Unter anderem, je kleiner das T-Stadium zu allen Untersuchungszeiten (prätherapeutisch mit p=0,002, präoperativ mit 0,001 wie auch pathologisch mit p=0,016) war, ein jeweils besseres OS vorgefunden wurde. Ähnliches Ergebnis zeigte das T-Stadium in Bezug auf das DFS. Bei der Anzahl der neoadjuvanten Zyklen verdeutlichte die 5-J-OS von 88,8% bei 6 Zyklen den Vorteil zu 1-5 Zyklen (5-J-OS: 69,1%) wie auch zu 7-14 Zyklen (5-J-OS: 66,7%). Weiters gab es bei brusterhaltender Therapie ein statistisch signifikant besseres OS p=0,027 als bei einer Mastektomie. Die Grundlagen der Entstehung finden sich dabei in genetischen Veränderungen von bestimmten Genen, welche in der klassischen Weg vom gutartigen Adenom zum bösartigen Karzinom führt. Dabei spielt in dieser Adenom-Karzinom-Sequenz des kolorektalen Karzinoms das K-RAS Onkogen eine wichtige Rolle. In den letzten Jahren hat der K-RAS Mutationsstatus immer mehr an Bedeutung als prädiktiver Marker für die Therapie bei kolorektalem Karzinom gewonnen. Hinsichtlich des Stellenwertes auf die Prognoseeinschätzung bei Patienten mit kolorektalem Karzinom konnte allerdings, vor allem bei kleineren Studien, noch kein einheitliches Ergebnis erzielt werden.\r\nMETHODEN: In die vorliegende retrospektive Studie haben wir 643 Patienten, welche am LKH Graz, LKH Fürstenfeld und LKH Leoben behandelt wurden, eingeschlossen. Vorrausetzung waren die Möglichkeit der Datenerhebung über das Krankenhausinformationssystem MEDOCS betreffend den K-RAS Status, sowie Angaben zu folgenden klinische Parametern: Geschlecht, Behandlungszentrum, Tumorlokalisation, Grading und TNM Stadium. Ziel der Studie war es, mittels statistischer Analysen, Unterschiede hinsichtlich des krebsspezifischen Überlebens bei Patienten mit und ohne K-RAS Mutation und Assoziationen von klinischen Parametern zu untersuchen.\r\nERGEBNISSE: Bezüglich der Assoziationen zwischen K-RAS Status und klinischen Parametern wurden für die Gruppe der Tumorlokalisation signifikante Unterschiede nachgewiesen (p<0.05). K-RAS Wildtyp zeigte sich gehäuft bei Sigmakarzinomen, wohin gehend im Caecum ein niedriger Anteil an K-RAS Wildtyp anzutreffen war. Im Caecum zeigte sich eine Häufung an Codon 13 Mutationen (p<0.05). Mittels Kaplan-Meier-Kurven und Log Rank Test wurde die Hypothese, dass der K-RAS Status Einfluss auf das krebsspezifische Überleben hat, abgelehnt. Signifikante Unterschiede im Überleben zeigten sich jedoch für verschiedene Tumorlokalisationen und erwartungsgemäß auch bei den bekannten Prognosefaktoren Grading, sowie T- und M-Stadium. Die abschließende Prüfung der Variablen auf deren unabhängigen prognostischen Stellenwertes mittels multivariater Cox Analyse bestätigte dies für Grading, T- und M-Stadium (p<0.05).\r\nSCHLUSSFOLGERUNG: Häufigeres Auftreten von K-RAS Mutationen im proximalen Kolon bestätigt die Ergebnisse von anderen kleineren Studien wieder. The purpose of this doctoral thesis was to determine the impact of intensity and volume golf play on health and performance related variables, based on two observation periods (short-term, 8 to 12-weeks; long-term, 30-weeks). To determine baseline results, a heterogeneous group of 66 healthy subjects, 42 men and 24 women, varying in age and handicap (age, 53.3 ± 13.0 years; HCP, 23.5 ± 11.3), completed a sports medical examination. During the game, the heart rate (HR), duration, distance, walking speed, ascent and descent were measured via a GPS/HR monitor. When playing 9 or 18-holes of golf, independent of the golf course design, the average HR was not significantly different between men vs. women or the subgroups with mild exertion level of (<3 METs), but with a significant increase for the elderly and untrained test subjects. The total energy expenditure in men was almost twice as high as in women. In both longitudinal observation periods, the subjects significantly decreased body mass, subcutaneous fat layer thickness, and resting blood pressure. Furthermore, the participants improved exercise performance variables (e.g. VO2max and (sub)-maximal power output) while they lowered the HR. The variables of the blood profile remained stable. A minimum threshold ranging from 200 to 420 minutes of golf per week was found to be significant to provide health and fitness benefits. Muscoskeletal effects of inadequate vitamin D status have long been investigated including its impact on osteoporotic fractures. In the last several years vitamin D got popular for a series of benefical consequences. Many associations of low serum 25 hydroxyvitamin D (25-OH-vitamin D) status and extraskeletal effects such as cardiovascular disease, cancer and mortality have been observed. It remains unclear whether vitamin D is the cause of these clinical patterns or the consequence of an impaired health status.