            "title": "Verletzungen bei Jugendlichen in Folge von Mopedunfällen in der Steiermark\r\nRetrospektive Analyse von 2004 bis 2022",
            "abstract": "Einleitung\r\nMopedunfälle gehören zu den häufigsten Gründen für die Verletzung von Jugendlichen im Straßenverkehr. Das Ziel dieser Arbeit waren weitere allgemeine Recherchen zu Unfallrisiken, Unfallmechanismen, Verletzungsmustern und Ursachen von Mopedunfällen durchzuführen. In der Einleitung erfolgte zusätzlich ein gesetzlicher Exkurs zu Änderungen im Führerscheingesetz.\r\n\r\nMaterial und Methoden\r\nMit dieser Diplomarbeit und der damit verbundenen retrospektiven Studie wurden alle Mopedunfälle von 2004 bis 2022, welche an der Universitätsklinik für Kinder- und Jugendchirurgie Graz behandelt wurden, analysiert und ausgewertet. Hierfür wurde erstmals ein langer Studienzeitraum gewählt, um ein ausreichend großes Patient*innenkollektiv zu schaffen. Als Bewertungsmöglichkeit für die Verletzungsschwere wurde der Injury-Severity-Score (ISS) gewählt. \r\n\r\nErgebnisse/Diskussion \r\nIn den Jahren von 2004 bis 2022 kam es zu einer Zunahme der Mopedunfälle, wobei am häufigsten 15-jährige (52,8%) männliche Jugendliche (56,8%) verletzt wurden. Bei 73,3% der Unfälle waren andere Fahrzeuge beteiligt und 16,7% aller verletzten Jugendlichen wurden aus einem anderen Krankenhaus zugewiesen, während die Zuweisungen im Studienzeitraum zugenommen haben. Mit Abstand am häufigsten waren die Patient*innen leicht verletzt (94,7%) und hatten einen durchschnittlichen ISS von 3,00. Schließlich hat sich gezeigt, dass männliche Jugendliche um 33,6% schwerer verletzt wurden als weibliche Jugendliche (♀=2,34/♂=3,51). Die deutlich am häufigsten verletzte Körperregion waren die Extremitäten (95%), gefolgt von Kopf (15%), Becken (7,2%), Thorax (7%), Abdomen (4,5%) und Gesicht (3,5%). Bei Unfällen mit Fremdbeteiligung zeigten sich nicht nur eine höhere Verletzungswahrscheinlichkeit in allen Körperregionen, sondern auch ein höherer mittlerer ISS (1,96/7,00) und eine 4,1-mal höhere Wahrscheinlichkeit intensivmedizinische Therapie zu benötigen. Bei zugewiesenen Jugendlichen nach Mopedunfällen waren Extremitäten- und Beckenverletzungen seltener als bei nicht zugewiesenen Patient*innen, außerdem hatten sie einen höheren ISS (4,28/2,74). Als häufigste Diagnostik wurde, das Röntgen (84,5%) verwendet und es zeigte sich, dass die schwerer Verletzten meist mit einem Notarztmittel eingeliefert wurden. Jugendliche auf der Intensivstation hatten einen höheren ISS (19,7), eine größere Verletzungswahrscheinlichkeit in allen Körperregionen und mussten im Schnitt 4,0 Tage beatmet werden (wenn nötig). Die Gesetzesänderungen, sowie die Covid-Pandemie könnten vorbehaltlich Einflüsse auf den ISS gehabt haben. Zuletzt wurde noch deutlich, dass die meisten Mopedunfälle im Juli und an Freitagen passieren. \r\n\r\nZusammenfassung\r\nSchließlich konnten im Laufe dieser Arbeit umfangreiche Erkenntnisse zu Mopedunfällen gesammelt und mit der aktuellen Studienlage verglichen werden. Dafür wurden die Ergebnisse und die dazugehörigen Forschungsfragen sowie Hypothesen mit den Ergebnissen anderer internationaler Studien und diverser Literatur verglichen.",

            "title": "Applying optical coherence tomography angiography to evaluate potential retinal vascular alterations in pregnant women both with and without diabetes mellitus",
