ANH is often associated with changes in hemodynamics and blood chemistry that could possibly alter the pharmacokinetics and pharmocodynamics of neuromuscular blocking agents. This might lead to overdosing of underdosing of the NMBD. METHODs: Twenty eight consecutive, ASA I-III, patients aged 50-70 yr, undergoing general anethesia with ANH for scheduled elective radical prostatectomy or radical cystectomy were included n the study. Patients were equally divided in two groups; the pharmacodynamics and pharmacokinetics groups. Each recruited ANH patient was matched, in another two pharmacodynamics and pharmacokinetics control groups for gender, ASA classification, approximate age and body weight with a patient undergoing scheduled elective surgical procedure without ANH. Oral midazolam 3.75-7.5 mg was the only premedication given 1h before anesthesia induction. Anesthesia was induced with propofol 2-3 mg/kg i.v. and a remifentanil 0.1-0.3 g kg-1 min-1 infusion until the eyelash reflex was lost. Following induction of anesthesia and before the start of surgery, 15 ml/kg blood (approximately 20/ of blood volume) up to a maximum 1000ml of the patients blood were withdrawn from the cubital vein and stored in an acid citrate in dextrose solution reservoir bag to be simultaneously replaced by 1000ml Hydoxyethyl Starch 130/0.4, rapidly infused via a separate venous cannula. The hematocrit (Hct) target after hemodilution was 25%-30%. The neuromuscular block at the adductor pollicis muscle was evaluated using the Relaxometer mechanomyograph (Groningen University Holland). RESULTS: Our study showed that the potency of rocuroium is increased during ANH, as indicated by a parallel shift to the left of rocuronium dose-response curve, indicating an enhancement of rocuronium neuromuscular block. The slope of the dose-response curve for rocuronium in the hemodilution group was not significantly different from the control. There were significant differences in ED50, ED90 and ED95 between the two groups. The ED50, ED90 and ED95 of rocuronium in the hemodilution group were decreased by 53.3%, 24.4% and 13% respectively, compared with the control group. We compared the efficacy and safety of BIS-guided regimen to a standard clinical SAS-guided regimen for the provision of optimal analgesia-sedation. We also assessed the feasibility of using Bispectral index-XP (BISxp) monitoring in quantifying the level of sedation. We hypothesized that the efficacy of a BIS-guided regimen was more than that of a standard clinical SAS-guided analgesia and sedation regimen.\r\n\r\nMETHODS Forty patients were randomly allocated to either the BIS-guided regimen of the SAS-guided group. In the intent-to-treat patients remifentanil (0.05 g kg-1min-1) infusion was titrated, in the first intent, to achieve optimal sedation defined as Sedation Agitation scale of 4. Propofol 1.5 mg kg-1h-1 infusion was started if additional sedation was required. Sedation Agitation scale of 4 achieved by the titration of remifentanil in the first intent followed by propofol infusion, to maintain patients at optimal sedation defined as SAS 4 (patient calm and cooperative and easily arousable).\r\n\r\nRESULTS The mean percentage hours of optimal sedation was significantly longer in the BIS-group (78.5 plus/minus 24.5) than in the SAS-group (66.5 plus/minus 8.5) (P value 0.0428). This was chieved with less frequent infusion rate adjustments in the BIS-group (2.33 changes/h remifentanil and 2.64 changes/h propofol) than in the SAS-group (3.46 changes/h remifentanil and 2.89 changes/h propofol). The mean total time of sedation and analgesia was not significantly different between the BIS-guided and SAS-guided group (59.0 plus/minus 18.61 and 60.5 plus/minus 23.50 min respectively). Remifentanil mean infusion rate was 0.0661 plus/minus 0.0264 g/kg/min in the BIS-group and 0.0713 plus/minus 0.0356 g/kg/min in the SAS-group, whereas propofol mean infusion rate was 1.55 plus/minus 0.44 mg/kg/h in the BIS-group and 1.82 plus/minus 0.44 mg/kg/h in the SAS-group. More subjects in the SAS-group (17/20) required propofol than in the BIS-group (11/20). The incidence of reported hypotension and apnea adverse events was low and comparable across the two treatment groups. Fifty-three patients undergoing general surgery were randomly allocated to be ventilated via one of the three airway devices.\r\nAll three devices could be inserted easily and rapidly, providing adequate ventilation and oxygenation in most of the study patients.\r\nMean arterial pressure (MAP) significantly increase with the insertion/intubation via the ILMA from 698 mmHg to 8418mmHg (p=0.0029). Whereas MAP did not significantly change with the insertion of the PLMA (from 6610 to 7112mmHg, p=0.1008) or with the insertion of the LTS (from 699 to 7516mmHg, p=0.186).\r\n\r\nThere was no significant difference between the MAP following the insertion of the PLMA of the LTS (p=0.152). However, with the extubaton of the ILMAs endotracheal tube, or with the removal of the PLMA or LTS, MAP significantly increased from 748, 8218, 749mmHg to 9714, 10119, 9416 mmHg respectively with no difference between the three groups (p=0.7331).\r\n\r\nThe mean norepinephrine plasma concentrations significantly increased with the insertion/intubation via the ILMA and the insertion of PLMA and LTS from 88.926.5, 13670.1, 84.427.5 pg/ml to 135.738.6, 158.474.9, 112.733.5 pg/ml respectively with no difference between the three groups (p=0.058).\r\n\r\nWhereas epinephrine only significantly increased with intubation in the ILMA group from 18.58.1 to 35.912.3 pg/ml (p=0.001) and with the insertion of the LTS from 15.56.4 to 26.77.5 pg/ml (p=0.0001) and did not change with the insertion of the PLMA (from 18.410.2 to 22.714.3 pg/ml, p=0.0898). The purpose of our study was to examine, in 30 patients undergoing general surgical procedures, the effect of different stages of neuromuscular block on BIS monitoring and to compare the conventional A-2000 BIS TM to the new BIS-XP TM system. At deep surgical anaesthesia of BIS of about 40, under propofol 3,61 g/ml target controlled infusion and 0,15-0,3 g kg-1 min-1 remifentanil infusion, mivacurium 0,15 mg/kg was administered.\r\nThe onset of neuromuscular block triggered a brief transient odd divergence in response manifesting as BIS increase from 43 +/- 4 to 49 +/- 7 (p=0,007) and BIS-XP decline from 41 +/- 3 to 35 +/- 3 (p=0,003) at 1 +/- 0,2 min and 1,4 +/- 0,5 min. After 2,5 +/- min after mivacurium administration, both monitors returned to baseline values of 43 +/- 5 and 40 +/- 4 respectively. Following that, BIS and BIS-XP did not significantly change during complete neuromuscular block or during various levels of neuromuscular recovery. Our study indicated that during deep surgical levels of propofol remifentanil anaesthesia, except for a brief transient odd divergence between BIS and BIS-XP artefacts in response to onset of neuromuscular block, BIS monitoring was not sensitive to the presence of absence of neuromuscular blockade.
