Die Pathogenese beruht auf einer Überproduktion von Sebum, Entzündungsprozessen, follikulärer Besiedlung vom Cutibacterium acnes sowie auf einer follikulären Hyperkeratose. Klinisch präsentiert sich diese Erkrankung mittels Komedonen, Papeln oder Pusteln. Zirka 85% der Jugendlichen oder jungen Erwachsenen sind von dieser Erkrankung betroffen. Von großer Bedeutung ist es, eine zeitnahe adäquate Therapie einzuleiten, um Aknenarben als mögliche Komplikationen zu verhindern. Dazu zählt auch die österreichische Ernährungspyramide. Der tatsächliche Vollkornkonsum liegt jedoch weltweit deutlich unter den Empfehlungen. Ein adäquater Vollkornverzehr hätte allerdings neben den individuellen gesundheitlichen Vorteilen auch einen nicht unerheblichen gesundheitsökonomischen Nutzen. Vor allem durch den hohen Anteil an Ballaststoffen und Antioxidantien wird ein gesundes Körpergewicht gefördert und das Risiko an Diabetes mellitus Typ II zu erkranken, sowie die damit einhergehende Mortalität gesenkt. Einen Beitrag dazu könnte auch der vermutete positive Einfluss auf das Darmmikrobiom leisten. Darüber hinaus kann mehr Vollkornkonsum im Gegensatz zu raffiniertem Getreide das Risiko für kolorektale Karzinome und Autoimmunerkrankungen, insbesondere Multiple Sklerose, senken. Dabei spielt die antiinflammatorische Wirkung eine wichtige Rolle. Durch den Reichtum an Mikronährstoffen, bioaktiven Komponenten sowie Ballaststoffen ist Vollkorn auch mit einer höheren Knochenmineraldichte assoziiert, weshalb Vollkorn in der Ernährung auch bei Osteoporose eine wichtige Rolle spielt. In unserer Erhebung, die die Verfügbarkeit von Vollkornprodukten in 6 Bäckereien und 5 Supermärkten in Graz untersuchte, waren von zwei Ausreißern abgesehen durchschnittlich weniger als ein Fünftel (15,8%) der angebotenen Brote Vollkornprodukte. Mutationen in den für das 21-Hydroxylase- oder 11-Beta-Hydroxylaseenzym codierenden Genen, legen den Grundstein für eine durchaus komplexe Stoffwechselerkrankung, das adrenogenitale Syndrom. Diese Enzymdefekte beeinträchtigen mitunter die Synthese des lebenswichtigen Hormons Cortisol. Die physiologischerweise über die Hypothalamus-Hypophysen-Nebennierenrinden-Achse ablaufende, auf den Sekretionskreislauf wirkende, negative Feedbackhemmung von Cortisol, ist demnach unzureichend oder fehlt gänzlich. Infolgedessen wird die Produktion der Androgene in der Zona glomerulosa der Nebennierenrinde immer weiter angeregt und resultiert letztlich in einer vielschichtig symptomatischen Hyperandrogenisierung. Grundsätzlich können zwei verschiedene Ausprägungsformen unterschieden werden. Die klassische, die bedingt durch einen kompletten Enzymfunktionsverlust mit einem weitreichenden Symptomenkomplex verbunden ist und im Rahmen des Neugeborenenscreening beinahe immer direkt postnatal diagnostiziert wird. Bei dieser Form kann es auch zu einem Mangel am Mineralkortikoid Aldosteron kommen. Ein rasches Engraftment und eine effektive Immunrekonstitution sind zwei der wichtigsten Schlüsselfaktoren, um eine niedrige transplantationsbedingte Morbidität und Mortalität (TRM) zu erreichen. Der ideale Verlauf einer HSZT zeichnet sich durch ein stabiles Engraftment, einem Gleichgewicht zwischen effektiven Graft-versus-Leukämie (GVL) / Graft-versus-Tumor (GVT) -Effekt, niedrigen GVHD-Raten und der Erholung der Immunabwehr durch ein quantitativ und qualitativ funktionsfähiges Immunsystem aus. Für den Erfolg der HSZT spielen unterschiedliche Faktoren eine Rolle. Um die Einflüsse auf die Immunrekonstitution vollständig zu erfassen, wurden Aspekte wie die Wahl der Spender*innen, die Stammzellquelle, die Konditionierungstherapie, auftretende Graft- versus-Host Disease (GVHD) und Infektionen in die Analyse integriert.
