Ziel dieser Diplomarbeit war es, den Verlauf sowie die Auswirkungen der Pandemie auf Hämodialyse-Patient*innen, die eine Risikogruppe für schwere Verläufe darstellen, zu analysieren. \r\nRetrospektiv konnten vom Zeitraum März 2020 bis Ende August 2022 die Daten von 274 SARS‑CoV‑2 positiven Hämodialyse-Patient*innen an der Abteilung für Nephrologie der Universitätsklinik für Innere Medizin Graz identifiziert werden. Mittels deskriptiver Statistik sowie logistischer Regression erfolgte die Analyse von potenziellen Risikofaktoren für Hospitalisierung sowie Mortalität. Für den Vergleich der Delta- und Omikron-Welle wurden zudem Daten von nicht infizierten Hämodialyse-Patient*innen aus dem Zeitraum August 2021 bis Ende August 2022 erfasst. \r\nAnhand des zeitlichen Verlaufes der Pandemie und der Auswertung der Sequenzierungsergebnisse konnten fünf Wellen nachweisen werden, die von verschiedenen Virusvarianten dominiert wurden. Der Pandemiebeginn war von hohen Hospitalisierungs- (58,2-100 %) und Mortalitätsraten (20-42,9 %) in unserer Hämodialysekohorte geprägt. Mit dem Aufkommen der Omikron-Variante ging erstere auf 16,2 % und zweitere auf 1,2 % zurück. Impfungen reduzierten in der univariablen Analyse, nicht aber in der multivariablen Regression, das Risiko schwerer Verläufe. Pulmonale Vorerkrankungen sowie Diabetes mellitus konnten in der multivariablen Regression als Risikofaktoren sowohl für Hospitalisierung als auch Mortalität identifiziert werden.\r\nDie Analyse der Daten deutet darauf hin, dass insbesondere das Auftreten der weniger virulenten Virusvariante Omikron für eine deutliche Reduzierung schwerer COVID‑19 Verläufe bei Hämodialyse-Patient*innen verantwortlich war, während Impfungen in der Analyse einen geringeren Einfluss hatten. Der HbA1c kann jedoch von vielen Bedingungen beeinflusst werden (Glykationsrate von Hämoglobin, Nierenfunktion, Alter, Hämoglobinopathien) und hat auch den Nachteil, dass er nur einen Durchschnitt über 3 Monate zeigt, ohne Probleme wie Hypo- oder Hyperglykämien zu erfassen. Zunehmende Verbreitung fanden in den letzten 5 Jahren kontinuierliche Gluukosemonitoringsysteme (CGM-Systeme). Mit diesen kann man nicht nur akute Therapieentscheidungen bei Diabetes mellitus treffen, sondern kann die Daten auch zur Bewertung der glykämischen Kontrolle heranziehen. Diese CGM-basierten Metriken haben sich in den letzten Jahren als Ergänzung zu oder sogar Ersatz für den HbA1c zunehmend durchgesetzt. CGM-Metriken wie die Zeit in verschiedenen Zielbereichen und der Glucose Management Indicator (GMI) können einen besseren Einblick in die glykämische Kontrolle bieten. Es wird berichtet, dass eine Zeit im Zielbereich (TIR) von 70-180mg/dl von 70% einem HbA1c von 7,0% (53 mmol/mol) entspricht. Bei einigen Menschen mit Diabetes wurde jedoch ein Abweichung vom GMI zum HbA1c festgestellt. Das Ziel der Arbeit ist es, zu untersuchen, ob der Unterschied und die Übereinstimmung zwischen HbA1c und GMI bei Menschen mit Diabeets über die Zeit stabil ist, oder ob dieser Zusammenhang, sich über die Zeit verändert. Daten von Menschen mit Diabetes, die an der Diabetesambulanz des LKH Univ. Doch neben klassischen schulmedizinsichen Ansätzen, kamen in China - einem Land mit langer Tradition für alternative Heilmethoden (vgl. Hays 2005 : 113-117) - auch TCM-Extrakte zum Einsatz (vgl. Guo et al. 2022). Den Erfolg dieser, bewies eine Reihe klinischer Studien (vgl. Tian et al. 2020; Xiong et al. 2020). Allerdings sind die Wirkmechanismen bis dato noch wenig verstanden und die Datenlage weitestgehend unzureichend (vgl. Qiu et al 2020). \r\n\r\nIm Atemweg eines schwer an COVID-19 erkrankten männlichen Patienten konnte man Mukus reich an MUC5AC feststellen, währenddessen die Konzentration von MUC5B stark reduziert war (vgl. Yin et al. 2021). Neben der Menge an Muzin, dürfte also auch das Verhältnis unterschiedlicher Muzine für den Krankheitsverlauf von Bedeutung sein.\r\n\r\nZiel dieser Arbeit war es daher die Wirkung zweier Hanshiyi Dekokte in unterschiedlichen Konzentrationen auf die Synthese der Muzinproteine MUC5AC und MUC5B in physiologischen humanen epithelialen Zellen (BEAS-2B) zu testen und sowohl qualitativ als auch quantitativ zu analysieren. \r\n\r\nNach dreitägiger Inkubation der BEAS-2B Zelllinie auf ALI-Kultur mit den jeweiligen Hanshiyi Dekokten konnten lediglich sieben der acht verwendeten Konzentrationen aufgrund von Kontamination weiter aufgearbeitet werden. \r\nIn den histologischen Schnittbildern zeigte sich ein sehr dichter Zellrasen, der sich teilweise oder vollständig von der perforierten Membran gelöst hatte. Hierbei werden in der Prävalenz dieser Erkrankungen Unterschiede in verschiedenen Bevölkerungsgruppen beschrieben. Während die Ursachen und das Patient:innenkollektiv heterogen sind, konnte ein gehäuftes Auftreten in jenen Gruppen beschrieben werden, welche durch die sozioökonomischen Faktoren wie dem formellen Bildungsgrad und dem Einkommenslevel schlechter gestellt sind als ihre Mitmenschen. Diese Tatsache stellt eine mögliche Basis für zielgerichtete gesundheitspolitische Maßnahmen dar, um Hochrisikogruppen zu identifizieren und der Krankheitsentstehung frühzeitig vorzubeugen. In der folgenden Diplomarbeit wird hierzu die aktuelle sozialmedizinische Literatur in Bezug auf das Einkommen, sowie den Bildungsstand erörtert und der Status Quo in Österreich festgehalten.