            "abstract": "Introduction. Pregnancy is considered a period of physiological and hemodynamic changes with potentially altering effects on the vascular system of the eye. It is known that pregnancy may promote the progression of diabetic retinopathy in the presence of diabetes mellitus. The aim of this pilot study is to use optical coherence tomography angiography (OCTA) to perform a detailed examination of retinal vessels in order to assess the effects of pregnancy, preexisting diabetes mellitus and gestational diabetes mellitus on retinal capillary plexus density.\r\n\r\nMethods. A total number of 20 pregnant, including 12 women with preexisting diabetes mellitus (PexD), 5 women with gestational diabetes mellitus (GDM), and 3 healthy pregnant women, were compared to 19 healthy non-pregnant women by using OCTA over two different times of examination throughout pregnancy. The vessel density of the superficial capillary plexus (SCP), the deep capillary plexus (DCP), and the radial peripapillary capillary plexus (RPCP), as well as the size of the foveal avascular zone (FAZ) were examined.\r\n\r\nResults. In descriptive terms, the vessel density of the SCP increased throughout pregnancy across all groups. Regarding the whole macula, the SCP vessel density increased by an absolute value of 0.40% in healthy pregnant women, by 0.53% in women with PexD and by 0.29% in the control group. In line with these results, the DCP vessel density increased across all groups, with a mean increase of 7.17% within the healthy pregnant women, of 1.92% within the women with PexD and of 0.71% within the healthy controls. However, while no change in vessel density was observed in the healthy pregnant women regarding the RPCP vessel density, a decrease of -0.50% in the pregnant women with PexD and -0.42% in the healthy non-pregnant women was observed.\r\n\r\nDiscussion. Regarding the retinal plexus vessel density, the current data indicates a trend which suggests that there is an increase in vessel density during pregnancy. Furthermore, these changes appear to differ between healthy pregnant women and pregnant diabetics. To assess these differences more precisely, further evaluations are necessary in the future, which will be carried out as part of the ongoing study project.",

            "title": "Langzeitverlauf nach Ösophagektomie wegen Ösophagusperforation nicht maligner Genese",
            "abstract": "Introduction: Esophageal perforations caused by non-malignant diseases are still \r\nassociated with high morbidity and mortality rates. The purpose of this study was to \r\nidentify risk factors associated with survival. \r\nMaterial and Methods: Eighty-two patients between 18 and 90 years of age treated \r\nfor non-malignant esophageal perforations between 1\r\nst of January 2004 to 1\r\nst of \r\nJanuary 2023 at the Division of Thoracic Surgery and Hyperbaric Surgery at the \r\nMedical University of Graz were included in this study. The primary objective was \r\noverall survival. Secondary objectives were biometric data (height, weight and BMI), \r\nsex, risk factors (alcohol, nicotine), comorbidities, intra- and perioperative \r\ncomplications as well as the duration of stay at intensive care units and general \r\nwards. The data were analyzed with STAT-A.\r\nResults: 29 (35,37%) women and 53 (64,63%) men were included. With a hazard \r\nratio (HR) for risk of death for men of 1,992708 when compared to women the risk \r\nof the former was almost twice as high (p=0,039), though men were 10 years \r\nyounger than women (mean age: 59,3 ±14,7 years versus 69,9 ±14,9 years; \r\np=0,0057) at the time of perforation. Risk factors in the patients´ history, such as \r\nsmoking and high consumption of alcohol, were much more prevalent in men. In the \r\ntotal collective mortality was significantly higher in smokers with a hazard ratio of \r\n2.094124, indicating a risk twice as high than in non-smokers. In general, patients \r\nwith comorbidities had a three times higher mortality risk than patients who had been\r\nhealthy until perforation (HR: 3.62818; p=0.002). Especially nicotine-associated \r\ndiseases had a high impact on the long-term survival. Hazard ratio for COPD was \r\n1.739515 (p=0.015) and the HR for heard diseases was as high as 2.854773 \r\n(p=0.002). The mean hospital stay (ICU and general ward combined) in the entire \r\nstudy population was 29 days. On an average, the patients had to be treated for 13 \r\ndays in an intensive care unit, and the risk of early death correlated with the duration \r\nof the ICU stay (p=0.005). CRP and albumin, measured before the operation, on \r\nthe 1st and 3\r\nrd day postoperative had no impact on long term-survival. However, the \r\nrate of complications was statistically significant (p=0.0068) lower in patients who \r\nhad higher albumin values before the operation. \r\nConclusion: In this study it was possible to identify potential factors associated with \r\nhigher mortality in non-malignant esophageal perforation. The risk of men, who were \r\nv\r\nmore frequently affected by this condition was twice as high than that of women, \r\neven though men were significantly younger at the time of perforation. This could \r\nbe due to a comparatively higher consumption of nicotine and alcohol in men, habits \r\nthat have been known to trigger comorbidities such as COPD and heart disease\r\nwhich had a significant, negative impact on the survival rate in the present study.",

            "title": "Aktuelle pharmakologische Entwicklungen zur Beeinflussung der Hämodynamik bei septischem Schock",
            "abstract": "Der septische Schock, welcher mit sehr hoher Mortalität einhergeht, stellt die schwerste Form der Sepsis dar und bedarf in seiner Therapie unter anderem einer adäquaten hämo-dynamischen Stabilisierung. Trotz ausgiebiger Forschung in diesem Bereich sind weiter-hin Fragen zur Menge und Art der Flüssigkeitstherapie sowie zu Art, Zeitpunkt und Men-ge der Vasopressoren offen.\r\nUm zu beantworten, ob es neue Entwicklungen in der pharmakologischen Therapie der Hämodynamik im septischen Schock, verglichen mit den Surviving Sepsis Campaign Leitlinien von 2021 gibt, wurde im Juli 2022 im Rahmen dieser Arbeit eine Literatur-recherche auf der Datenbank Public Medicine zu diesem Thema durchgeführt. Als primä-res Recherchekriterium wurde die 28-Tage Mortalität herangezogen. Zusätzlich wurden Studien analysiert, welche andere Kriterien der Leitlinien untersuchen. \r\nIm Rahmen der Literaturrecherche zeigt sich, dass die Therapie mit Noradrenalin sowohl bei hohen Dosierungen als auch verzögertem Ansetzen zu einer erhöhten Mortalität führt. Zusätzlich findet unter Noradrenalin eine vermehrte Ausschüttung antiinflammatorischer Zytokine statt. Dieser Effekt konnte bei Gabe von Vasopressin nicht beobachtet werden. Allerdings zeigt Vasopressin, bis auf in einer Subgruppe von Patient*innen, welche weni-ger als 15µg/min Noradrenalin erhielten und mit dem Serumlaktat unter 2,4±2,0mmol/l lagen, keinen signifikanten Effekt hinsichtlich der 28-Tage-Mortalität. Bei der Therapie mit Angiotensin II konnte in einer Subgruppe aus Patient*innen mit einem Serumrenin über 172,7 pg/ml im septischen Schock hingegen eine Mortalitätsreduktion festgestellt werden. Neben den vasopressiven Medikamenten führt auch eine frühe adäquate Antibio-se zur Verkürzung der Hypotensionsperiode. Unterstützend können Kortikosteroide, wenn früh eingesetzt, ebenfalls zu einer Mortalitätsreduktion führen. Dem entgegen zeigen die Studien zur Flüssigkeitstherapie trotz Unterscheidung der Reagibilität keine signifikanten Ergebnisse hinsichtlich der Mortalität.\r\nDie analysierten Studien weisen wenige Wiedersprüche, sondern vielfach Konkretisie-rungs- und Erweiterungsvorschläge bezüglich der SSC-Leitlinien 2021 auf. Diese betref-fen vor allem Dosierungen von Medikamenten oder konkrete Zeitpunkte für ihre Verab-reichung. In bestimmten Studien wird eine Reduktion der 28-Tage Mortalität gezeigt. Etwa in der Angiotensin II-Therapie, bei einem Serumreninwert über 172,7 pg/ml. Zudem zeigen die analysierten Studien weitere Kriterien, die zu einer Verbesserung des Outco-mes beitragen. Diese stellen einen wesentlichen Forschungsansatz für zukünftige Leitli-nien dar.",

            "title": "Das verkürzte Zungenbändchen in der pädiatrischen Praxis",
