SLs maintain BBB function by structural and signaling input. In a first step, I tested primers for enzymes involved in SL turnover in subsequent conventional RT-PCR and RT-qPCR reactions. I also tested whether 2-chlorohexadecanal (2-ClHDA) – a myeloperoxidase-derived chlorinated fatty aldehyde – regulates the expression of these genes.\r\nBackground: The neurovascular unit (NVU) separates the brain from peripheral circulation and regulates the neuronal microenvironment. The cerebral microvessels are sensitive to oxidative stress (i.e., aberrant redox control). Oxidative processes can alter the integrity of the NVU and are implicated in the pathophysiological sequelae of neurodegenerative diseases. The phagocyte heme peroxidase, myeloperoxidase (MPO) converts H2O2 into the strong oxidant hypochlorous acid (HOCl) in the presence of chloride ions (Cl-). HOCl can attack and modify macromolecules generating a series of chlorinated products that can inflict tissue/organ damage on the host.\r\nMethods: RNA isolated from pBMVEC was reverse transcribed into complementary DNA of enzymes involved in SL metabolism. The cDNA was used in PCR and real time quantitative PCR (qPCR) analyses and in a first set of experiments primer efficiency was analyzed. I then went on to assess whether 2-ClHDA regulates the transcription of these genes. Cell cultures of pBMVEC were incubated in the absence or presence of 2-ClHDA and changes in gene expression were analyzed.\r\nResults: PCR conditions for the following set of genes involved in SL synthesis/degradation were established: serine palmitoyltransferase 1, serine palmitoyltransferase 2, ceramide synthase 5, ceramide synthase 6, sphingomyelin synthase 2, sphingosine kinase 2, alkaline ceramidase 1, alkaline ceramidase 2, alkaline ceramidase 3, neutral ceramidase2, sphingomyelin phosphodiesterase, sphingosine-1-phosphate phosphatase 1, and sphingosine-1-phosphate lyase 1. In response to 2-ClHDA, expression of CerS1, CerS6, SPHK2, and SGPL1 were upregulated.\r\nConclusion: The results obtained during the present pilot study demonstrate the expression of genes known to be involved in SL homeostasis. The treatment is directed at the causes. Although there have been important technological advances for the treatment of cardiac arrhythmias, antiarrhythmic drugs(AADs) remains the cornerstone treatment for patients with arrhythmias. AADs can modify impulse generation and conduction to prevent arrhythmias from occurring or re-occurring and reduce symptoms that come with them.\r\n\r\nIn the past few decades, the understanding of the molecular and cellular basis of cardiac electrophysiology and mechanisms underlying cardiac arrhythmias has increased immensely. Research is needed to develop, subclassify, and identify new therapeutic targets aiming to provide mechanism-based therapy.\r\n\r\nAlthough they have been many attempts to provide a more mechanistic classification such as the Sicilian gambit in 1991, AADs remain most commonly classified according to Vaughan Williams. AADs are divided into 4 groups: sodium channel blockers (class I), beta-blockers (class II), potassium channel blockers (class III), and calcium channel blockers (class IV). There are also unclassified AADs such as adenosine, which do not belong to the above mentioned groups.\r\n\r\nMost of the current available AADs have different electrophysiological targets which may give an anti or pro-arrhythmic effect that means while trying to suppress arrhythmias with AADs, AADs themselves, can lead to other (potentially more dangerous) arrhythmias. For this reason, caution should be taken before, during, and after administering the medication. Regular re-evaluation of the drug is recommended. The clinical significance of the AADs lies in the type of arrhythmias that each drug or class can treat and the potential side effects of each overall classification or individual medication. Trials such as CAST and SWORD have shown that class I and class III AADs causes excess mortality in patients where benefits were expected. Beta-blockers have shown to prolong life compared to other AADs. They reduce sudden death and total mortality in patients with heart failure. Class IV AADs may be inappropriate in heart failure because they decrease conduction in AV node and shorten the cardiac action potential. They have not shown to reduce mortality and in case of nifedipine, may even increase mortality in patients after MI.\r\n\r\nMany cardiac medicines appear safe for use during pregnancy and the time mothers breastfeed their infants. However, the evidence is lacking for some drugs and many of them carry risks of adverse events. Shared decision making, considering the risks and benefits of each AAD is vital to help pregnant women weigh up potential risks to themselves and their unborn baby.