The role of eukaryotic initiation factors (eIFs) in the pathophysiology of psoriasis is poorly understood. Through regulation of protein synthesis, eIFs affect many cellular processes, which are essential for normal skin function. Dysregulation in protein synthesis and can explain the uncontrolled skin growth observed in psoriasis. We therefore believe that eIFs are involved in pathophysiology of psoriasis and might be promising therapeutic targets. Here we study the pathophysiology of psoriasis in human skin and in imiquimod (IMQ) mouse model. Topical application of IMQ induces psoriasis-like skin inflammation in mice, which closely resemble human psoriasis lesions. We further examine the effect of ultraviolet B (UVB) treatment on eIF expression in IMQ mice, since the exact mechanisms of UVB treatment are not fully understood. Aufgrund von Impfungen sind Infektionskrankheiten und Sterblichkeit statistisch gesehen in den letzten 10 Jahren rückläufig.\nDiese Diplomarbeit befasst sich mit wichtigen Infektionskrankheiten im Kindesalter und den dazugehörigen medikamentösen Therapieformen. Der wesentliche Kern besteht darin aufzuzeigen, dass Impfungen einen elementaren und besseren Schutz gegen Infektionskrankheiten bieten. Es gibt einen Rückblick und einen Vergleich zwischen den europäischen Ländern und den Nachfolgestaaten der Sowjetunion.\nMasern und Rotavirusdiarrhoe gehören zu den häufigsten Infektionskrankheiten mit weltweit hoher Sterblichkeitsrate. Laut WHO gab es 2014 global 114.900 Todesfälle durch Masern bei Kindern unter fünf Jahren über 314 Tote pro Tag(3) und 215.000 Todesfälle durch Rotavirusinfektionen im Jahr 2013, etwa 1440 Tote pro Tag. Aufgrund von Impfmüdigkeit in der Bevölkerung und steigender Migrationsströme kam es immer wieder zu Ausbrüchen von Infektionskrankheiten in Europa. Im Jahr 2015 wurden in Österreich 309, in Deutschland 2.464 Fälle von Masern registriert. In Kasachstan und Russland, wo vermehrt Wiedererkrankungen und eine höhere Mortalität bei Rotavirusdiarrhoe und Pneumokokkenfällen beobachtet wurden, gab es hingegen nur Einzelfälle von Masern, Mumps und Röteln.\nDer Schutz gegen Kinderinfektionskrankheiten ist wichtig – auch für Erwachsene. Eine Rötel-Infektion bei schwangeren Frauen kann zu Spontanabort, Frühgeburt oder kongenitalem Röteln-Syndrom(CRS) führen. Mumpsorchitis kann bei Jugendlichen Unfruchtbarkeit verursachen. \nImmunisierung ist der beste Schutz vor schweren sekundären bakteriellen und neurologischen Komplikationen und tödlicher SSPE nach einer Maserninfektion. Wichtig ist die Herdimmunität, dabei werden nicht nur geimpfte Personen geschützt. Krankheiten wie Masern, Röteln oder Mumps können durch einen ausreichenden Impfschutz der gesamten Bevölkerung in einer Region, bzw. einem Land weitgehend eliminiert werden.\nWichtig ist einerseits die Überwachung von Infektionskrankheiten und andererseits die Immunisierung, sie hilft neue Ausbrüche rechtzeitig zu erkennen und präventive Maßnahmen zu setzen. In the presence of FMS-related tyrosine kinase 3 ligand (FLT3L), multipotent progenitors (MPPs) and common myeloid progenitors (CMPs) differentiate to monocytic dendritic progenitors (MDPs) and subsequently to common dendritic precursors (CDPs). Finally, CDPs differentiate either into conventional DCs or plasmocytoid DCs. C/EBPα (CCAAT/enhancer binding protein alpha) is a basic region-leucine zipper transcription factor and indispensable for formation of mature neutrophils and eosinophils. Mice with a homozygous deletion of the Cebpa gene have normal to elevated numbers of CMPs, but completely lack downstream granulocyte/monocyte precursors (GMPs) and all subsequent granulocytic stages indicating that C/EBPα is essential for early myelopoiesis by enabling transition of CMPs to GMPs. Though the gene is well studied in myelopoiesis, little is known about its role in early steps of development of DCs. \r\nUsing a CebpaCre-EYFP reporter mouse model, we identified the majority of splenic conventional DCs to be derived from Cebpa-expressing hematopoietic stem and progenitor cells (HSPCs). Detailed analysis of DC progenitor stages revealed an increase of Cebpa/EYFP+ cells in these mice from early progenitors, such as MPPs via CMPs to its maximum at the MDP stage. Since in later stages no further increase of Cebpa/EYFP+ cells was observed, these results together with data from published gene expression studies clearly indicated Cebpa expression during early DC development. To assess a functional role of C/EBP in early DC development, we next used an inducible bone marrow-specific Cebpa knockout (KO) mouse model (Mx1Cre-CebpaF/F mice), isolated HSPCs of these mice and studied their capacity to form mature DCs after in vitro culture with FLT3L. While Cebpa wildtype (WT) HSPCs produced high numbers of mature DCs after eight days of culture, HSPCs from Cebpa KO mice exhibited a profound reduction in DC numbers. In a step-wise analysis, we observed decreased formation of