Gastrointestinale (GI) Symptome gehören zu den vielfältigen Symptomen die durch eine HIT auftreten können. Die Betrachtung einer HIT mit Anwendung einer histaminfreien Diät als Therapie bei GI Symptomen, ist jedoch in der Literatur bisher kaum vorhanden.\nZielsetzung: Führt eine histaminfreie Diät bei Patientinnen/Patienten mit GI Symptomen und verminderter DAO Aktivität, zu einem Anstieg der Enzymwerte bei gleichzeitigem klinischem Ansprechen; besteht dabei eine Abhängigkeit zur Genauigkeit der Diäteinhaltung.\nMaterial und Methoden: Retrospektive Erhebung im Zeitraum eines Jahres. Patientinnen/Patienten einer internistischen Praxis mit der Diagnose einer HIT, vermindertem DAO Ausgangswert (unter 10U/mlsc) und mindestens einer zweiten DAO Messung im zeitlichen Verlauf wurden eingeschlossen. Die Diäteinhaltung wurde durch einen Fragebogen und die Symptome im Rahmen des ärztlichen Gespräches evaluiert. \nErgebnisse: 101 Patientinnen und Patienten (66% weiblich). 79% der Patientinnen/Patienten zeigten eine Symptomfreiheit bzw. verbesserung. Ein klinisches Ansprechen auf die Diät sowie eine genaue/gelegentliche Diäteinhaltung gehen mit einem signifikanten DAO Anstieg einher. Der Unterschied zwischen den einzelnen Gruppen erreichte keine Signifikanz. \nSchlussfolgerung: Eine histaminfreie Diät führt bei Patientinnen/Patienten mit GI Symptomen und verringerter DAO zu einer Verbesserung ihrer Beschwerden, bei gleichzeitigem Anstieg der DAO. Da der pathophysiologische Mechanismus einer HIT im Sinne eines DAO Mangels in Studien nicht bestätigt werden konnte, bleibt die klinische Relevanz des statisch signifikanten DAO Anstieges unklar. Ziel dieser Diplomarbeit war es, die gesundheitsbezogene Lebensqualität, und damit verbundene Faktoren (soziale Unterstützung, Kohärenzsinn und Depressivität), bei Frauen mit vorgeburtlichen Eingriffen wegen komplizierter monochorialer Zwillingsschwangerschaften, zu untersuchen.\n\nMaterial und Methoden: In einer Fragebogenanalyse wurden die Daten von Studienteilnehmerinnen, die im Zeitraum zwischen Februar 2011 und Juli 2015 an der Abteilung für Geburtshilfe der Medizinischen Universität Graz betreut wurden und bei denen die Geburt der Kinder mindestens sechs Monate zurücklag, analysiert. Es wurden vier Vergleichsgruppen gebildet, wobei Frauen mit intrauterinen Eingriffen und 1.) einem, beziehungsweise 2.) zwei überlebenden Zwillingen und Frauen ohne Eingriffe bei unkomplizierten 3.) monochorialen und 4.) dichorialen Zwillingsschwangerschaften, verglichen wurden. Als Fragebögen wurden die deutschen Versionen des Short Form-12 (SF-12), des Patient Health Questionnaire (PHQ-9), des Sense of Coherence Scale (SOC-13), des Multidimensional Scale of Perceived Social Support (MSPSS), sowie selbst entwickelte Fragen zur Befindlichkeit verwendet. \n\nErgebnisse: Die Daten von 73 Studienteilnehmerinnen konnten ausgewertet werden. Darunter fanden sich 18 der Gruppe 1, 12 der Gruppe 2, 20 der Gruppe 3, 23 der Gruppe 4. Es zeigten sich keine signifikanten Unterschiede hinsichtlich der physischen und psychischen gesundheitsbezogenen Lebensqualität. Die gesundheitsbezogene Lebensqualität war jedoch im Vergleich zur Durchschnittsbevölkerung für alle Gruppen vermindert. Depressive Symptomatik war bei Studienteilnehmerinnen nach unkomplizierten dichorialen Zwillingsschwangerschaften signifikant häufiger ausgeprägt, als bei jenen mit vorgeburtlichem Eingriff. Studienteilnehmerinnen mit vorgeburtlichem Eingriff und zwei überlebenden Zwillingen hatten ein signifikant höheres Kohärenzgefühl und erfuhren signifikant mehr soziale Unterstützung als die anderen Gruppen. Studienteilnehmerinnen mit vorgeburtlichem Eingriff erlebten hingegen signifikant mehr Angst und Stress während der Schwangerschaft als diejenigen mit unkomplizierten Zwillingsschwangerschaften, jedoch auch signifikant mehr Mitbestimmung.\n\nDiskussion: Die Hypothese, dass Frauen mit komplizierten Zwillingsschwangerschaften und vorgeburtlichem Eingriff eine verminderte gesundheitsbezogene Lebensqualität gegenüber jenen mit unkomplizierten Zwillingsschwangerschaften haben, konnte in dieser Studie nicht bestätigt werden.
