Neben der Notwendigkeit der Therapiesteuerung – wie Blutzuckermessungen, Insulingaben und Ernährungsmaßnahmen – sind Betroffene oft auch mit stigmatisierenden Herausforderungen im täglichen Leben konfrontiert. Diese Anforderungen können zu emotionalen Belastungssituationen führen, welche als Diabetes „Distress“ zusammengefasst werden können. Die österreichische Selbsthilfe-Organisation „Diabär“ veranstaltet regelmäßig ein einwöchiges Abenteuercamp in den österreichischen Alpen für Jugendliche (12-18 Jahre) mit T1D. Der Schwerpunkt liegt auf erlebnisorientierten Aktivitäten und Spaß ohne strengem Schulungscharakter, um den eigenen Umgang mit der chronischen Erkrankung unter Begleitung zu stärken und Kompetenzen zum Selbstmanagement zu forcieren. Im Gegensatz zu traditionellen Schulungscamps wird die formale Wissensvermittlung bewusst minimiert. Einmal täglich erfolgt jedoch gemeinsam mit dem medizinischen Fachpersonal eine Analyse der am Vortag aufgezeichneten Glukosewerte, welche mittels kontinuierlicher Glukosemonitoring (CGM) Systemen erhoben und retrospektiv analysiert werden. Ein Großteil leidet zusätzlich an motorischer Fatigue – einer ausgeprägten körperlichen Erschöpfung, die unabhängig von physischer Belastung oder Einschränkung auftreten kann. Die Fatigue wird oft als größte Alltagsbeeinträchtigung wahrgenommen. Zusätz-lich können Übelkeit, Erbrechen, Licht- oder Geräuschempfindlichkeit auftreten. Un-terschieden wird zwischen einer Migräne mit Aura und ohne Aura. Während man beim Auftreten von 0-14 Kopfschmerztagen pro Monat von episodischer Migräne spricht, treten die Kopfschmerzen bei der chronischen Migräne an mindestens 15 Tagen pro Monat auf. Es gibt außerdem noch Sonderformen wie die familiäre hemiplegische Migräne oder die menstruelle Migräne. Weltweit sind mehr als eine Milliarde Men-schen von der multifaktoriell bedingten Migräne betroffen, das ergibt eine Prävalenz von 14%. Die Diagnostik umfasst eine genaue Anamnese mit Fragebogen und Kopf-schmerztagebuch und anhand der internationalen Klassifikationskriterien ICHD-3 wird in Folge eine Diagnose gestellt. Die Therapie umfasst Medikamente zur Akuttherapie und Medikamente zur Migräneprophylaxe. The pathophysiology is incompletely understood, therefore specific therapeutic approaches are not available yet. Cerebrovascular reactivity (CVR), increased arterial pulsatility and arterial stiffness are possible mechanisms of CSVD. CSVD causes lacunar ischaemic stroke, depicted as recent small subcortical infarcts (RSSI) on neuroimaging. Little is known about the incidence and aetiology of recurrent cerebrovascular events in RSSI patients.
