The incidence of HCC continues to rise globally, despite preventative efforts being made.\r\nAims: This study aimed to investigate epidemiological changes observed in the aetiology and outcomes of HCC patients at Klinikum Klagenfurt am Wörthersee between 2012 and 2023.\r\nMethods: This was a single-centre cohort observational study. Data was collected retrospectively from the hospitals’ electronic patient database. Two time-periods (2012-2017 and 2018-2023) were created to enable comparison between the two intervals.\r\nResults: More patients were diagnosed with HCC during the second time-period, proving that the incidence of HCC is rising. The incidence rate of HCC remained similar in males and females over time, with males having a 5 times higher risk to develop HCC. The median age of diagnosis was 72.5 years. Patients were on average 2 years younger in the second time-period compared to the first. Alcohol remained the leading underlying cause of HCC and no statistically significant change was seen over time, although a rise in the number of NASH cases was observed. The survival time for HCC patients remained similar, despite changes in management strategies and the availability of new systemic treatments. Nearly half of the patient population did not have a raised AFP at the time of diagnosis.\r\nOverall, an increasing number of HCC patients without liver cirrhosis were identified during recent years. NASH was the most common underlying aetiology in HCC patients without liver cirrhosis.\r\nConclusions: Hepatocellular carcinoma continues to be an important health concern in our society. The number of patients with HCC and no liver cirrhosis is steadily increasing. In patients without liver cirrhosis, NASH due to underlying life-style diseases, played an important aetiological role. Eine Single Center Analyse", "abstract": "Die vorliegende Diplomarbeit mit dem Titel bzw. der Fragestellung, ob Frühgeburtlichkeit steuerbar ist, beschreibt die Hauptmerkmale einer Frühgeburt und eines frühgeborenen Kindes. Im Rahmen dieser Diplomarbeit wird anhand der Daten eines Kalenderjahres des Perinatalzentrums LKH Leoben abgeschätzt, wie hoch der Anteil der „Notfallentbin-dungen“ ist. \n\nDer theoretische Teil dieser Diplomarbeit umfasst das Ergebnis einer Literaturrecherche in ausgewählten Fachbüchern, Journalen wie auch Online-Plattformen und verschafft einen Überblick über Ursachen, Diagnosen und Behandlungsmöglichkeiten bei Frühgeburtlichkeit, sowie über die Merkmale, die typischen Probleme und das Outcome frühgeborener Kinder .\n\nUm der Fragestellung auf den Grund zu gehen, ob Frühgeburtlichkeit „steuerbar“ ist, werden anschließend in Zusammenarbeit mit Herrn. Prim. Univ.-Prof. Dr. Reinhold Kerbl, Frau OA Dr. Anna Trinkl und Herrn Prim. Priv.Doz.Dr. Thomas Aigmüller, Daten von frühgeborenen Kindern und ihren Müttern vom Landeskrankenhaus (LKH) Leoben aus dem Jahre 2016 zusammengefasst und analysiert. Zu diesem Zweck dient eine vorliegende Datenbank, aus der anonymisierte Daten zur präpartaler Anamnese, Entbindung und Neugeborenenversorgung für alle Neugeborenen mit einem Gestationsalter unter 32 + 6 SSW entnommen sind. Dabei zeigt sich, dass etwa ein Drittel aller Geburten „notfallmäßig“ erfolgte und eine Weitertransferierung nicht möglich bzw. vertretbar gewesen wäre.\n\nEine große Bedeutung spielt das pränatale Management, das einen wichtigen Einfluss auf das postnatale Vorgehen hat. Eine pränatale Lungenreifung wird als sinnvoll erachtet, die im LKH Leoben im Jahr 2016 in den meisten Fällen bei Frühgeborenen mit einem Gestationsalter unter 32 + 6 SSW auch tatsächlich stattfand. Für die Verbesserung des kindlichen Outcomes und eine gute Langzeitprognose ist ein in utero-Transfer des Fötus ab 22 (0/7) SSW in ein Perinatalzentrum empfehlenswert. Auf Basis der vorliegenden Daten wird ein Bezug zu nationalen und internationalen Empfehlungen für die Versorgung Frühgeborener hergestellt.