\r\n\r\nIn elderly patients, there are changes in pharmacokinetic and pharmacodynamic, not only for AADs but for every drug taken as well. Aging changes absorption, distribution,\r\nmetabolism, and elimination of drugs. So the liver and kidney function should be regularly checked to avoid side effects and the dose may be needed to be adjusted. Furthermore, the choice of AADs in the elderly is complicated due to the presence of co-morbidities and\r\npolypharmacy, which may cause drug interactions.\r\n\r\nWhen the patient under current AAD remains symptomatic and his/her situation does not improve, there are other treatments which might relieve the symptoms or cure arrhythmias. Ein Hauptschwerpunkt der Untersuchung lag dabei auf dem Einfluss unterschiedlich erfahrener Operateurinnen/Operateure auf diese Präzision.\r\n\r\nMethoden: Ein Referenzmodell mit Einzelzahnlücke in Regio 21 wurde gedruckt und anschließend mit Superhartgips (Fujirock® EP; GC, Leuven, Belgien) 25-fach dubliert. Die Planung erfolgte virtuell in Implantatplanungssoftware (Implant Studio; 3Shape, Kopenhagen, Dänemark) und die physische Umsetzung der Implantationsschienen über 3-D-Druckersoftware (Composer; Asiga, Alexandria, New South Wales, Australien) per 3-D-Drucker (Pro 2; Asiga). Anschließend erfolgte mit 25 dieser Schienen jeweils eine vollständig geführte Implantation (Xive®; Dentsply Sirona, Charlotte, North Carolina, USA) in eines der Gipsmodelle. 23 implantierenden Ärztinnen/Ärzten (n = 11) und Studentinnen/Studenten (n = 12) waren der Insertionsprozess und das einzuhaltende Bohrprotokoll zuvor erklärt und grafisch vor Augen geführt worden. Für die beiden restlichen Modelle wurde, um etwaige Materialfehler im Vergleich auszuschließen, das Implantatlager in Regio 12 in Form von Kunststoff (n = 1) oder simuliertem Knochen (n = 1) aufbereitet. Jedes inserierte Implantat wurde mit einem Scankörper (T1405; Medentical Care, Regau, Österreich) versehen, das Modell per Laborscanner (D2000®; 3Shape) digitalisiert, der Scan in Modellbausoftware (Model Builder; 3Shape) bearbeitet, das Resultat als STL-Datei ausgegeben, das digitalisierte Model danach in CAD-Software (Solidworks®; Dassault Système, Vélizy-Villacoublay, Frankreich) mit dem Referenzmodell überlagert und das Modellpaar gegeneinander vermessen.\r\n\r\nResultate: Die Studentinnen/Studenten produzierten an der Implantatschulter bzw. Implantatspitze mittlere Abweichungen von mesiodistal 0,11 bzw. 0,20 mm, orovestibulär 0,15 bzw. 0,11 mm und apikokoronal 0,86 bzw. 0,9 mm. Die Ärztinnen/Ärzte lieferten analog hierzu mittlere Abweichungen von mesiodistal 0,05 bzw. 0,18 mm, orovestibulär 0,03 bzw. 0,06 mm und apikokoronal 0,92 bzw. 0,9 mm. Die mittlere Abweichung von der Achsenausrichtung im Referenzmodell betrug bei den Studentinnen/Studenten 2,54° und bei den Ärztinnen/Ärzten 2,38°.\r\n\r\nSchlussfolgerung: Gute Annäherungen der Ist- an die Soll-Positionen erzielen bei vollständig schablonengeführten Implantationen nicht nur erfahrene Behandlerinnen/Behandler, sondern auch Studierende ohne praktische Erfahrung. Aufgrund seines großen Nebenwirkungsprofils wurde der Vorläufer Benziodaron verworfen. Einige Jahre später entdeckte man als verträglicheren und potenteren Vasodilatator das heutige Amiodaron und die Popularität des Arzneimittels nahm ab 1975 zu. Bis heute wird das Arzneimittel regelmäßig verwendet und ist vor allem aus der Notfallmedizin nicht mehr wegzudenken.\r\nDas stark lipophile Medikament besitzt einen Iodanteil von 37,7 Gewichtsprozent, was einerseits namensgebend ist, andererseits auch zu spezifischen Nebenwirkungen führen kann. Für die hepatische Metabolisierung sorgen die Enzyme CYP3A4 sowie CYP2C6, die Ausscheidung erfolgt biliär. Eine hohe Gewebeakkumulation bedingt eine verlängerte Eliminationshalbwertszeit, die bei dem Einsatz des Arzneimittels stets im Hinterkopf behalten werden muss. Aufgrund des dadurch verzögerten Eintrittes der Wirkung von Amiodaron wird eine Loading-Phase am Beginn der Langzeittherapien gestartet, die Hauptindikation dazu ist das Vorhofflimmern.\r\nIm Advanced Life Support kommt Amiodaron nach drei frustranen Schockabgaben des Defibrillators in einer Dosierung von 300 mg sowie in Kombination mit Adrenalin zum Einsatz. Nach zwei weiteren erfolglosen Defibrillationsversuchen wird zur erneuten Applikation von 150 mg geraten.\r\nDas breite Nebenwirkungsprofil des Arzneimittels umfasst Schädigungen fast aller Organe, am stärksten – aber nicht häufigsten – ist die Lunge betroffen. Lungenfibrosen bis hin zur Entwicklung eines ARDS wurden als Konsequenz einer Amiodaroneinnahme bereits beschrieben. Des Weiteren können Leber, Schilddrüse, Augen, Haut, sowie das Gehirn und die Gefäße von toxischen Wirkungen betroffen sein. Um dem entgegenzuwirken, wird ein strenges Monitoring bei Patienten unter Langzeittherapie empfohlen.