Im Rahmen des Wiederaufbaus der Brust hat das Lipofilling in den letzten Jahren zunehmend Anwendung gefunden. In dieser Arbeit soll dieses Verfahren hinsichtlich der onkologischen Sicherheit und Komplikationen, dem ästhetischen Outcome und der daraus resultierenden Lebensqualität untersucht werden.\r\n\r\nMethoden: Von 53 Patientinnen, die im Zeitraum von 2013 bis 2018 an der Abteilung für Plastische, Ästhetische und Rekonstruktive Chirurgie und der Universitätsklinik für Frauenheilkunde und Geburtshilfe Graz mit Lipofilling behandelt wurden, wurden retrospektiv Daten zum Vorliegen einer familiären Belastung, zu den Tumoreigenschaften, zum therapeutischen Prozedere, zur Anzahl der Lipofilling-Anwendungen und der jeweils transferierten Fettmenge, zum Patientinnenalter und zu Komplikationen und Mammografieergebnissen bei Zustand nach Lipofilling erhoben. Für die Evaluierung der Lebensqualität wurden zwei Fragebögen (EORTC QLQ C30 und BRECON23) ausgesendet. Das ästhetische Outcome wurde anhand prä- und postoperativer Bildaufnahmen durch eine 4-köpfige Expertenjury unter Zuhilfenahme der Harvard Breast Cosmesis Scale und unabhängig davon mit der Computersoftware Breast Analyzing Tool (BAT) ermittelt.\r\n\r\nErgebnisse: 53 Studienteilnehmerinnen (70 Brüste) wurden therapiert: 37 hatten ein invasives Karzinom (in 3 Fällen bilateral), 7 ein DCIS und 11 Patientinnen wurden prophylaktisch mastektomiert. Nach einem mittleren Follow-up von 30,4 Monaten konnte ein Rezidiv (Mb. Paget) diagnostiziert werden. Insgesamt wurden zwischen 1 und 6 Lipofillingsitzungen durchgeführt, mit einem Injektionsvolumen von durchschnittlich 141ml (± 101ml). Komplikationen traten bei 7 Patientinnen (13%) auf und verteilten sich auf 3 Hämatome, 1 Ölzyste, 2 Fettgewebsnekrosen und 1 Entzündung. Die beiden Fragebögen wurden von 35 Studienteilnehmerinnen retourniert und ergaben eine subjektive Bewertung der Lebensqualität im EORTC QLQ C30 von durchschnittlich 5,6 von bestenfalls 7 Punkten. Der EORTC QLQ BRECON23 zeigte, dass Tumorpatientinnen im Vergleich zu Patientinnen nach prophylaktischer Mastektomie signifikant mehr chirurgische Nebenwirkungen aufwiesen. Bezüglich des ästhetischen Outcomes zeigten sich die Patientinnen subjektiv zufriedener als objektiv mittels Expertenjury und BAT erfasst.