            "abstract": "Einleitung\r\nIn der pädiatrischen Praxis gewinnt die Diskussion über das verkürzte Zungenband, auch Ankyloglossie genannt, als potenzielle Ursache für Schwierigkeiten beim Stillen und der oralen Funktion zunehmend an Bedeutung. Es liegen allerdings für die Indikation und die wissenschaftliche Aufarbeitung nur wenige evidenzbasierte Daten vor. \r\n\r\nMaterial und Methoden\r\nIm ersten Teil wurde eine Literaturrecherche mittels Pubmed durchgeführt. Die Ergebnisse wurden nach Relevanz und Aktualität gefiltert, um das Krankheitsbild des verkürzten Zungenbandes vor allem in Bezug auf Diagnostik, Behandlung und einhergehende Komplikationen darzustellen. Im zweiten Teil wurde eine Online-Umfrage erstellt und an 528 niedergelassene KinderärztInnen in Österreich, Deutschland und der Schweiz gesendet. Insgesamt wurden 104 Fragebögen retourniert, was einer Rücklaufquote von 19,4% entspricht.\r\n\r\nErgebnisse\r\nDie Diagnose einer Ankyloglossie erfolgt mit Hilfe verschiedener Merkmale, welche entweder die Optik oder funktionelle Kriterien miteinbeziehen. Als weitere Möglichkeit wurde die Evaluierung der Symptomatik nach subjektiven Kriterien der stillenden Mutter angegeben, da sich eine Ankyloglossie häufig durch Probleme beim Stillen äußern kann. Eine operative Durchtrennung des Zungenbändchens hat keinen nachgewiesenen Vorteil für die Kinder, ein kurzfristiger positiver Effekt beim Stillen für die Mutter ist jedoch wahrscheinlich. Die Umfrage zeigte, dass es sich zwar um einen komplikationsarmen Eingriff handelt, jedoch gelegentlich unerwünschte Effekte auftreten können. \r\n\r\nDiskussion\r\nEs gibt wenig Konsensus bezüglich der Diagnose und Behandlung einer Ankyloglossie. Eine operative Intervention soll kritisch hinterfragt und nur in klaren Fällen indiziert werden, da in der Literatur ein langfristiger Nutzen für das Kind nicht eindeutig ersichtlich ist. Die Umfrage bestätigt die Literatur, da zwar im Großteil der Fälle eine Verbesserung ersichtlich war, jedoch viele KinderärztInnen angemerkt haben, dass eine Überprüfung des Erfolges schwierig sei. Zudem bleibt unklar, ob die Problematik mit dem operativen Eingriff dauerhaft behoben ist, oder ob es sich um eine kurzfristige Verbesserung handelt.",

            "abstract": "Acute lymphoblastic leukemia (ALL) is a rare and aggressive malignant disease of the hematopoietic system with great biological diversity, which requires intensive and long-term therapy. Additionally, it only accounts for approximately 20% of acute leukemia cases in the adulthood and patients’ prognosis is, despite considerable progress in therapy, still unsatisfying. To date, there is no comprehensive evaluation of patients diagnosed with ALL, who have been treated at or transferred to the Division of Hematology, Department of Internal Medicine, of the Medical University of Graz. Therefore, the aim of this study is to evaluate the outcome of these patients treated from 2001 to 2021 and to compare these real-world data with published cancer registries and studies.",

            "abstract": "Diffuse Gliome des Erwachsenenalters werden in drei verschiedene Gruppen eingeteilt, Astrozytome, Glioblastome und Oligodendrogliome. Die Einteilung erfolgt vor allem anhand der histologischen Befunde und anhand von zwei genetischen Markern: IDH-Mutationen und 1p/19q Kodeletionen. Glioblastome sind weder IDH mutiert noch weisen sie eine 1p/19q Kodeletion auf. Astrozytome und Oligodendrogliome sind IDH mutiert, wobei Oligodendrogliome zusätzlich noch eine 1p/19q Kodeletion aufweisen müssen. Bei diesen Tumoren sind zusätzlich noch einige andere Gen-Mutationen und Copy Number Variations bekannt deren prognostische und diagnostische Bedeutung noch Großteiles unbekannt ist. Außerdem wirkt sich die Methylierung des MGMT-Promotors auf das Survival aus. Die Therapie ist von der Tumorentität abhängig, häufig wird eine Resektion mit einer Strahlentherapie und Chemotherapie kombiniert. Die Prognose ist bei den Tumoren höchst unterschiedlich, während das Overall Survival bei Glioblastomen in der Literatur mit eineinhalb Jahren angegeben wird, liegt das Progression Free Survival bei Oligodendrogliomen bei über 10 Jahren.",