\r\nBased on recently reported genetic determinants of hypovitaminosis D I aimed to investigate (1st) the impact of three single nucleotide polymorphisms (SNPs) on Vitamin D and vitamin D related outcomes in the fields of bone, prostate cancer and mortality studies. Using these SNPs as a tool for Mendelian randomization, I wanted to elucidate (2nd) the question if low vitamin D levels are causal for higher mortality rates.\r\nMethods: Genotypes of SNPs in the group-specific component gene (GC, rs2282679), 7-dehydrocholesterol reductase gene (DHCR7, rs12785878) and cytochrome P450 IIR-1 gene (CYP2R1, rs10741657) were determined in different cohorts and investigated for 25-OH-vitamin D and vitamin D related outcomes.\r\nResults and conclusions: The GC polymorphism was the strongest genetic determinant of 25-OH-vitamin D status and was confirmed in all settings. CYP2R1 and the DHCR7 variants could be approved in the largest study with the highest power to detect statistical differences.\r\nBone: None of the genotypes was linked to bone mineral density (BMD) or past fractures in a BMD screening study (n=342) and a prospective cohort study with 1093 nursing home residents. Carriers of the G-allele of the DHCR7 polymorphism showed an increased risk for prospective fractures.\r\nProstate cancer: GC genotypes were not associated with biomarker based recurrence, development of metastases or overall survival in 702 prostate cancer patients. The investigated polymorphisms do not play a major role for the investigated bone and prostate cancer related endpoints.\r\nMortality: GC, CYP2R1 and DHCR7 genotypes predicted 25-OH-vitamin D status reliably but not all-cause-, cardiovascular-, or non-cardiovascular mortality. Diese Studie wurde durchgeführt um die Prävalenz und genetischen Eigenschaften von resistenten Bakterien in gemischtem Faschiertem vom Schwein und Rind zu untersuchen. Besonderes Augenmerk wurde dabei auf ESBL-Produzenten (Escherichia coli und Klebsiella spp) und MRSA (Methicillin-resistenter Staphylococcus aureus) gelegt.\r\nMethode: Im Zeitraum von September 2011 bis September 2012 wurden 100 Proben gemischtes Faschiertes vom Rind und vom Schwein aus Supermärkten (n=70) und Fleischereien (n=30) gesammelt. Nach Anreicherung in einem Flüssigmedium (Peptonwasser) und Beimpfung auf feste Selektivmedien wurden Bakterien von chromeIDTM ESBL (bioMérieux) und OXA Agar selektiert und nachfolgend mit MALDI-TOF MS AximaTM Assurance (Shimadzu, Japan) und VITEK®2 System (bioMérieux) identifiziert. Für alle isolierten Bakterien wurden Resistenztestungen mittels Agardiffusion durchgeführt. Die ESBL-Produktion wurde phänotypisch mit dem ¿double disk diffusion¿-Test nach CLSI Richtlinien bestätigt. Die Resistenzgene von den ESBL-Produzenten und die spa Typisierung von MRSA wurden mit Hilfe von PCR und Sequenzierung bestimmt.\r\nErgebnisse: Insgesamt wurden 277 Stämme vom ESBL Agar isoliert und auf Antibiotikaresistenzen getestet. Dabei fand man in 20 von 100 Fleischproben ESBL-produzierende Escherichia coli. Der häufigste ESBL Typ bei Escherichia coli war CTX-M-1, es wurden aber auch CTX-M-14, CTX-M-32; SHV-12 und TEM-52 gefunden. Außerdem wurde in 10 Proben MRSA gefunden und mittels spa Typisierung bestätigt. Gefundene spa Typen waren LA (livestock-associated) t011, t034 und t2241, aber auch CA (community-acquired) t3928.