Durch die forcierte Entwicklung neuer nicht-invasiver radiologischer Untersuchungsmethoden kann heute die Multi-Slice-Computertomographie (=MSCT) Untersuchung mit hoher Sensivität und Spezifität bezüglich der Graftoffenheits- und Okklusionsrate durchgeführt werden. Ziel unserer Studie war es, bislang fehlende Daten zur Langzeitoffenheitsrate (mittlerer Beobachtungszeitraum 42 Monate) maschinell mittels Symmetry-Aortic-Connector-Device (= SACD, St-Jude-Medical) an die Aorta ascendens anastomosierter venöser Bypassgrafts zu erlagen, da das längste bisher in der Literatur beschriebene Follow-up bei 12 Monaten liegt. Als diagnostisches Tool wurde das Somatom Sensations Cardiac 64-Zeiler-MSCT verwendet.\r\nPATIENTEN und METHODIK: Zwischen November 2001 und Juli 2003 wurden 42 Patienten an der klinischen Abteilung für Herzchirurgie der Medizinischen Universität Graz mit SACD operiert. Die Nachuntersuchung wurde bei 24 Patienten durchgeführt; 22 Patienten wurden mittels MSCT, zwei Patienten aufgrund einer Flimmerarrhythmie > 85 Schlägen/Minute mittels Electron Beam Tomography (=EBT) untersucht. Die Bypassgrafts wurden anhand axialer Schichtbilder und dreidimensionaler VRT-Bildrekonstruktion auf ihre Offenheit beurteilt. Mit Hilfe der Syngo Vessel View Software wurde eine Stenoseevaluierung offener Bypassgrafts durchgeführt, offene Grafts wurden nach den Fitzgibbon-Kriterien in A (< 50% Stenose) und B (> 50% Stenose) klassifiziert. Mittels logistischer Regression wure in einer mulitvarianten Analyse der Einfluss der Zielgefäßcharakteristika, der intraoperativen Flussraten im Bypassgraft und des postoperativen Antikoagulationsregimes auf die Offenheit von Bypassgrafts untersucht. \r\n\r\nRESULTATE: Die Bypassgrafts konnten zu 100% im MSCT visualisiert werden. Von 54 untersuchten Grafts waren 30 offen und 24 verschlossen. Die Bypassoffenheitsrate arterieller LIMA-Grafts lag bei 100%, es lag keine Graftstenose vor. Die Offenheitsrate maschinell gefertigter Saphenagrafts lag nach 41 Monaten bei 45%; 41% der offenen Venengrafts wiesen eine Grad-B-Stenose nach den Fitzgibbon-Kriterien auf. Die Okklusion der Venengrafts war in allen Fällen an der Stelle der proximalen Anastomose lokalisiert. Alle 7 Graftstenosen waren aortoostial lokalisiert. Zielgefäße mit einer über 90-prozentigen Stenose zeigten eine Bypassoffenheitsrate von 60%; solche mit einer Stenose unter 90% eine Graftoffenheitsrate von 33% (p=0,041).
