Es umfasst eine Anzahl unterschiedlicher Pathologien, denen die Optikusneuropathie gemein ist. Der Augeninnendruck gilt als einziger modifizierbarer Faktor gegen das Fortschreiten der Erkrankung. Das Glaukom kann primär oder sekundär verursacht sein. Ab 21 mmHg oder höher spricht man von einem erhöhten Augeninnendruck, wobei es unterschiedliche Methoden gibt diesen zu messen. Da es beim Glaukom zu einer Optikusneuropathie kommt ist wichtig zu entscheiden, ob das Verhältnis einer Exkavation zur Papillengröße sich innerhalb eines gewissen Rahmens befindet. Als wichtiger Bestandteil der Glaukomdiagnostik gilt die Untersuchung des Gesichtsfeldes (Perimetrie).\r\nVerschiedene Medikamente können den Augendruck senken. Diese werden meistens in Tropfenform appliziert und haben unterschiedliche Angriffspunkte. Wenn die medikamentöse Therapie nicht ausreichend ist, können operative Therapien eingesetzt werden. Auch hier gibt es verschiedene Operationsmethoden, die alle gemeinsam haben, dass sie den Druck in der Vorderkammer senken.\r\nIn dieser Diplomarbeit werden zwei Operationsmethoden miteinander verglichen. Eine davon ist die herkömmliche Methode der Trabekulektomie bei der das trabekuläre Maschenwerk eröffnet und das Kammerwasser unter die Bindehaut abgeleitet wird. Die andere Methode namens Preserflo gehört zu den MIGS (minimal invasive glaucoma surgery) und stellt eine weniger invasive Methode dar. Um die Methoden miteinander vergleichen zu können, wurden von beiden über sechs Monate hinweg Daten bei Routineuntersuchungen gesammelt, die hier präsentiert und ausgewertet werden. Folgende Ergebnisse haben sich eingestellt: Präoperativ wurde bei der Trabekulektomie ein intraokulärer Druck (IOD) von 29,5 +- 8,8 mmHg gemessen, bei Preserflo ein IOD von 25,4 +- 8,4 mmHg, ein signifikanter Unterschied (p = 0,001) von Anfang an, der einen Bias in der Vorauswahl darstellt. Sechs Monate nach den Operationen befanden sich die Werte bei 16.1 +- 7.2 mmHg und 15.6 +- 5 mmHg. Die Differenz zum Ausgangswert ist jeweils signifikant (p < 0,001), die Differenz der Methoden untereinander nicht mehr.\r\nDie mittlere Medikamentenanzahl betrug präoperativ 2,6 +- 1,1 und nach sechs Monaten 0,8 +- 1,3 bei der Trabekulektomie. Beim Preserflo betrug sie 2,1 +- 1,2 beziehungsweise 0,5 +- 1 nach sechs Monaten. Folglich bestand in der Medikamentenzahl zwischen den Methoden anfangs eine signifikante Differenz (p = 0,005), die sich aufgelöst hat nach sechs Monaten. Die Differenz innerhalb der jeweiligen Methode nach sechs Monaten ist jedoch signifikant (p < 0,001).\r\nEin vollständiger chirurgischer Erfolg wurde festgelegt als postoperativer IOD <= 18 mmHg nach 6 Monaten. Erreicht haben diesen 56,9% der Trabekulektomie- und 64,5% der Perserflo-Gruppe. Trabekulektomie unterschied sich dabei nicht signifikant vom Preserflo (p > 0,05).\r\nDiese Arbeit bestätigt, dass beide Operationsmethoden den Druck meistens suffizient senken können. Obwohl ein Bias der Vorauswahl in der Fortgeschrittenheit der Erkrankungen bestand, kam es bei beiden Operationsmethoden zu ähnlichen Druckwerten nach sechs Monaten. We discovered that hexokinase 1 (HK1), a key glycolytic enzyme, clusters into ring-like structures around mitochondria during energy stress. These HK1-rings constrict mitochondria at contact sites with the endoplasmic reticulum (ER) and prevent mitochondrial fission by displacing the dynamin-related protein 1 (Drp1) from mitochondrial constriction sites. Mechanistically, we identified that the lack of ATP and glucose-6-phosphate (G6P) promotes the clustering of HK1. Moreover, we found critical mutations that are critical for the formation of HK1-rings. Utilizing these mutations, we could show that HK1-rings