\r\nInteraktionen mit Grapefruitsaft und anderen Medikamenten müssen im Hinterkopf des Behandlers bleiben: hier geht es hauptsächlich um Hemmungen der für die Metabolisierung verantwortlichen Enzyme, woraus ein erhöhter Plasmaspiegel von Amiodaron oder den anderen beteiligten Arzneimitteln resultiert.\r\nAufgrund der Gefahr eines Stillstandes des Sinusknotens, ist Amiodaron bei schrittmacherlosen Patienten mit AV-Block zweiten oder dritten Grades kontraindiziert. Durch die namensgebende strukturelle Ähnlichkeit des Arzneimittels mit Schilddrüsenhormonen, darf Amiodaron bei Patienten die unter einer Hyper- bzw. Permanent metal implants like titanium, stainless steel and other metallic alloys are used for fracture treatment. Currently, implant removal is performed frequently in order to avoid possible long term side effects such as growth disturbance, discomfort or accumulation of corrosion products. Accordingly, another anesthesia and hospitalization is needed. New implant materials composed of magnesium (Mg) and Mg alloys exhibit promising features including excellent biocompatibility and mechanical properties meeting the demands for an enhanced fracture stabilizing implant in pediatric orthopedics and traumatology. Moreover, Mg-based implants dissolve after a certain period of time to their full extent, rendering a second surgery for implant removal unnecessary. In this study we investigated the in vivo degradation behavior of an Mg-based alloy synthesized with zinc and calcium (ZX00) in rats over a time period of 24 weeks. \r\nMethods: Seven, female 6-weeks old Sprague Dawley rats underwent bilateral and transcortical implantation of cylindrical ZX00 pins into the proximal metaphysis of the tibia. Three Sprague Dawley rats underwent the same surgical intervention without implant, thereby serving as sham controls. To observe implant degradation and osseointegration, in vivo low-medium resolution micro-computed tomography was performed immediately after as well as 2, 6, 12, 15, 18 and 24 weeks after implantation. Images were three-dimensionally reconstructed and implant volume, implant surface and hydrogen gas formation was calculated using MIMICS Software, version 22.\r\nResults: Our results showed a good osseointegration and homogenous degradation behavior of ZX00 with appropriate hydrogen gas formation. Although implants were inserted in close proximity to the growth plate, no negative effect on longitudinal bone growth could be observed.\r\nConclusion: We showed homogenous degradation, adequate gas evolution and good osseointegration of Mg-based ZX00 implants without bone growth determination. Mit der Steigerung der Lebenserwartung ändern sich auch die Ursachen für Krankheit und Tod. Viele Menschen sind vor allem an chronischen Krankheiten wie Herz- Kreislauferkrankungen, Diabetes oder Demenz erkrankt. Für die verschiedenen Berufsgruppen im Gesundheitswesen und vor allem auch für pflegende Angehörige zählt Demenz zu den häufigsten psychischen, sowie herausforderndsten altersbedingten Erkrankungen. Ziel: Das Ziel dieser Arbeit ist es, darzustellen welche Bedürfnisse pflegende Angehörige von Personen mit Demenz haben und Unterstützungsangebote aufzuzeigen, um ihnen eine angemessene Basis für die Pflege und Betreuung zu Hause bieten zu können. Methode: Das Design dieser Arbeit ist ein integratives Review, dessen Durchführung an der Grundlage der Empfehlungen von Whittemore und Knafl (2005) angelehnt ist. Die Literatursuche nach relevanten Studien wurde in den Datenbanken PubMed, Cumulative Index to Nursing & Allied Health Literature (CINAHL) und Web of Science, sowie durch eine Internetrecherche in Google Scholar durchgeführt. Zur Überprüfung der Studienqualität wurden diese anhand des Mixed Methods Appraisal Tool (MMAT) von Hong et al. (2018) kritisch beurteilt. Ergebnisse: Die 14 inkludierten Studien zur Beantwortung der ersten Forschungsfrage zeigten unter anderem auf, dass Pflege und Betreuung“, „Aufrechterhaltung des eigenen Wohlbefindens“ sowie „Finanzielle Angelegenheiten“, Bedürfnisse sind, welche für Angehörige bei der Versorgung von Personen mit Demenz von Bedeutung sind. Die 5 inkludierten Studien zur Beantwortung der zweiten Forschungsfrage zeigten auf, dass Internetforen, Selbsthilfegruppen, Informationstechnologien und Websites mögliche Unterstützungsangebote dafür sind. Diskussion: Es sollte beachtet werden, dass pflegende Angehörige im Bedürfnisbereich „Bedarf an Wissen und Informationen“ über einige Probleme berichteten. Die bereits bestehenden Strukturen scheinen den hohen Bedarf in Bezug auf Wissen und Information speziell zum Krankheitsbild Demenz nicht zu decken.
