Die antithrombozytäre Therapie stellt einen wichtigen Baustein in der Sekundärprävention dar. Dafür kommen vor allem Aspirin 100 mg 1x/Tag und/oder Clopidogrel 75 mg 1x/Tag zur Anwendung. Auf Basis der COMPASS-Studie von 2017 wird als intensivierte Form einer antithrombozytären und gerinnungshemmenden Therapie die Kombination aus Aspirin 100 mg 1x/Tag und Rivaroxaban 2,5 mg 2x/Tag in Erwägung gezogen. Die COMPASS-Studie zeigte eine deutliche Senkung des kardiovaskulären Risikos unter der Kombinationstherapie im Vergleich zu Aspirin 100 mg 1x/Tag allein, vor allem bei pAVK-Patient*innen. Aufgrund des vermeintlich erhöhten Blutungsrisikos unter der Kombinationstherapie wird diese aber weiterhin eher zurückhaltend eingesetzt. Wir denken, dass deutlich mehr Patienten für die Kombinationstherapie in Frage kommen als angenommen.\r\n\r\nZielsetzung\r\nZiel dieser Arbeit war es herauszufinden, wie groß der Anteil der Patient*innen mit pAVK im Stadium I bis IIb ist, die für eine Therapie nach dem COMPASS-Schema in Frage kommen.\r\n\r\nMethoden\r\nEs erfolgte eine, im Zeitraum vom 01.07.2020 bis zum 31.07.2021 fortlaufende, Erhebung von Patient*innen in der Angiologischen Ambulanz der Universitätsklinik für Innere Medizin am LKH-Universitätsklinikum Graz. Es wurden dabei 200 Patient*innen mit einer pAVK im Stadium I bis IIb rekrutiert. Mithilfe eines Fragebogens wurden soziodemographische Faktoren, Vorerkrankungen und bisherige kardiovaskuläre Ereignisse, Risikofaktoren für eine pAVK, Allergien und die Medikamenteneinnahme erhoben. Augenmerk wurde vor allem auch auf die Erhebung des Blutungsrisikos gelegt. Diesbezüglich erfolgte eine detaillierte Erhebung von Blutungskomplikationen in der Vergangenheit. Zudem erfolgte auch mittels Laboranalysen eine Beurteilung von etwaigen Veränderungen des Blutbildes, der Leberwerte und der Nierenwerte. Die erhobenen Parameter ermöglichten die Berechnung des HASBLED-Scores, eines validierten Maßes für das Blutungsrisiko. Der primäre Endpunkt war definiert als das Verhältnis von Patient*innen mit pAVK, welche für die Kombinationstherapie nach dem COMPASS-Schema in Frage kommen. \r\n \r\nErgebnisse\r\nVon 200 untersuchten Patient*innen mussten 35 exkludiert werden, da sie aufgrund von Antikoagulation (15,5 %; n = 31) oder Hypersensitivität gegenüber Aspirin oder Rivaroxaban (2,0 %; n = 4) nicht für das Schema geeignet waren.\r\nUnter den restlichen 165 potentiell therapierbaren Patient*innen waren 8 (4,8 % von 165) schon mit dem COMPASS-Schema therapiert. 3 Patient*innen hatten eine Niereninsuffizienz mit GFR < 15 ml/min (1,8 % von 165) und 20 Patient*innen hatten ein erhöhtes Blutungsrisiko (12,1 % von 165).\r\nÜbrig bleiben also 134 (81,2 % von 165) potentiell therapierbare Patient*innen. Diese unterteilen sich weiter in 49 (29,7 % von 165) welche akut therapierbar wären. Zu dieser Gruppe zählen wir auch die 4,8 % welche schon therapiert werden, was in Summe 57 bzw. 34,5 % ergibt. Die restlichen 85 wurden im Moment des Studieneinschlusses mit einer dualen antithrombozytären Therapie behandelt (n = 65; 39,4 % von 165) oder einem anderen Thrombozytenaggregationshemmer als Aspirin (n = 20; 12,1 % von 165) und müssten erst umgestellt werden, kommen aber theoretisch in Frage. \r\n\r\nSchlussfolgerung\r\nDie untersuchte Stichprobe bezog sich erstmals primär auf Patient*innen mit pAVK. Dabei konnte gezeigt werden, dass ein erheblicher Anteil dieser Patient*innen für eine Therapie nach dem COMPASS in Frage käme. Über 80 HSP-Genorte wurden bisher identifiziert. Die Diagnose basiert auf neurologischen und genetischen Untersuchungen. Die Therapie ist symptomatisch, jedoch sind spezifische Therapien Gegenstand aktueller Forschungen. Während der COVID19-Pandemie wurden, bevor es vorgegebene Register gab, von Mitarbeiter*innen der KAGes, mit SARS-CoV-2 infizierte Patient*innen in einer Excel- Liste inkl. Zusatzdiagnosen erfasst. These fractures are notoriously difficult to diagnose accurately due to the complex anatomy of the pediatric elbow and the evolving nature of bone development in children. Misdiagnosis or delayed diagnosis of such fractures can lead to devastating complications, including long-term functional impairment or permanent disability in the affected child. Currently, the standard diagnostic tool for these injuries remains radiography, which, while widely used, suffers from inherent limitations in sensitivity. Human factors, such as diagnostic inexperience in junior clinicians or fatigue affecting even seasoned professionals, can further complicate accurate assessment. Artificial intelligence (AI) has already demonstrated the potential to assist in medical imaging by accurately identifying and localizing pathologies in various radiological studies.
