            "title": "Dietary Interventions to Support Cancer Therapy",
            "abstract": "One of the biggest global health issues is cancer. Therefore, adjuvant approaches that increase the effectiveness or mitigate the side effects of current anticancer treatments are needed. In this thesis, a comprehensive and up-to-date literature overview that evaluates fasting and ketogenic diet (KD) as preventive measures and adjunct to existing cancer therapy will be presented. Fasting deprives the organism of calorically usable nutrients for a specific period and exerts profound metabolic adaptations in the host organism. This adaptive response leads to altered cellular signaling and behavior, that is exploited in cancer treatment and prevention. Similar to fasting, KD aims to lower blood glucose and induce a state of ketosis, which can be maintained for longer periods of time without nutrient deprivation. The rationale to utilize fasting or KD as preventive measures lies in their inherent ability to reduce blood glucose and insulin and induce autophagy, thereby improving insulin sensitivity. Furthermore, they lower inflammation and oxidative stress and may enhance anti-tumor immunity. Low nutrients (such as glucose) and the ketogenic state pose a serious threat to cancer cells, due to their rewired metabolism and ravaging need for nutrients to support rapid growth. Decreasing nutrients might exert an anti-Warburg effect by forcing cancer cells to use oxidative phosphorylation (OXPHOS) as a means to derive energy, which makes them more vulnerable to cancer therapies via increases in oxidative stress, DNA damage, and apoptosis. Fasting might further be able to effectively increase protective qualities in healthy cells (differential stress resistance (DSR)) while sensitizing cancer cells to toxic insults (differential stress sensitization (DSS)), as demonstrated in various preclinical cancer models. This thesis further provides an overview of ongoing clinical trials exemplifying settings that methodically integrate fasting protocols or KD in cancer patients. Overall, the current literature proposes fasting and KD as a safe and feasible option to combine with various existing therapies. Multiple plausible mechanisms are discussed as to how fasting could benefit therapies ranging from surgery, radiotherapy, immunotherapy, and conventional anti-cancer drugs. In many cases, fasting greatly reduces side effects of existing therapy regimens. In addition, dietary restriction (DR) is easy to implement at zero cost and therefore a viable clinical adjunct cancer therapy. Nevertheless, implementing dietary interventions in clinical settings faces multiple challenges, including patient heterogeneity, compliance, potential nutritional deficiencies, long-term sustainability and potential side effects.             "title": "Kieferorthopädische Miniimplantate in der Buccal-Shelf und Infrazygomatic Crest-Region\r\nEine Literaturrecherche",
            "abstract": "Hintergrund: Temporary anchorage devices haben sich als wichtiges kieferorthopädisches Hilfsmittel etabliert und haben das Behandlungsspektrum erweitert. Durch die dadurch geschaffene skelettale Verankerung sind Distalisierungen ohne laborgefertigte Apparaturen möglich geworden. Ausschlaggebend für eine erfolgreiche Therapie ist die exakte Positionierung im Ober- und Unterkiefer, sowie Insertionstechnik und die Dimension des gewählten Implantats.\r\nZielsetzung: Ziel dieser Literaturrecherche ist die Erarbeitung einer Übersicht, welche den aktuellen klinischen Stand bezüglich der Lokalisation, Insertion, Planung und der biomechanischen Belastbarkeit von kieferorthopädischen Miniimplantaten im Bereich der Buccal-Shelf-Region und Infrazygomatic Crest-Region liefert.\r\nMethode: Für die Datenerhebung und Analyse wurde eine Suchstrategie mit zuvor definierten Schlagwörtern angewandt. Die in den Datenbanken ermittelten Ergebnisse wurden tabellarisch zusammengefasst.\r\nDafür wurden in der ersten Selektionsphase die Artikel in den Datenbanken durch das Lesen des Abstracts auf ihre Relevanz hin geprüft. Passende Artikel wurden auf diese Weise ausgesucht und anschließend durch lesen des vollständigen Papers auf Eignung geprüft.\r\nIn ihrer Ausführung unzureichende Artikel wurden auf diese Weise ermittelt und aussortiert.\r\nErgebnisse: Anhand der Selektionskriterien wurden 625 Artikel nach Eingabe der Suchbegriffe in den wissenschaftlichen Datenbanken gesammelt.\r\nWeiterhin wurden die Artikel, die nicht den zuvor definierten Parametern entsprachen, ausselektiert. Nach einer anschließenden Prüfung auf Duplikate wurden 565 Artikel ausgeschlossen.\r\nVon den 60 verbliebenen Artikeln wurde der Abstract gelesen und anschließend 32 Artikel ausgemustert. Von den Verbliebenen 28 Artikeln wurde das vollständige Paper gelesen. Am Ende der Selektion verblieben 21 Artikel, die für die Erstellung der Diplomarbeit genutzt wurden.