Deutsche von chronischen Schmerzen betroffen und verursachen in Deutschland volkswirtschaftliche Gesamtkosten von etwa 38 Milliarden Euro pro Jahr. Für die Behandlung dieser Schmerzpatient*innen ist eine multimodalen Schmerztherapie in speziellen schmerztherapeutischen Einrichtungen anzustreben. In Anbetracht der Anzahl an Schmerzpatient*innen, gibt es derzeit jedoch viel zu wenige solcher Einrichtungen, ebenso ist die Ausbildung von Fachkräfen für Schmerztherapie unzureichend. Die COVID-19-Pandemie verschärfte diese Situation.\r\nIm Rahmen der vorliegenden Arbeit soll der postpandemische Ist-Versorgungszustand 2022 der Schmerzambulanzen erhoben und dessen Veränderung in den letzten Jahren aufgezeigt werden, um gegebenenfalls Defizite sowohl im therapeutischen, personellen als auch organisatorischen/administrativen Bereich diskutieren zu können.\r\n\r\nMethoden\r\nIn dieser Querschnittstudie wurden 145 deutsche Kliniken – gelistet als Schmerzzentren der Schmerzgesellschaften DGS und DGSS – per Online-Umfragebogensystem LimeSurvey kontaktiert. Der Umfragebogen enthielt insgesamt 34 Fragen zur schmerztherapeutischen Versorgung in Schmerzambulanzen und des Akutschmerzdienstes und konnte von Anfang Mai 2022 bis Ende Juni 2022 von den Teilnehmer*innen ausgefüllt werden. Im Rahmen dieser Arbeit erfolgte eine Auswertung der erhobenen Daten zur chronischen Schmerztherapie mittels rein deskriptiver Datenanalyse. Die erhobenen Ergebnisse sollen die aktuelle Versorgungssituation und die Veränderungen über die letzten Jahre darstellen.\r\n\r\nErgebnisse\r\nVon den ursprünglich 145 kontaktierten Kliniken nahmen 82 teil, was einer Rücklaufquote von 57% entspricht. Zwei der teilnehmenden Kliniken bestätigen eine Schließung der Schmerzambulanz an, zehn führten noch nie eine, was durch Ressourcenmangel auf verschiedenen Ebenen begründet wird. In den derzeit bestehenden Schmerzambulanzen zeigt diese Umfrage, dass sich die Hauptindikationen für eine Behandlung in einer Schmerzambulanz mit der aktuell vorherrschenden Prävalenz decken und erfreulicherweise eine dem aktuellen Forschungsstand entsprechende multimodale Therapie durchgeführt wird. Ob ausreichend spezialisiertes Personal (3 [2,5] Ärzt*innen, 3 [2,4] Pain Nurses und 2 [1,2] Physio-/Ergotherapeuten mit spezieller Schmerzausbildung pro Schmerzambulanz) vorhanden ist, um eine multimodale Schmerztherapie durchzuführen, muss noch geklärt werden. Verbesserungsbedarf gibt es im organisatorischen bzw. administrativen Bereich, vor allem hinsichtlich Wartezeiten (60 [30,90] Tage) und Betriebszeiten (29 [20,39,625] Stunden/Woche). Weiter konnte gezeigt werden, dass die COVID-19-Pandemie lediglich eine Erhöhung der Erstkontakte bei Schwerpunktkrankenhäusern (SP: 2019: 800 [300;1600]; 2020: 900 [287,5;1575]; R = 6 < W(11) = 10) bewirkte, bei diesen jedoch die Frequenz der Patient*innenbesuche gesunken ist (SP: 2019: 4 [2,25;4]; 2020: 3 [2;4]; R = 0 < W(8) = 3).\r\n\r\nSchlussfolgerung\r\nDie Ergebnisse unserer Studie zeigen die Notwendigkeit einer Versorgungsverbesserung von Patient*innen mit chronischen Schmerzen in deutschen Kliniken auf. Die Schmerztherapie chronischer Schmerzpatient*innen an sich entspricht erfreulicherweise den aktuellen Standards, es wird zunehmend mehr Wert auf die psychische Komponente des biopsychosozialen Modells gelegt. Jedoch gilt es administrative und personelle Hindernisse zu überwinden, um den Bedürfnissen dieser Patientengruppe gerecht zu werden. Eine stärkere Unterstützung seitens der Gesundheitspolitik und eine wirkungsvollere Integration von Schmerzambulanzen in das Gesundheitssystem könnten dazu beitragen, die Versorgungssituation zu verbessern.