Jaffe (1953) beschreibt die Läsion erstmals als eine lokal reparative Reaktion des Knochens, die möglicherweise aufgrund intramedullärer Blutung oder vorangegangenem Trauma verursacht wird. Später wurde der Begriff reparativ gestrichen, da das ZRZG auch in einen destruierenden Prozess übergehen kann. In der neuesten WHO Kalssifikation (Barnes et al 2005) wird das ZRZG zentrale Riesenzellläsion bezeichnet. Das ZRZG tritt zumeist in den ersten drei Lebensdekaden auf und betrifft häufiger Frauen als Männer. Es ist überwiegend in den zahntragenden Kieferanteilen lokalisiert, wobei die Läsion häufiger in der Mandibula als in der Maxilla zu finden ist. Radiologisch tritt das ZRZG als Knochenaufhellung mit ein- oder mehrkammriger Architektur auf. Das klinische Erscheinungsbild reicht von einer langsam wachsenden, asymptomatischen Schwellung bis zu einer aggressiven Läsion, die sich mit Schmerzen, Wurzelresorption, Zahnverlagerung oder Knochendestruktion bis hin zur Frakturgefahr manifestiert. Als Therapieformen werden klassischerweise eine Cürettage bzw. eine en block Resektion, seltener Applikation von Calcitonin, Cortikosteroiden und Interferon angewendet. Aufgrund der Seltenheit dieser Läsion wurde aus einem unselektionierten Patientengut der Grazer Univ.-Zahnklinik in einem Zeitraum von 1984 bis 2004 10 Fälle von ausschließlich sporadischen ZRZG gefunden und mit der bekannten Literatur verglichen. Das Durchschnittsalter der Patienten betrug 34 Jahre und das Geschlechtsverhältnis weiblich: männlich war 9:1. In 70% der Fälle war die Mandibula, in 30%, die Maxilla betroffen, wobei eine Bevorzugung der linken Seite (70%) vorlag. Die Symptome dokumentierten sich in 40% der Fälle in einer Schwellung, in je 10% in Schmerzen und einer Parästhesie des N.mentalis. Das radiologische Erscheinungsbild ergab in 6 Fällen eine einkammrige, in 2 eine mehrkammrige gut begrenzte Läsion. Bei 2 Patienten lag eine Wurzelspitzenverlagerung vor, bei keinem Patienten kam es zu einer Wurzelresorption. Die Therapie umfasste bei 7 Patienten (70%) in eine Cürretage, bei 3 Patienten (30%) eine en block Resektion. Während einer durchschnittlichen Nachuntersuchungszeit von 34,8 Monaten trat bei keinem einzigen Patienten ein Rezidiv auf. The cysts came from an unselect population an concerned each 13 nin HIV-infected males and females. Age range was 2-81 years (mean 53, median 55.5). All cysts arose unilateral with the right parotid gland being preferred. 10 cases appeared as unicystic, 16 as multicystic lesions. Preoperative fine needle biopsy for cytodiagnostic purposes was performed in 14 cases, whereas preoperative clinical imaging including sialography, sonography, CT and MRI was applied in 12 patients. Recurrences were seen in two mulitcystic cases 2 and 12 years, and 1 year respectively after first operation. Macroscopy revealed cystic spaces of 2 to 40 mm in diameter with an either smooth or granulated inner surface, and were filled by a serous to coseous material. Cytologically, the material obtained was usually characterized by a mixture of lymphoid cells and macrophages set in an inflammatory proteinaceous background. Only in single cases epithel cells in good condition corresponding either to metaplastic of proper squamous epithelium were present. Histological examination exhibited cysts with an inner lining by a multilayered or rarely squamous epithelium, which was peripherally surrounded by lymphoid tissue with follicle- and germinal center formation. In some cases goblet or oncocytic cells appeared within the lining epithelium, and duct-like, sometimes ecstatic structures could be found within the lymphoid component. Immunohistochemistry using different CD-antibodies proved the non-neoplastic character of the lymphoid tissue with the lymphocytes mostly being of B-lineage, whereas cytokeratins highlighted the epithelial lining and its infolding to the cyst wall. Contrary to the literature this study is the first to present a predominance of a multicystic architecture of parotid lymphoepithelial cysts in an unselected, non HIV-infected population. Moreover, the histopathological findings obtained support impressively the theory that these sporadic cysts arise from heterotopic sialogenic duct like structures in intra- and periparotid lymph nodes.