\r\nSchlussfolgerung: In der Buccal-Shelf-Region wird mehrheitlich eine Lokalisation distal vom zweiten Molaren empfohlen. Ein Vorbohren führt zur Schonung der Knochenstruktur und reduziert das postoperative Schmerzintervall. Die Angulation beträgt 30 Grad. Eine Implantatlänge von 9-12 mm und ein Schaftdurchmesser von 1.8-2.0 mm ermöglichen ein breites Behandlungsspektrum.\r\nIn der Infrazygomatic Crest-Region liegt die bevorzugte Insertionsposition auf Höhe der mesio-bukkalen Wurzel des zweiten Molaren. Selbstbohrende Implantate erzielen, aufgrund der weniger stark ausgeprägten Kortikalis, gute Ergebnisse. Der von der Mehrheit der Autoren empfohlene Insertionswinkel beträgt 60 Grad. Die Dimensionierung der Implantate liegt hier ebenfalls bei einer Länge von 9-12 mm und einem Durchmesser von 1.8-2.0 mm.",

            "title": "Gender aspects in phoniatrics: A study on the gender distribution of benign vocal fold lesions",
            "abstract": "In today`s communication society, the human voice is of great importance in both professional and private life. Voice overload and vocal misuse can lead to structural changes of the vocal folds (VF) and thus to hoarseness and limited capacity of the voice. These benign VF lesions occur in various forms. In the thesis on hand, five typical lesion types are compared regarding their incidence, their age- and gender dependency and the VF side affected: VF nodules, polyp, cyst, granuloma and Reinke´s edema.\r\n\r\nIn a retrospective cohort study, the data of all patients who had visited the Division of Phoniatrics of the ENT University Hospital Graz over a period of 13 years due to voice complaints and had been diagnosed with one of the five selected lesion types were recorded. Based on video-documented laryngoscopies, a detailed review process was conducted according to strictly defined criteria. Subsequently, the included data was subjected to statistical analysis.\r\n\r\nThe study included 535 lesions, with both genders affected equally often. However, while the included female patients were diagnosed with VF nodules and Reinke’s edema in one third each, more than half of the male patients were affected by VF polyps. The majority of the patients were of working age, while only a third of the diagnoses concerned children, adolescents or elderly. Each of the lesion types examined showed a significant association with gender, and with the exception of VF cysts, all diagnoses were also significantly related to age. Among the unilateral lesions, the right and left VF were affected equally often overall, but the distribution of the affected side was found to be gender-related.\r\n\r\nA comparison of the results with previous studies was complicated by the fact that nomenclature, definition and classification framework of benign VF lesions are still handled inconsistently today. In particular, different perspectives on these structural alterations (endoscopic, operative, histological) lead to sometimes contradictory results. Further research is needed here, but the results of the present study also have practical impact. As people of all genders and ages can be affected by secondary organic dysphonia, voice prophylactic measures are to be demanded for all populations.\r\n",

            "title": "Die Lp(a)/HDL-Ratio als prognostischer Marker bei Patient*innen mit pAVK",
            "abstract": "Einleitung: Die periphere arterielle Verschlusskrankheit (pAVK) ist eine weit verbreitete Folge der Atherosklerose mit steigender Prävalenz. Diverse Risikofaktoren führen zur Progression der Krankheit und schließlich zur Okklusion der Gefäße, was sich klinisch typischerweise als Claudicatio Intermittens, oder schlimmstenfalls als kritische Extremitätenischämie (CLI) präsentiert. High density lipoprotein (HDL) und Lipoprotein (a) (Lp(a)) gehören der Klasse der Lipoproteine an und spielen eine wichtige Rolle in Bezug auf die Entstehung und Progression von Atherosklerose und der pAVK. Diese Arbeit soll untersuchen, ob das Verhältnis von Lp(a) und HDL als prognostischer Marker für die Progression der pAVK und die Entstehung einer CLI verwendet werden kann.\r\n\r\nMethoden: In dieser retrospektiven Querschnittsanalyse wurden die Daten von 2677 Patient*innen, die an der Abteilung für Angiologie der Medizinischen Universität Graz aufgrund von pAVK behandelt wurden, analysiert. Hierbei wurden demographische Parameter, Grunderkrankungen, Begleitmedikation, etwaige Risikofaktoren sowie Laborparameter, im Speziellen die Lipidwerte Lp(a) und HDL, bzw. deren Ratio, erhoben. Die Patient*innen wurden in Patient*innen mit CLI und in Patient*innen ohne CLI aufgeteilt und bezüglich dieser Parameter analysiert.