\r\nDie angemessene Versorgung von chronischen Schmerzpatient*innen ist von großer Bedeutung, da Schmerzen die Lebensqualität beeinträchtigen und einen erheblichen Anteil an den volkswirtschaftlichen Gesamtkosten ausmachen. Es wird aus den Kernen des Steirischen Ölkürbis (lat. Cucurbita pepo subsp. pepo var. styriaca) nach einem traditionellen Verfahren hergestellt. Die Bezeichnung Steirisches Kürbiskernöl ist eine geographisch geschützte Angabe und darf nur für Öle aus bestimmten Anbauregionen verwendet werden. In dieser Arbeit wird die derzeit bestehende Evidenz in Bezug auf die Auswirkungen auf den menschlichen Organismus des Steirischen Kürbiskernöls zusammengefasst. Insbesondere wird auf die im Öl enthaltenen Inhaltsstoffe sowie auf deren Auswirkungen auf den Stoffwechsel eingegangen. Die Literaturrecherche erfolgte über PubMed und Google Scholar, wobei sie sich auf in PubMed-gelistete Studien konzentrierte. Im Steirischen Kürbiskernöl sind einfach und mehrfach ungesättigte sowie gesättigte Fettsäuren enthalten. Der Anteil an mehrfach ungesättigten Fettsäuren liegt bei etwa 84 % der Gesamtfettsäuren. Die dominanten Fettsäuren im Steirischen Kürbiskernöl sind die Linolsäure, Ölsäure, Palmitinsäure und Stearinsäure. Ebenfalls sind zahlreiche Mineralstoffe, vor allem Kalium, sowie fettlösliche Vitamine, insbesondere α- und γ-Tocopherol enthalten. Diesen Inhaltsstoffen werden gesundheitsfördernde Eigenschaften zugesprochen, weshalb das Kürbiskernöl längst zahlreiche Anwendungsmöglichkeiten in der traditionellen Medizin findet. Einige dieser Effekte des Steirischen Kürbiskernöls konnten sowohl in Tierstudien als auch in klinischen Studien bestätigt werden. Besonderer Fokus lag bei den Tierstudien auf den antioxidativen, kardioprotektiven, wundheilungsfördernden, antimikrobiellen und antitumoralen Eigenschaften sowie auf dessen Einfluss auf das Plasmalipidprofil. Bei den klinischen Studien wurden vor allem die Effekte auf die benigne Prostatahyperplasie, die androgenetische Alopezie sowie die kardioprotektive Wirkung insbesondere bei postmenopausalen Frauen untersucht. Additionally, sclerostin improves the endothelial features that are altered as a consequence of PAH progression such as nitric oxide (NO) production and barrier integrity via its inhibitory effect on Src/PYK-2 pathway. Moreover, sclerostin influences the endothelial metabolism dependent on Adenosine monophosphate (AMP)-activated protein kinase (AMPK) expression. It enhances expres-sion of genes involve glucose uptake and glycolysis rate within hPAECs. The expression of lipogen-esis and lipolysis genes show a significant increase upon sclerostin stimulation. In order to further understand if sclerostin enhances lipogenesis or lipolysis, enzyme activity assays should be imple-mented. In order to investigate the molecular mechanisms underlying its protective effect, we took advantage of an unbiased approach such as the kinase array. Hence, we screened several kinases in order to understand which molecular pathway is preferentially activated by sclerostin and we con-firmed these results by western blot on sclerostin stimulated hPAECs. To note, sclerostin shows anti-proliferative effect via its inhibitory action on mTOR and STAT signaling, which could be a novel therapy against altered proliferation, which is one of the main features of endothelial dysfunction in PAH.\r\nIn conclusion, sclerostin seems to be a promising mediator that could improve many endothelial features within the pulmonary vasculature.