\r\nDiskussion: Eine Kontamination von Rinder- und Schweinefleisch mit multiresistenten Bakterien konnte in dieser Studie bestätigt werden. Es konnte gezeigt werden, dass sowohl ESBL-bildende Keime, welche u.a. für Harnwegs- und Wundinfektionen verantwortlich sind, aber auch MRSA, ein Auslöser von Septikämien, Pneumonien, sowie Haut- und Weichteilinfektionen, in den Fleischproben präsent war. A number of different molecular pathways have been proposed to mediate the process of cellular dysfunction and death in different tissues which mainly focus on apoptosis as a major cell death pathway. Recent developments have provided strong evidence on the contribution of necrosis as a programmed cell death pathway (necroptosis) which was otherwise considered as a random or accidental phenomenon. The exact molecular pathways of PA-induced cell death are still mysterious and one or a combination of different signaling cascades may be involved. Here, we show that PA triggers autophagy and lysosomal dysfunction simultaneously leading to endothelial cell death. The PA-induced cell death was predominantly necrotic as indicated by annexin V and propidium iodide (PI) staining, absence of caspase activity, low levels of DNA hypoploidy, and an early ATP depletion. In addition PA induced a strong elevation of mRNA levels of ubiquitin carboxyl-terminal hydrolase (CYLD), a known mediator of necroptosis. Moreover, siRNA-mediated knockdown of CYLD significantly antagonized PA-induced necrosis of endothelial cells. In contrast, inhibition and knockdown of receptor interacting protein kinase 1 (RIPK1) had no effect on PA-induced necrosis, indicating the induction of a CYLD-dependent but RIPK1-independent cell death pathway. PA was recognized as a strong and early inducer of autophagy. The inhibition of autophagy by both pharmacological inhibitors and genetic knockdown of the autophagy-specific genes, vacuolar protein sorting 34 (VPS34), and autophagy-related protein 7 (ATG7), could rescue the PA-induced death of endothelial cells. Moreover, the initiation of autophagy and cell death by PA was reduced in endothelial cells loaded with the Ca(2+) chelator 1,2-bis(o-aminophenoxy)ethane-N,N,N',N'-tetraacetic acid-(acetoxymethyl) ester (BAPTA-AM), indicating that Ca(2+) triggered the fatal signaling of PA. At the same time PA was found to activate calpain 1 and induces lysosomal dysfunction. A siRNA-mediated knock down of calpain 1 inhibited the PA-induced cell death significantly. Ihr Anteil der intrakraniellen Gefäßmalformationen beträgt 10-15%. Ursprünglich noch als kongenitale benigne Läsionen eingestuft, wurden in den frühen 70er Jahren nachgewiesen, dass DAVFs intrakranielle Blutungen verursachen können. Von aggressiven DAVFs mit einer retrograden Drainage in leptomeningeale Venen, geht ein hohes Risiko für Blutungen, Ischämie oder intrakranieller Hypertension aus. Bleiben diese unbehandelt, besteht eine jährliche Mortalitätsrate von ca. 10%.\r\nZiel: Die Evaluierung des technischen und klinischen Erfolges der endovaskulären Behandlung duraler Av-Fisteln unter Berücksichtigung der verwendeten Technik, des Materials, des Fisteltyps und der Lokalisation. \r\nMethode: Bei der Studie handelt es sich um eine monozentrische retrospektive Datenanalyse und Vergleichsstudie. Es wurden die Daten vom Patienten im Zeitraum zwischen 01.06.2005-09.12.2011 an der Klinischen Abteilung für Neuroradiologie Graz ausgewertet. Von 222 Interventionen wurden an 34 Patienten, welche in die Studie aufgenommen wurden, 68 Interventionen durchgeführt. Die Einschlusskriterien waren definiert durch die Lokalisation und Art der Gefäßmalformation.\r\nErgebnisse: Was den technischen Erfolg betrifft, so konnten von insgesamt 4 Sinus cavernosus Fisteln (SCFs) 3 (20% aller Erfolge) vollständig verschlossen werden, sowie 1 Teilerfolg (5,3% aller Teilerfolge) erzielt werden. Bei den 4 DAVFs konnte die gleiche Aufteilung beobachtet werden. Die 10 duralen arteriovenösen Malformationen (AVMs) zeigten 3 (20%) Erfolge, sowie 7 (36,8%) Teilerfolge. Unter den gemischten AVMs waren sowohl 5 (33,3%) Erfolge, wie auch 9 (47,4%) Teilerfolge zu vermerken. Bei der Kombination aus SCF und duraler AVM konnte ein Erfolg (6,7%) und bei der Kombination aus SCF und DAVF ein Teilerfolg erzielt werden. Es gab keinen Patienten an dem nicht zumindest ein partieller Erfolg gelungen ist. Bis auf das Auftreten von Nachblutungen in zwei Fällen und einer postinterventionellen Hemianopsie, kam es zu keinen schweren Komplikationen. Klinisch bewirkten die Eingriffe vorwiegend eine Besserung der neurologischen und ophthalmologischen Symptome.
