag, konnte sogar eine niedrigere (17,7%) Rezidivrate verzeichnet werden. Die Datenauswertung zeigte zudem die Relevanz des präoperativen Ausgangsbefundes bzw. des präoperativen Gradings, sowie, ob es sich um ein Primärpterygium oder bereits um ein präoperatives Rezidivpterygium handelte. Anhand der Ergebnisse konnte auch gezeigt werden, dass der zusätzliche Einsatz von Fibrinkleber (und die daraus entstehende Reduktion der Anzahl an notwendigen Fixations-Nähten) weniger Rezidive zu verzeichnen hatte. Schlussfolgernd und anhand der Ergebnisse der postoperativen adjuvanten Therapie mit Strontium 90 lässt sich ein positiver Einfluss auf die Rezidivrate feststellen. Resulting z-scores for the ctDNA were compared to tumor fractions established with the ichorCNA algorithm and associated with the clinical outcome. Then, we compared the performance of ctDNA, CTCs detected by size-based CTC microfilter enrichment, and blood-based parameters CEA and CA15-3. In a second step, plasma samples and tumor tissue from 69 patients with metastatic hormone-receptor positive breast cancer were analyzed for PIK3CA hotspot mutations. A high-resolution SiMSen-Seq method was used for plasma samples covering 11 PIK3CA mutations. Again, mFAST-SeqS was used to estimate the tumor fractions in the plasma samples.\r\nResults\r\nWe observed a close correlation between mFAST-SeqS z-scores and ichorCNA ctDNA quantifications. Patients with mFAST-SeqS z-scores above three (34.5%) showed significantly worse overall (p=0.014) and progression-free survival (p=0.018) compared to patients with lower values. The baseline CTC count, CEA, and CA15-3 had no prognostic impact on the outcome of patients in the analyzed cohort. PIK3CA mutations were detected in 46.1% of the tissue samples and in 50.7% of the plasma samples. In 7 patients, PIK3CA mutations were found only in plasma. In 5 patients, such mutations found in tissue were not detectable in ctDNA, 2 of which had a low tumor fraction. Together, in 33/67 plasma samples without detectable PIK3CA mutations, 18 samples had elevated tumor fraction, implicating true negative results. In the remaining 15 patients with low tumor fraction, additional tissue analysis was needed.\r\nConclusion\r\nHere, the prognostic impact of ctDNA levels detected with mFAST-SeqS demonstrates a tool to assess the ctDNA fraction without prior knowledge of the genetic landscape of the tumor. In addition, SiMSen-Seq-based detection of PIK3CA mutations in plasma shows advantageous concordance with the tissue analyses. Da die einzigen Therapieindikation bei den vulgären Warzen ein psychischer Leidensdruck und die Verhinderung der Ausbreitung und Ansteckung anderer Personen ist sowie gegebenenfalls eine funktionelle Störung dadurch ausgelöst wird, sollte die Behandlung hierbei nach bestem Gewissen und größter Evidenz gewählt werden. \r\nIn dieser Arbeit wurden Daten von Therapiemodalitäten, welche in den letzten Jahrzenten veröffentlicht wurden, gesammelt und analysiert. Insgesamt wurden rund 2.163\r\nPersonen, welche an Verrucae vulgares litten, in über 20 Studien erfasst. The primary treatment goal in localised STS consists of surgical resections with tumour-free margins. In case of metastatic disease, developing in 20-30% of patients with initially localised disease, however, treatment options are limited, owing to the moderate response to systemic anti-tumour agents currently available.\r\nIn the current cumulative thesis, four articles published since 2017 about diagnosis, treatment, or follow-up of patients with STS will be summarised and presented. \r\nThe first article constitutes a review about the clinical presentation, diagnosis, therapeutic management, and outcome of STS-patients. Due to unspecific clinical signs (swelling, pain, lump increasing in size over time), imaging with ultrasound and – preferably – magnetic resonance imaging with contrast agent is required to initiate further treatment. A biopsy adhering to specific onco-surgical rules confirming the suspected diagnosis is followed by wide surgical resection. Optionally, (neo)adjuvant radiotherapy and chemotherapy may be administered. \r\nThe incidence, treatment and prognosis of STS-patients with abdominal metastases (AM) is discussed in the second article. Of 769 patients with STS included in a multi-centre study, 202 developed secondary metastases (26.3%), of whom 24 presented with abdominal metastases (3.1%). Patients with liposarcoma had a significantly higher AM-risk, already highlighting differences in biological behaviour of the over 50 STS subtypes. Post-metastasis survival of patients with AM was poor, although patients undergoing metastasectomy had a better prognosis than those treated with radiotherapy, chemotherapy, or best supportive care (BSC).\r\nRelated to this, the third study discusses the independent impact of metastasectomy on survival of STS patients with metachronous metastases. Of 135 patients included in a bi-centre study, 68 underwent metastasectomy, whilst the remaining 67 patients had been treated by radiotherapy, chemotherapy, or BSC. Using inverse-probability-weighting to account for “treatment-selection” bias regarding metastasectomy vs. non-operative treatment, an independent benefit of metastasectomy on patients’ survival was observed, with 1- and 10-year survival rates of 86% and 17%, respectively, for surgically treated patients as compared with 39% and 3% for non-operatively managed STS patients with metachronous metastasis.\r\nIn the fourth article, an individual follow-up protocol for patients with extremity STS was developed within a multicentre study. The statistical models (flexible parametric competing risk regression models) developed on a test cohort of 1913 patients with intermediate or high-grade STS, and validated on an independent cohort of 1085 STS-patients, allow for assessment of patients’ individual risk for local recurrence and distant metastasis.