\r\n\r\nResultate: In der untersuchten Population waren 1659 (62%) Personen männlich und 1018 (38%) Personen weiblich, was einer Gesamtpopulation von 2677 Proband*innen entspricht. Über die Hälfte der Patient*innen wies Adipositas auf (55,9%) und die Mehrheit der Patient*innen gab an zu Rauchen (57,6%). In der multivariaten Regressionsanalyse zeigte sich ein signifikanter Zusammenhang zwischen Alter (p<0,001), männlichem Geschlecht (p<0,001), Diabetes mellitus (p<0,001) und Lp(a) (p=0,028) in Bezug auf eine CLI. Hinsichtlich der Lp(a)/ HDL Ratio konnte kein statistisch signifikanter Zusammenhang mit einer CLI festgestellt werden (p=0,118).\r\n\r\nSchlussfolgerung: In dieser Arbeit konnte zwar eine Korrelation zwischen erhöhten Lp(a)-Werten und einer CLI nachgewiesen werden, jedoch ist die Lp(a)/HDL Ratio bei Patient*innen mit CLI nicht signifikant höher. Somit scheint diese Ratio als prognostischer Parameter für die pAVK bzw. die Entstehung einer CLI nicht geeignet.",

            "title": "Does targeted education about HPV have a positive effect on HPV vaccination intention in a sample of 12- to 14-year-old students?\r\nReport from a high school in Graz",
            "abstract": "Background: In 2018, the WHO (World Health Organization) called for the elimination of cervical cancer. About 99% of cervical carcinomas are caused by HPV (human papillomavirus) infection and the nonavalent vaccine Gardasil®9 is thought to prevent up to 90% of HPV-associated carcinomas. To reach the WHO´s goal, a 90% vaccination coverage of all females by age 15 is one of the measures required to be taken until 2030. The Austrian Vaccination Plan recommends the 9-valent HPV vaccine to all genders starting from the age of 9. However, in Austria, in 2022 the HPV vaccination coverage (both doses) is estimated to be approximately 53% for children aged 14. No data on gender distribution amongst the vaccinated population exists. Therefore, this study investigated the (minimal) vaccination coverage (total and per gender) in a predefined sample group of high school students in Graz. Furthermore, it aimed to explore if targeted education on HPV can improve the willingness to receive the vaccine amongst students with a negative or uncertain vaccination status.\r\n\r\nMaterial & Methods: In June 2021, we conducted a prospective questionnaire-based study in a high school in Graz. 135 consent forms were handed out to the intention-to-treat population. Students with parentally signed consent forms filled out a questionnaire concerning their HPV immunization status, HPV knowledge and willingness to get vaccinated. Then we conducted a targeted education class about HPV and HPV vaccination. Successively, they filled out a second, similar, questionnaire. Inclusion criteria were a parentally signed consent form and age between 12 – 14 years. The questionnaire data was analyzed via descriptive statistics. \r\n\r\nResults: 55/135 students from the intention-to-treat population (41%) handed in signed consent forms. 54 students (40%) were included. 38/54 students (70%) were vaccinated, which leads to a minimal vaccination coverage (full immunization with two doses) within the overall intention-to-treat population of 28% (38/135 students). 3 students (6%) had received one dose by the time of the study. 77% (26/34) of all female students were vaccinated, 85% (29/34) received the 1st dose. \r\nBefore receiving targeted education, knowledge about HPV and the HPV vaccine was reported by 18 (33%) and 27 students (50%), respectively. After the educational class, 54 (100%) students knew about HPV and 53 (98.15%) knew what the HPV vaccine was.\r\nBefore the educational class, 10/13 (77%) students with a negative or uncertain immunization status, would choose to get vaccinated in a cost-reduced vaccination program. After being educated, 11/13 (85%) students would get vaccinated. \r\n\r\nConclusion: In our cohort, only one of 13 unvaccinated or uncertain whether vaccinated students was willing to get vaccinated after targeted education. Our results do not permit conclusive recommendations on whether targeted education can raise HPV vaccination intention. However, targeted education did lead to an increase in the number of students with knowledge regarding primary prevention of HPV-associated cancers. Therefore, education may increase knowledge regarding HPV and the vaccine, which might possibly have an indirect effect and lead to increased HPV vaccination intention in the long run. Furthermore, as we only know the rate of vaccination of the study participants (n=54), we cannot state the actual vaccination rate within the group that was intended to be investigated (n=135). \r\nFurther studies with larger sample sizes, in multiple schools from diverse settings, randomization into interventional and control groups and analysis of demographic background data would be necessary to answer these questions more conclusively.\r\n",
