Bei entsprechender Indikation kann die IVT als „Bridging-Thrombolyse“ (BT) mit der MT kombiniert werden, was laut derzeitiger Datenlage zu einem verbesserten Outcome führt. Durch die IVT kann es jedoch zu Komplikationen, insbesondere Blutungen und Embolien kommen. Ziel der Arbeit ist die retrospektive Datenanalyse von PatientInnen, welche mittels mechanischer Thrombektomie behandelt wurden, um den Einfluss der Bridging-Thrombolyse auf das Outcome zu untersuchen. Consequently, the number of patients developing bone metastases has increased, with a subsequent need for adequate therapy goals and treatment. In the decision-making progress regarding best treatment for patients with metastatic bone disease, their prognosis plays an integral role. As prognosis depends on various factors, prognostic scoring models have been developed in patients with spine and extremity metastases. To implement such a score in everyday clinical practice, it should be repeatedly validated, easy to use, and reliably and accurately estimate the patient's prognosis. In this study, we set out to assess nine different prognostic scoring models regarding their prognostic accuracy. Dabei spielt das mangelnde Wissen um die vielfältige Morphologie der Vulva eine nicht zu vernachlässigende Rolle.\r\nDas Ziel dieser vorliegenden Arbeit ist es, mittels standardisierten Vermessungen die morphologischen Unterschiede in der Anatomie der Vulva zu erfassen. Darüber hinaus soll die Zufriedenheit mit dem eigenen Genital und die Einstellung zur Intimchirurgie der Teilnehmenden erhoben werden. Die bis dato bestehende Datenlage zu dieser Thematik umfasst einige wenige internationale Studien.\r\n\r\nMaterial und Methoden\r\nIm Zuge dieser prospektiven Studie wurden zwischen Juli und Oktober 2023 die Vulven von 42 Personen vermessen. Dies geschah in Zusammenarbeit mit dem Vulvarium Graz, in dem Gipsabdrücke der Vulva zur Aufklärung angefertigt werden. Die nach Vorlage von Lloyd et al. erhobenen anatomischen Strukturen umfassten: Labia majora (Länge), Labia minora (Länge), Labia minora (Breite), Präputium clitoridis (Länge), Abstand von Klitoris – Ostium urethrae externum, Perineum (Länge) und Farbe. Die Sichtweisen der Teilnehmenden wurden mittels Fragebogen in Form einer 4-teiligen Likert-Skala erhoben. Anschließend wurden die Ergebnisse mittels deskriptiver Statistik ausgewertet und mithilfe des Chi-Quadrat-Tests Zusammenhänge zwischen den Messwerten und den gegebenen Antworten im Fragebogen evaluiert.\r\n\r\nErgebnisse\r\nDas mediane Alter der 42 Personen lag bei 31,5 Jahren. Für die Labia majora ergab sich eine durchschnittliche Länge von 92 ± 14 mm, für die Labia minora eine Länge von 44 ± 13 mm und eine Breite (rechts) von 17 ± 7 mm sowie eine mittlere Distanz von Klitoris zur Urethra von 29 ± 8 mm. Es zeigten sich statistisch signifikante Zusammenhänge, einerseits zwischen Labia minora >2 cm und der Angabe von Irritationen, andererseits zwischen gemessener Asymmetrie >5 mm und\r\niv\r\nwahrgenommener Asymmetrie. Die Resultate der Befragung wiesen auf eine mehrheitliche Zufriedenheit mit dem eigenen Genital hin, wohingegen die Studienpopulation der Intimchirurgie eher ablehnend gegenüberstand.\r\n\r\nDiskussion\r\nDurch diese Arbeit konnten wertvolle Daten zur Darstellung der Diversität des Aussehens der Vulva erhoben werden. In der Gegenüberstellung mit bestehenden Publikationen konnten übereinstimmende Ergebnisse festgestellt werden. Die Messwerte dieser Studie können gemeinsam mit bereits bestehenden Daten als wichtiger Bestandteil der Aufklärungsarbeit, aber auch zur Indikationsstellung von intimchirurgischen Eingriffen herangezogen werden. It may morphologically resemble low-grade myxofibrosarcoma which, due to the absence of specific immunohistochemical markers in both entities, can cause diagnostic challenges. In recent years, DNA methylation and copy number profiling have been established as alternative tools that can provide valuable additional information for a more accurate classification of different neoplasms. This study aimed to identify the undescribed methylation profile of cellular myxoma and compare