Sie betrifft typischerweise einen Sulcus oder wenige Sulci der Großhirnhemisphären, aber nicht die basalen Cisternen. Eine häufige Ursache ist die zerebrale Amyloidangiopathie, gefolgt vom reversiblen zerebralen Vasokonstriktionssyndrom. In dieser Diplomarbeit liegt das das Hauptaugenmerk auf der Prognose konvexaler Subarachnoidalblutungen und dem Langzeitrisiko, im Verlauf eine intrazerebrale Blutung zu erleiden. Methoden: Bei dieser Arbeit handelt es sich um eine retrospektive Kohortenstudie. Eingeschlossen wurden PatientInnen mit einer spontanen konvexalen Subarachnoidalblutung, welche zwischen 01.01.2008 und 31.12.2023 stationär an der Universitätsklinik für Neurologie des LKH-Universitätsklinikums Graz behandelt wurden. PatientInnen wurden ausgeschlossen, wenn ein zugrundeliegendes Trauma oder eine begleitende intrazerebrale Blutung vorlag. Die Ätiologie der kSAB wurde mittels Durchsicht der vorliegenden klinischen und bildgebenden Befunde erhoben. Mittels elektronischer Dokumentation im Krankenhausinformationssystem MEDocs wurde der Langzeitverlauf betroffener PatientInnen einschließlich vaskulärer Ereignisse wie kSAB-Rezidive, intrazerebraler Blutungen und Mortalität erfasst, und diese mittels vorliegender zerebraler Bildgebung validiert. Ergebnisse: Wir konnten 64 PatientInnen mit einer spontanen kSAB in diese Arbeit einschließen. Im Durchschnitt waren die PatientInnen 68,5 Jahre alt. 40 davon (62,5%) waren weiblich. Die durchschnittliche stationäre Aufenthaltsdauer betrug 13,3 Tage. Die häufigsten Symptome bei Aufnahme waren Kopfschmerzen, Hemihypästhesien, sowie Seh- und Sprachstörungen (meist im Rahmen von transient fokal-neurologischen Episoden). Die häufigste Ursache von kSAB in der Kohorte war die zerebrale Amyloidangiopathie (CAA, 64,1%), gefolgt vom reversiblen zerebralen Vasokonstriktionssyndrom (9,4%). Im Follow-Up-Zeitraum (medianes Follow-Up 7,2 Jahre, IQR 4,5 Jahre) haben zwölf PatientInnen (18,8%) zumindest eine intrazerebrale Blutung erlitten. Acht dieser zwölf PatientInnen erlitten diese innerhalb des ersten Jahres nach Auftreten der kSAB. Zwei der zwölf PatientInnen sind an den direkten Folgen der intrazerebralen Blutung verstorben. Bei allen zwölf PatientInnen lag eine CAA als Ursache zugrunde. 12,8% aller PatientInnen erlitten im Langzeitverlauf epileptische Anfälle. Diskussion: Konvexale Subarachnoidalblutungen sind im Vergleich zu aneurysmatischen Subarachnoidalblutungen seltener. Sie weisen eine mildere Symptomatik und eine geringere Akutmortalität auf und zeigen grundsätzlich einen guten Outcome. Wenn eine kSAB durch eine CAA verursacht wurde, besteht aber ein hohes Risiko intrazerebrale Blutungen, insbesondere im ersten Jahr nach einer kSAB, zu erleiden.
