Best possible functionality necessitates precise alignment of the patella within the trochlear groove. Patellofemoral stability is supported by ligaments, bony structures and neuromuscular factors. Instability in this system can lead to dislocations and impairments, particularly affecting young, active individuals. The complex etiology of patellofemoral instability complicates the in vivo assessment of confounding factors affecting joint stability. Recent advancements in computational models have facilitated the study of these factors through musculoskeletal simulations. Although the impact of several morphological parameters on the stability of the patellofemoral joint has been explored, substantial gaps remain in understanding the specific influences of altered gait patterns and other morphological variations, such as lower limb torsion. Die Kardinalsymptome sind Rigor, Tremor und Akinese. Pathophysiologisch kommt es zu einem Verlust der dopaminergen Neurone in der Substantia nigra. Dieser führt zu einem absoluten Mangel an Dopamin im Neostriatum. Aus diesem Dopaminmangel resultiert ein Ungleichgewicht zwischen hemmenden und erregenden Neurotransmittern. Schließlich führt dies zu einer pathologisch gesteigerten GABAergen Hemmung des Thalamus, wodurch dieser nun eine verstärkte Filterfunktion für sensorische und motorische Signale aus der Körperperipherie besitzt, was die Parkinson-Symptomatik bedingt. Zur pharmakologischen Therapie stehen L-Dopa, Dopaminagonisten, MAO-B-Hemmer, NMDA-Rezeptor-Antagonisten, Anticholinergika, Decarboxylasehemmer und COMT-Hemmer zur Verfügung. Die medikamentöse Erstlinientherapie richtet sich primär nach dem Alter der Patient*innen. Ältere Patient*innen erhalten als Therapie der ersten Wahl eine Monotherapie mit L-Dopa. Bei jüngeren Patient*innen ist als Therapie der ersten Wahl aufgrund einer geringeren Langzeitproblematik eine Monotherapie mit einem D2 -Rezeptor-Agonisten empfohlen. Bei milder Symptomatik kann alternativ auch eine Monotherapie mit MAO-B-Hemmern erfolgen.