it to low grade myxofibrosarcoma. We performed molecular analysis on twenty cellular myxomas and nine myxofibrosarcomas and analyzed the results using the methylation-based DKFZ sarcoma classifier. A total of 90% of the cellular myxomas had GNAS mutations (four loci had not been previously described). Copy number variations were found in all myxofibrosarcomas but in none of the cellular myxomas. In the classifier, none of the cellular myxomas reached the 0.9 threshold required for a valid classification. Unsupervised tSNE analysis demonstrated that cellular myxomas form their own clusters, distinct from myxofibrosarcomas. Our study shows the diagnostic potential, but also the limitations of molecular analysis in cases where morphology and immunohistochemistry are not sufficient to distinguish cellular myxoma from myxofibrosarcoma, particularly regarding GNAS wild-type tumors. The DKFZ sarcoma classifier only provided a valid prediction for one\nmyxofibrosarcoma case; this limitation could be improved by training the tool with a more considerable number of cases. In dieser Hinsicht haben sowohl das Orexin- als auch das Melanin-Concentrating Hormone (MCH)-System großes Potential. Die Orexin-Rezeptoren 1 und 2 sind G-Protein-gekoppelte-Rezeptoren, die im Gehirn exprimiert sind. An ihnen binden die Liganden Orexin A und B, die im Hypothalamus gebildet werden und eine aktivitätssteigernde Wirkung besitzen. Durch eine pharmakologische Antagonisierung dieser beiden Rezeptoren kann eine schlaffördernde Wirkung erreicht werden. MCH hingegen ist ein Neuromodulator, der an multiplen physiologischen Funktionen wie der homöostatischen Energieregulation und am Essverhalten beteiligt ist. Dieser Prozess wird zumeist von einer Entzündung des angrenzenden Myokards begleitet, wodurch der Begriff Peri-(Myo-)karditis Verwendung findet. Die zeitliche Einteilung erfolgt in akut, rezidivierend, persistierend und chronisch. Bei thorakalen Beschwerden ist ein breites Spektrum an Differentialdiagnosen zu berücksichtigen. Jedoch besteht die Gefahr, dass die Symptome falsch interpretiert werden, da der Fokus zumeist auf häufig auftretenden kardialen Ereignissen liegt. Daher wurde ein Schwerpunkt der Diplomarbeit auf die differentialdiagnostische Unterscheidung der Peri- (Myo-)karditis von ischämischen Ereignissen, wie einem Myokardinfarkt oder einer restriktiven Kardiomyopathie gelegt. Diese Diplomarbeit befasst sich ebenso mit der Klinik, den Möglichkeiten der Diagnostik, den Komplikationen und der Therapie.\r\nMethoden: Im Zuge des Verfassens der Diplomarbeit wurde eine detaillierte Literaturrecherche durchgeführt, um die oft verkannte Diagnose Peri-(Myo-)karditis zu beschreiben und Unterscheidungsmerkmale zu den differentialdiagnostisch möglichen Erkrankungen herauszuarbeiten. Die Literaturrecherche erfolgte via PubMed sowie den Portalen Google Scholar und Ovid MEDLINE, um aktuelle Studienergebnisse vergleichen zu können. Als weitere Quellen sind Fachbücher, Guidelines, wissenschaftliche Reviews und Studien zu nennen, die sich mit (peri-)kardialen Erkrankungen befassen.\r\nErgebnisse: Trotz einiger Merkmale, die sowohl bei der Peri-(Myo-)karditis als auch bei den weiteren Differentialdiagnosen vorgefunden werden können, ist eine Unterscheidung möglich. So sind im Rahmen der akuten Erkrankung vier Hauptdiagnosekriterien feststellbar, die mithilfe des Elektrokardiogramms, der Auskultation und der Echokardiographie detektiert werden können. Es ist vor allem in Bezug auf die Diagnostik zu erwähnen, dass die Perikarditis in einigen Fällen klinisch inapparent verbleiben kann. Doch können als Folge der Herzbeutelentzündung schwerwiegende Komplikationen auftreten, zu denen der Perikarderguss, die Herzbeuteltamponade, eine Pericarditis constrictiva und die tuberkulöse Perikarditis zählen. Therapeutisch ist die invasive von der medikamentösen Vorgehensweise zu unterscheiden.\r\nKonklusion: Obwohl die Peri-(Myo-)karditis eine seltenere Differentialdiagnose bei thorakalen Schmerzen darstellt, sollte sie dennoch im Rahmen der Diagnostik bedacht werden, da schwerwiegende Komplikationen die Folge sein können.
