Zwischen 2005 und 2021 empfahl der Europäische Rat für Wiederbelebung (ERC) nach Atemwegssicherung eine standardisierte Beatmungsfrequenz von 10 min-1 und warnte explizit vor Hyperventilation. Diese Empfehlung stützte sich maßgeblich auf die tierexperimentellen Ergebnisse von Aufderheide et al. aus dem Jahr 2004. Bei der Übertragung von klinisch beobachteten Tachyventilationen in ein Tierexperiment wurden deletäre ROSC-Raten (Return of Spontaneous Circulation) festgestellt. Das Auftreten des untersuchten exzessiven Atemminutenvolumens von etwa 80 Litern pro Minute erscheint in der präklinischen Behandlungspraxis allerdings unplausibel. Das reguläre Auftreten metabolischer und respiratorischer Azidosen sowie rezente Beobachtungen deutlich niedrigerer Beatmungsfrequenzen bei der Behandlung des OHCA stellen das Auftreten von Hyperventilation infrage. Damit solche Systeme zuverlässig funktionieren, müssen sie medizinische Inhalte korrekt erkennen und interpretieren. Die Grundlage hierfür bildet die sogenannte Annotation – die gezielte Kennzeichnung und Strukturierung medizinischer Daten.. Diese Arbeit bietet eine umfassende Literaturübersicht über Klassifikationen, Terminologien und Ontologien, die in der medizinischen Annotation verwendet werden, und analysiert deren Einfluss auf maschinelle Lernanwendungen. Besonders hervorgehoben wird die zentrale Rolle qualitativ hochwertiger, konsistenter Annotationen, da diese maßgeblich zur Leistungsfähigkeit und Sicherheit von KI-Modellen in der klinischen Praxis beitragen. Die Arbeit untersucht bestehende medizinische Klassifikationssysteme wie Internationale statistische Klassifikation der Krankheiten und verwandter Gesundheitsprobleme (ICD), Systematisierte Nomenklatur der Medizin – Klinische Termini (SNOMED-CT), Medical Subject Headings (MeSH) und andere und bewertet deren Rolle bei der Strukturierung medizinischer Daten für KI-gestützte Anwendungen. Darüber hinaus werden die Bedeutung von Natural Language Processing (NLP) sowie der MATTER-Annotationszyklus im Annotationsprozess erläutert. Neben praxisbezogenen Anwendungen von NLP und KI in der Medizin – darunter klinische Dokumentation, Bildverarbeitung, Patienteneinbindung und diagnostische Unterstützung – werden auch ethische und regulatorische Herausforderungen analysiert. Zahlreiche Mechanismen dieses als diabetische Herzerkrankung bezeichneten Phänomens wurden postuliert, von denen man annimmt, dass sie zu strukturellen Veränderungen im Herz führen. In dieser Arbeit wurde linksventrikuläres Myokard von drei Patient*innengruppen hinsichtlich der Merkmale Kardiomyozyten-Hypertrophie und myokardiale Fibrose untersucht. Eine Gruppe mit Diabetes mellitus Typ 2 und normaler Ejektionsfraktion (Db-pEF) soll einen milden Phänotyp der diabetischen Herzerkrankung repräsentieren und wurde mit einer Kontrollgruppe ohne Diabetes mellitus (Non-Db-pEF) und einer Gruppe mit Diabetes mellitus Typ 2, ischämischer Kardiomyopathie und reduzierter Ejektionsfraktion (Db-ICM) verglichen. Die mittlere Kardiomyozyten-Querschnittsfläche betrug 620 ± 141 µm² in der Gruppe Non-Db-pEF, 602 ± 88 µm² in der Gruppe Db-pEF und 730 ± 195 µm² in der Gruppe Db-ICM, wobei kein signifikanter Unterschied zwischen den Gruppen nachgewiesen werden konnte (ANOVA, F(2, 16) = 1,342, p = 0,289). Der Bindegewebsanteil war 0,67 ± 0,45 % in der Gruppe Non-Db-pEF, 1,83 ± 0,86 % in der Gruppe Db-pEF und 11,42 ± 12,92 % in der Gruppe Db-ICM. Es konnte ein signifikanter Unterschied zwischen den Gruppen detektiert werden (Welch-ANOVA, F(2, 7,899) = 5,845, p = 0,028, η² = 0,340). Die Gruppe Db-pEF zeigte signifikante Fibrose gegenüber der Gruppe Non-Db-pEF (Games-Howell-Post-hoc-Test, p = 0,048, +1,15, 95 %-CI [0,01, 2,30]).