MDPs and a block in their transition to CDPs in KO HSPCs, whereas WT HSPCs underwent successful transition through the MDP and CDP stages towards mature DCs. Gene expression analysis of FLT3L-stimulated HSPCs from Cebpa WT and KO mice revealed a significant change in transcription factors associated with DC development, like IRF8 and PU.1, inflammatory cytokines like TNFα and IL1β and several genes related to DC development, such as Cx3cr1, as well as related to the TNFα- and NFκB-pathways. Interestingly, TNFα was also reduced in supernatants of Cebpa KO HSPCs stimulated with FLT3L. Accordingly, addition of TNFα to HSPCs cultured in vitro with FLT3L partially restored DC formation in Cebpa KO cells by upregulating Sfpi1 (the gene for PU.1) and Cx3cr1 expression. Mixed lymphocyte reactions showed that DCs generated from Cebpa KO HSPCs by TNFα addition were functionally fully active. \r\nIn conclusion, we identified a critical role of C/EBPα in early DC development in this study. Expression of Cebpa was indispensable for the formation of MDPs and their transition to CDPs during FLT3L-induced DC development. The inflammatory cytokine TNFα was able to partially overcome this maturation block probably by interfering with Sfpi1 and Cx3cr1 expression. Auf Grund der im Kindes- und Jugendlichenalter oftmals atypischen Präsentation kann sich die Unterscheidung der beiden schwierig gestalten. Ziel der vorliegenden Studie war die Analyse histopathologischer Merkmale in Biopsien aus dem terminalen Ileum bei Kindern und Jugendlichen mit CED und deren Korrelation mit der klinischen Diagnose.\r\nMaterial und Methoden:\r\nInsgesamt wurden 130 PatientInnen mit CED und Ileocolonoskopie mit Biopsieentnahme (n=216), welche im Zeitraum vom 01.01.2002 bis 31.12.2016 an der Universitätsklinik für Kinder- und Jugendheilkunde Graz erfolgte, in die Studie eingeschlossen. Die histopathologischen Befunde wurden im Diagnostik- und Forschungsinstitut für Pathologie der Medizinischen Universität Graz retrospektiv ausgewertet. Die verschiedenen Merkmale wurden einzeln und paarweise kombiniert hinsichtlich Sensitivität, Spezifität und positiv prädiktiver Werte für MC untersucht. \r\nErgebnisse:\r\nZu den erhobenen Merkmalen zählten neutrophile Granulozyten in der Lamina propria (LP; 52,8 %; n=114), chronisches Entzündungsinfiltrat (44,4 %; n=96), Architekturstörungen (43,1 %; n=93), neutrophile Granulozyten im Epithel (35,6 %; n=77), Eosinophilie der LP (32,9 %; n=71), basale Plasmazytose (32,4 %; n=70), Epitheldefekte (31,9 %; n=69), Epitheloidzellen (25,0 %; n=54), epitheloidzellige Granulome (16,2 %; n=35) und Pylorusdrüsenmetaplasie (9,3 %; n=20). \r\nFür die einzelnen Merkmale zeigte sich eine Sensitivität von 13,3 bis 65,3 %, eine Spezifität von 75,8 bis 100 % und ein PPW von 86,0 bis 100 % in Bezug auf das Vorliegen eines Morbus Crohn. Die Kombination zweier Elemente führte zu einer signifikanten Steigerung der Werte, mit Höchstwerten bei Verknüpfung von zwei der drei Merkmale „Architekturstörung“, „chronisches Entzündungsinfiltrat“ und „neutrophile Granulozyten in LP“. \r\nSchlussfolgerung:\r\nBiopsien aus dem terminalen Ileum sind für die Unterscheidung von CU und MC von enormer Bedeutung. Herausforderndes Verhalten von Menschen mit Demenz zeigt sich in Störungen des Verhaltens, der Stimmung, der Wahrnehmung und des Denkens. Im Management von herausforderndem Verhalten gewinnen nicht-medikamentöse Maßnahmen zunehmend an Bedeutung und werden als wichtige Alternative zu den derzeit zur Verfügung stehenden Medikamenten gesehen. Das Ziel dieser Arbeit ist ein systematisch entwickelter Überblick effektiver nicht-medikamentöser Maßnahmen zur Förderung evidenzbasierter Pflege von Menschen mit Demenz.\r\nMethode: Im Rahmen einer systematischen Übersichtsarbeit wurde eine Literaturrecherche in den Datenbanken PubMed, CINAHL und PsycINFO sowie in den Referenzlisten der gefundenen Literatur und in Google Scholar durchgeführt. In den Datenbanken NICE, SIGN, NHMRC, AHRQ, und AWMF wurde nach Leitlinien gesucht. Die methodologische Qualität der systematischen Übersichtsarbeiten und Leitlinien wurde von zwei Personen unabhängig voneinander mittels validen geeigneten Bewertungsinstrumenten geprüft. Die extrahierten Daten wurden narrativ zusammengefasst.\r\nErgebnisse: Eingeschlossen wurden sieben Übersichtarbeiten und zwei Leitlinien. Musiktherapie, körperliche Aktivitäten, sensorische Interventionen, Massage und Berührung, Reminiszenzverfahren und individuell angepasste Aktivitäten erwiesen sich bei herausforderndem Verhalten oder einzelnen Verhaltensweisen als effektiv. Bei Störungen des Schlafverhaltens zeigten ein Programm aus mehreren Komponenten und eine tagsüber strukturierte soziale Aktivierung Verbesserungen. Lichttherapie, simulierte Präsenztherapie und kognitives Training erwiesen sich als nicht oder unsicher wirksam.\r\nDiskussion: Bestimmte nicht-medikamentöse Maßnahmen sind effektiv, um herausforderndes Verhalten zu verringern.