keep mitochondria connected and rewire cellular metabolism during energy stress. Our findings highlight that HK1 is a robust energy stress sensor that regulates the shape, connectivity and metabolic activity of mitochondria. Thus, the formation of HK1-rings may affect mitochondrial function in energy stress-related pathologies. HK1 has been shown to interact with the voltage-dependent anion channel 1 (VDAC1), a crucial gatekeeper in the outer mitochondrial membrane that controls metabolic and energy homeostasis. The available methodological approaches fell short of accurate visualization of VDAC1 in living cells. To permit precise VDAC1 imaging, we utilized the tetracysteine (TC)-tag and visualized VDAC1 dynamics in living cells. TC-tagged VDAC1 had a cluster-like distribution on mitochondria. The majority of VDAC1-clusters were localized at ER-mitochondria contact sites. Notably, we did not observe apparent colocalization between VDAC1 and HK1 during glucose depletion. However, VDAC1 colocalized with BCL-2 Antagonist/Killer (BAK)-clusters upon apoptotic stimulation. Additionally, VDAC1 was found at mitochondrial fission sites. Die steigende Prävalenz dieser Erkrankung erhöht den Bedarf an evidenzbasierten Leitlinien zur Unterstützung von Gesundheitsberufen bei der Betreuung von Personen mit Adipositas. Die bestehenden Leitlinien zu Ernährungs- und Bewegungsinterventionen für erwachsene Personen mit Adipositas sind teils widersprüchlich und von variabler Qualität. Ziel: Das Hauptziel dieser Arbeit ist es, einen Überblick über Handlungsempfehlungen für Ernährungs- und Bewegungsinterventionen bei erwachsenen Personen mit Adipositas auf der Grundlage aktueller internationaler Leitlinien zu geben. Diese Empfehlungen sollen Pflegepersonen und andere Gesundheitsberufe dabei unterstützen, eine evidenzbasierte Praxis umzusetzen und die Versorgung von Personen mit Adipositas zu verbessern. Methoden: Es wurde eine strategische Literaturrecherche in Datenbanken wie PubMed und Embase sowie auf den Websites relevanter medizinischer und pflegerischer Organisationen durchgeführt. Die Qualität jener Leitlinien, welche die Einschlusskriterien erfüllten, wurde mit dem AGREE-II-Instrument bewertet. Die extrahierten Ernährungs- und Bewegungsempfehlungen aus den Leitlinien wurden systematisch zusammengefasst und gegenübergestellt. Ergebnisse: Die Literaturrecherche ergab, dass nur wenige evidenzbasierte Leitlinien existieren, die spezifische Empfehlungen zu Ernährungs- und Bewegungsinterventionen für erwachsene Personen mit Adipositas enthalten. Die Empfehlungen sind teils widersprüchlich und die Qualität variiert. Die extrahierten Empfehlungen umfassen verschiedene Aspekte wie die Art der Ernährung, die Intensität und Art der körperlichen Aktivität und den Umgang mit Komorbiditäten. Es wird etwa zu einem individuellen Ansatz in der Ernährungstherapie, zu einer Reduktion der Kalorienzufuhr, zur Portionskontrolle, sowie zur Einhaltung des mediterranen Ernährungsmodells geraten. Die Bewegungsempfehlungen beinhalten unter anderem körperliche Bewegung allgemein, körperliche Aktivität bis zu 250 und auch über 250 Minuten/Woche, sowie intensives Bewegungstraining. Schlussfolgerung: Pflegepersonen haben in ihrer täglichen Praxis häufig wenig Zeit und Ressourcen, um sich ausführlich mit evidenz-basierten Leitlinien auseinander zu setzen. Die Entwicklung evidenzbasierter Handlungsempfehlungen für Ernährungs- und Bewegungsinterventionen bei erwachsenen Personen mit Adipositas ist daher eine wichtige Maßnahme zur Verbesserung der Pflegepraxis. Pflegepersonen können von klaren, praxisnahen Empfehlungen profitieren und damit leichter evidenzbasierte Empfehlungen in die Pflegepraxis implementieren.
