Dabei zeigte sich, dass bestimmte proinflammatorische Zytokine einerseits mit der Größe eines von einem Schlaganfall betroffenen Gehirnareals korrelieren können und andererseits Wirkungen auf die Blutgerinnung und die Beschaffenheit der Gefäßwände zeigen können. So zeichnete sich immer mehr ab, dass Zytokine nicht nur direkt mit der ischämischen Schädigung des Hirngewebes in Zusammenhang stehen, sondern auch wesentlichen Einfluss auf Veränderungen der Gefäßwände haben, die einer der Hauptrisikofaktoren für einen Schlaganfall sind.\r\n\r\nWir untersuchten in unserer Studie die Serumkonzentrationen des Immunaktivierungsmarkers Neopterin und folgender proinflammatorischer Zytokine oder ihrer Rezeptoren: löslicher Tumornekrosefaktor (sTNFR-2), Interleukin-2 löslicher Rezeptor (IL-2sR), und Interleukin-6 (IL-6). Die Unterschiede in den Konzentrationen zwischen reversiblen (TIA/PRIND) (n=10) und irreversiblen (n=25) Schlaganfallformen (completed stroke) sowie zu zerebralen Blutungen (n=12) wurden untersucht. Wir unterteilten die Schlaganfallpatienten in solche mit (n=9) und solche ohne Infektion (n=16). Diese Fragestellung sollte dazu dienen, erstmals den Einfluss von alltäglichen Entzündungen der Atem- oder Harnwege auf den Verlauf von proinflammatorischen Zytokinen nach einem akuten Insultgeschehen zu messen. Die Messungen erfolgten innerhalb von 48 Stunden und an den Tagen 2, 3, 4, 9-11 und 13-15 nach dem Beginn der klinischen Symptomatik. Unsere Studie brachte folgende Ergebnisse: Die Serumkonzentrationen von Interleukin-6 liegen bei Patienten mit einer ischämischen oder hämorrhagischen Schädigung des Hirngewebes wesentlich höher als bei Patienten mit einem reversiblen neurologischen Defizit. Zwischen Insultpatienten mit und ohne Infekt konnte am dritten Tag ein statistisch signifikanter Unterschied zwischen den Serumkonzentrationen von IL-2 sR und Neopterin nachgewiesen werden. Weiters konnten signifikante Korrelationen von IL-2 sR, Neopterin, TNRF-2, und IL-6 mit wesentlichen Faktoren der Blutgerinnung wie Fibrinogen, D-Dimer, Plasminogen, Antithrombin III und Protein C und S nachgewiesen werden. Die signifikanten Unterschiede in den Konzentrationen von IL-2R am Tag 3 zwischen Insultpatienten mit und ohne Infekt, und die von allen untersuchten Parametern nachgewiesenen Einflüsse auf die Blutgerinnung ergeben wesentliche Hinweise auf die Beteiligung von bakteriellen und/oder viralen Infekten vor und zum akuten Isultereignis.", "authors": [ "Saurugg, R" ], "year": 2001, "source": "[ Dissertation ] Graz; 2001.
