            "title": "Neue Therapiekonzepte der Sepsis mit speziellem Blick auf die Bedrohung durch multiresistente Keime",
            "abstract": "Zusammenfassung\r\nHintergrund: Die multiresistente Keimentwicklung ist ein aktuelles Problem und wird von\r\nder WHO als eines der größten Risiken für die globale Gesundheit gesehen. Gerade bei\r\nPatientInnen mit einem septischen Krankheitsbild ist der schnelle Einsatz wirksamer\r\nAntibiotika ein essenzieller Faktor, der das Therapieergebnis bestimmt. Die Arbeit befasst\r\nsich neben allgemeinen Aspekten des septischen Krankheitsbilds mit der\r\nResistenzentwicklung im weltweiten und europäischen Raum. Hier werden auch die\r\nhäufigsten Problemkeime und ihre speziellen Resistenzmechanismen vorgestellt. Es\r\nwerden neue antibiotische Therapien vorgestellt und aktuelle Daten zur Wirksamkeit bei\r\nschweren Infektionen herangezogen. Außerdem wird ein Ausblick gegeben, welche\r\nalternativen Therapiestrategien in der Zukunft interessant werden könnten.\r\nMethoden: Bei der Arbeit handelt es sich um eine Literaturrecherche.\r\nFür theoretische Grundlagen wurden Fachbücher, aktuelle Leitlinien sowie Internetportale\r\ngenutzt. Besonders wurden Portale wie PubMed und Google Scholar benutzt, um die\r\nEntwicklung neuer Antibiotika und ihr Potenzial gegen schwere Infektionen zu\r\ndiskutieren.\r\nErgebnisse: Die antibiotische Therapie der Sepsis sollte umgehend erfolgen. Die\r\nErwägungen zur Wahl des passenden Antibiotikums sind komplex, sollten jedoch immer\r\nden Infektionsherd und die lokale Resistenzsituation beinhalten. Die multiresistenten\r\nKeime, die global für die meisten Probleme sorgen, sind Staphylococcus aureus, E.coli,\r\nAcinetobacter baumannii, Pneumokokken, Klebsiella pneumoniae und Pseudomonas\r\naeruginosa. Ihre Verbreitung ist national sowie lokal sehr unterschiedlich. Genauso verhält\r\nes sich mit den Resistenzen, die sie tragen. Neben antibiotischen Substanzen, die neu auf\r\nden Markt kommen, werden auch etablierte Antibiotika mit neuen ß-Lactamasehemmern\r\nkombiniert, um Extended spectrum ß-Lactamase (ESBL) Bakterien erfolgreich zu\r\nbehandeln.\r\nDiskussion: Die Gefahr, die von multiresistenten Bakterien ausgeht, ist enorm. Es konnten\r\ngute Fortschritte gemacht werden, um für das Thema zu sensibilisieren. Jedoch verhindert\r\neine geringe Wirtschaftlichkeit weiterhin die pharmazeutische Entwicklung und neue\r\nTherapeutika zeigen bisher keinen durchschlagenden Erfolg. Ideen für Alternativen zu\r\nklassischen Antibiotika sind vielfältig. Phagentherapien, Antimikrobielle Peptide und der\r\nEinsatz von Impfungen werden alle erforscht, jedoch sind diese Methoden von einem\r\nbreiten klinischen Einsatz noch Jahre entfernt.",

            "title": "Zellbasierte Therapien in der Verbrennungsmedizin mit speziellem Fokus auf den möglichen Einsatz von mesenchymalen Stammzellen",
            "abstract": "Hintergrund: Verbrennungen sind häufig und gehen oft mit einer hohen Morbidität und Mortalität einher. Trotz des Fortschritts in der Verbrennungsbehandlung in den letzten Jahren, wurde noch keine Therapie gefunden, die problemlos bei allen Verbrennungen eingesetzt werden kann. Daher ist es unabdingbar neue Behandlungsansätze in der Therapie von Verbrennungsopfern zu finden. Mesenchymale Stammzellen (mesenchymal stem cells, MSCs) haben das Potential die Wundheilung zu verbessern und auch den Folgen der Verbrennungen wie etwa einer übermäßigen Narbenbildung entgegenzuwirken. Diese Arbeit setzt sich mit mesenchymalen Stammzellen und deren Anwendung bei Verbrennungswunden auseinander.  \r\n\r\nMethoden: Zur Literaturrecherche wurde die Datenbank Pubmed verwendet und nach präklinischen Tierversuchsstudien, die die Verwendung mesenchymaler Stammzellen bei tiefen zweit- und drittgradigen Verbrennungswunden beschreiben, gesucht. Durch die Wahl gezielter Ein- und Ausschlusskriterien wurden 60 Publikationen in die Arbeit aufgenommen und hinsichtlich des Ursprungs der angewandten mesenchymalen Stammzellen in die folgenden Gruppen unterteilt: MSCs aus dem Fettgewebe, aus dem Knochenmark, aus der Nabelschnur und aus der Plazenta. In den Gruppen wurden zusätzlich zu den therapeutischen Potenzialen der MSCs auch die Effektivität deren Vorkonditionierung und deren Kombination mit herkömmlichen Therapien untersucht.\r\n\r\nErgebnisse: Die mesenchymalen Stammzellen aus dem Fettgewebe, dem Knochenmark, der Nabelschnur und der Plazenta haben positiven Einfluss auf die Wundheilungsgeschwindigkeit, die Durchblutung, die Entzündungsreaktion, die Kollagen- und Granulationsgewebsbildung und die Re-Epithelisierung. Darüber hinaus können sie auch zu neuen Hautanhangsgebilden führen. Insbesondere die Vorkonditionierung der MSCs und die Kombination mit herkömmlichen Therapien und diversen Gerüsten führten zu den besten Ergebnissen. \r\n\r\nSchlussfolgerung: Trotz der großen Heterogenität der Studien, erwiesen sich die Anwendungen von MSCs in der Wundheilung von Tieren insgesamt als wirksam. Vor allem MSCs aus dem Fettgewebe gemeinsam mit Gerüsten führten zu vielversprechenden Ergebnissen. Für den routinemäßigen Einsatz am Menschen werden noch mehr (prä)klinische Studien benötigt. Insbesondere einheitliche Studien zur besseren Vergleichbarkeit sind notwendig.",

            "title": "Cloning and expression of a novel GLB1 mutant discovered in GM1 gangliosidosis patient\r\n\r\n\r\n\r\nImpact of GLB1 gene mutation on the GLB1 protein activity",
            "abstract": "Introduction: GM1 gangliosidosis is a lysosomal storage disorder leading to pro-gressive neurological symptoms. It is caused by pathogenic variants in the galac-tosidase beta 1 (GLB1) gene encoding β-galactosidase. This enzyme cleaves the terminal β-galactose from different substrates including gangliosides, glycopro-teins and glycosaminoglycans. Pathogenic variants in the GLB1 gene lead to ab-sent or reduced β-galactosidase activity resulting in an accumulation of the glyco-sphingolipid GM1 ganglioside in neuronal tissue. So far, around 300 pathogenic variants of the GLB1 gene have been described.\r\n\r\nAim: The aim of this diploma thesis was to clone and express a new mutant form of the GLB1 gene that was recently discovered by our group. By measuring its activity in vitro, it was investigated whether this new mutation leads to the expres-sion of dysfunctional protein. \r\n\r\nMethods: A mutant and a wild-type variant of the GLB1 gene were cloned in competent E. coli cells. Recombinant plasmid-DNA was isolated with a maxiprep DNA isolation kit. To check successful DNA isolation, restriction analysis and DNA electrophoresis were performed. For expression of the GLB1 gene variants, HEK-293T cells were transfected with the recombinant plasmid-DNA. GLB1 expression was detected with a western blot analysis. Enzyme activity was measured with a photometric enzyme assay.\r\n\r\nResults: Both, wild-type and mutated enzyme were successfully expressed and the enzyme assay showed a significantly diminished activity of the mutated GLB1 protein in comparison to the wild-type (96.9±26.1 vs. 283.5±44.1 nmol/h/mg, p=0.0033).\r\n\r\nDiscussion: The reduced enzyme activity suggests the novel mutation as a path-ogenic variant of the GLB1 gene, leading to the manifestation of GM1 gangli-osidosis.",
